Kognitivní trénink pro kognitivní poruchy související s rakovinou u žen, které přežily rakovinu prsu: multicentrická randomizovaná dvojitě zaslepená kontrolovaná studie

Tato studie fáze III bude zkoumat účinnost počítačového kognitivního tréninku (Brain HQ) ve srovnání s kontrolou pozornosti u vnímané kognitivní poruchy po intervenci, měřené pomocí FACT-Cog PCI stupnice.

Cílem této studie je určit účinnost pokročilého kognitivního tréninku u pacientů, kteří přežili rakovinu, kteří trpí kognitivní dysfunkcí související s rakovinou a léčbou rakoviny. Pro miliony lidí, kteří přežili rakovinu, je kognitivní dysfunkce převládajícím, závažným a přetrvávajícím problémem, který je dlouho spojován se špatnými výsledky souvisejícími s prací a zdravím. Důkazy naznačují, že významná podskupina pacientů, kteří přežili rakovinu prsu (BCS), trpí kognitivními změnami, které mohou přetrvávat roky po léčbě. Bohužel vědecký základ pro zvládnutí těchto kognitivních změn je extrémně omezený. Dostupné důkazy z pilotních studií, včetně naší práce, naznačují, že pokročilý kognitivní trénink, který je založen na principech neuroplasticity (schopnost mozkových neuronů reorganizovat se a vytvářet nové neuronové sítě), může být životaschopnou možností léčby. Dosavadní předchozí studie však byly omezeny nedostatkem návrhů kontrolovaných pozorností, malými vzorky BCS nebo omezenými výsledky. Proto, abychom překonali omezení minulých studií a stavěli na našich pilotních výsledcích, účelem této 2-skupinové, dvojitě zaslepené, randomizované kontrolované studie je provést plnohodnotnou studii účinnosti s cílem porovnat pokročilý kognitivní trénink s kontrolou pozornosti u BCS.

  • Účastník musí poskytnout informovaný souhlas specifický pro studii před jakýmikoli postupy specifickými pro studii a povolením umožňujícím zveřejnění osobních zdravotních informací.
  • Účastník musí mít poprvé diagnózu nemetastatického karcinomu prsu, což je stadium I-III.
  • Účastník musí mít skóre nižší než 12 na PROMIS Adult v2.0 – kognitivní funkce 4a.
  • Účastníci s více než nebo rovnými 6 měsíci až 5 let po léčbě (dokončení počátečního chirurgického zákroku +/- adjuvantní chemoterapie/radiační terapie), kromě toho, že mohou stále užívat endokrinní terapii nebo adjuvantní terapii zaměřenou na HER2.
  • Účastník musí být schopen porozumět, mluvit, číst a psát v angličtině nebo španělštině.

Kritéria vyloučení:

  • Skóre menší nebo rovné 3 na kognitivní obrazovce se 6 položkami.
  • Skóre položky dotazníku o zdraví pacienta-2 (PHQ-2) větší nebo rovné 3.
  • Definitivní klinický nebo radiologický důkaz metastatického onemocnění.
  • Předchozí nebo současná jiná rakovina, s výjimkou nemelanomové rakoviny kůže nebo in situ rakoviny děložního čípku během posledních 5 let.
  • Předchozí vystavení chemoterapii jiné rakoviny nebo kvůli jinému zdravotnímu stavu (např. expozice methotrexátu při léčbě revmatoidní artritidy).
  • Předchozí ozařování centrálního nervového systému (CNS), intratekální terapie nebo operace zahrnující CNS.
  • Účastníci s anamnézou mrtvice, traumatického poranění mozku, operace mozku, Alzheimerovy choroby nebo jiné demence.
  • Účastníci se zneužíváním účinných látek a/nebo v léčbě zneužívání návykových látek nebo s bipolární poruchou, psychózou, schizofrenií, ADHD nebo poruchou učení v anamnéze.

Aktivní komparátor: Arm 1: Počítačový kognitivní trénink – globální stimulační hry
Globální stimulační hry aktivní intervence kontroly pozornosti se skládají z počítačových her. Tyto hry jsou plně vyvinuty a jsou hodnoceny E (pro každého) od Entertainment Software Rating Board. Program představí 8 nezrychlených cvičení, která zahrnují vedlejší hry jako lámání hexu, sestava čtyři, bitevní loď, výměna drahokamů, dvojitý klondike, solitaire, reversi, hledání slov a sudoku.
Behaviorální: Arm 1: Počítačový kognitivní trénink – globální stimulační hry
Program bude obsahovat 8 nezrychlených cvičení, která zahrnují vedlejší hry jako je lámání hexu, sestava čtyři, bitevní loď, výměna drahokamů, dvojitý klondike, solitaire, reversi, hledání slov a sudoku. Tyto hry jsou dobře sladěny z hlediska času na úkol, novinek a postupu napříč úrovněmi. Jedná se o strategické hry bez adaptivních, zrychlených nebo přesných algoritmů, které jsou základem jejich personalizace.
Experimentální: Rameno 2: Počítačové kognitivní tréninkové hry s neuroplasticitou
Hry s neuroplasticitou poskytují základní prvky nezbytné pro vyvolání neuroplasticity. Mezi pět základních prvků patří principy rychlosti zpracování, přesnosti zpracování, adaptability, generalizovatelnosti a zapojení. Důležité je, že všechna cvičení se přizpůsobují dovednostem a schopnostem uživatele. Behaviorální sledování zabudované v programu se používá k monitorování individuálního výkonu a zajištění toho, že osoba trénuje na své nejvyšší prahové úrovni. Konkrétně použijeme osm z těchto cvičení, která jsou navržena tak, aby řešila kognitivní problémy, které jsou v BCS nejvíce zaznamenány, včetně cvičení na zlepšení pozornosti a pracovní paměti, rychlosti zpracování a výkonných funkcí.

Cvičení kognitivního tréninku zahrnují Double Decision (cílí na vizuální rychlost zpracování a užitečné zorné pole), Target Tracker (cílí na kapacitu visuoprostorové pracovní paměti), Visual Sweeps (cílí na rychlost zpracování), Syllable Stacks (cílí na sluchovou paměť), Eye for Detail ( se zaměřuje na rychlost pohybu očí a vizuální prostorovou pracovní paměť), Mind Bender (cílí na mentální flexibilitu), Divided Attention (cvičení inhibiční kontroly tvaru-barevného vzoru, které se zaměřuje na rychlé určení, zda blikající barvy, tvary nebo vzory splňují předem specifikované pravidlo), Pravé odbočení (cílí na prostorové uvažování).
