Målet med denne studien er å bestemme effektiviteten av avansert kognitiv trening for kreftoverlevere som lider av kreft- og kreftbehandlingsrelatert kognitiv dysfunksjon. For millioner av kreftoverlevere er kognitiv dysfunksjon et utbredt, alvorlig og vedvarende problem som lenge har vært assosiert med dårlige arbeidsrelaterte og helserelaterte utfall. Bevis tyder på at en betydelig undergruppe av brystkreftoverlevere (BCS) pådrar seg kognitive endringer som kan vedvare i årevis etter behandling. Dessverre er det vitenskapelige grunnlaget for å håndtere disse kognitive endringene ekstremt begrenset. Tilgjengelig bevis fra pilotstudier, inkludert vårt arbeid, tyder på at avansert kognitiv trening, som er basert på prinsippene om nevroplastisitet (evnen til hjerneneuroner til å reorganisere og danne nye nevrale nettverk), kan være et levedyktig behandlingsalternativ. Inklusjonskriterier:

  • Deltakeren må gi studiespesifikk informert samtykke før eventuelle studiespesifikke prosedyrer og autorisasjon som tillater utlevering av personlig helseinformasjon.
  • Deltakeren må ha en førstegangsdiagnose for ikke-metastatisk brystkreft som er stadium I-III.
  • Deltakeren må ha en poengsum på mindre enn 12 på PROMIS Adult v2.0 - Kognitiv funksjon 4a.
  • Deltakere med mer enn eller lik 6 måneder til 5 år etter behandling (fullføring av innledende kirurgi +/- adjuvant kjemoterapi/strålebehandling), bortsett fra at de fortsatt tar endokrin terapi eller HER2-rettet adjuvant terapi.
  • Deltakeren må kunne forstå, snakke, lese og skrive på engelsk eller spansk.

Ekskluderingskriterier:

  • Score mindre enn eller lik 3 på den kognitive skjermen med 6 elementer.
  • Patient Health Questionnaire-2 element (PHQ-2) score større enn eller lik 3.
  • Definitiv klinisk eller radiologisk bevis på metastatisk sykdom.
  • Tidligere eller nåværende annen kreft i anamnesen, med unntak av ikke-melanom hudkreft eller in situ livmorhalskreft i løpet av de siste 5 årene.
  • Tidligere eksponering for cellegiftbehandling for en annen kreftsykdom eller på grunn av annen medisinsk tilstand (f. metotreksateksponering for behandling av revmatoid artritt).
  • Tidligere stråling fra sentralnervesystemet (CNS), intratekal terapi eller CNS-involvert kirurgi.

Aktiv komparator: Arm 1: Datastyrt kognitiv trening-Globale stimuleringsspill
De globale stimuleringsspillene aktiv oppmerksomhetskontrollintervensjon er sammensatt av datastyrte spill. Disse spillene er fullt utviklet og er vurdert E (for alle) av Entertainment Software Rating Board. Programmet vil presentere 8 ikke-hastighetsøvelser som inkluderer spin-offs av slike spill som breaking hex, lineup four, battleship, gems swap, double klondike, solitaire, reversi, ordsøk og sudoku.
Atferdsmessig: Arm 1: Datastyrt kognitiv trening-Globale stimuleringsspill
Programmet vil inneholde 8 ikke-hastighetsøvelser som inkluderer spin-offs av slike spill som breaking hex, lineup four, battleship, gems swap, double klondike, solitaire, reversi, ordsøk og sudoku. Disse spillene er godt tilpasset tid på oppgave, nyhet og progresjon på tvers av nivåer. Dette er strategibaserte spill uten adaptive, hastighetsbaserte eller nøyaktighetsbaserte algoritmer som ligger til grunn for deres personalisering.
Eksperimentell: Arm 2: Datastyrt kognitiv trening-nevroplastisitetsspill
Nevroplastisitetsspillene gir kjerneelementer som er nødvendige for å indusere nevroplastisitet. De fem kjerneelementene inkluderer prinsippene om prosesseringshastighet, nøyaktighet av prosessering, tilpasningsevne, generaliserbarhet og engasjement. Viktigere er at alle øvelsene tilpasser seg brukerens ferdigheter og evner. Atferdssporing bygget i programmet brukes til å overvåke individuell ytelse og sikre at personen trener på sitt øverste terskelnivå. Spesifikt vil vi bruke åtte av disse øvelsene som er designet for å adressere kognitive bekymringer som er mest kjent i BCS, inkludert øvelser for å forbedre oppmerksomhet og arbeidsminne, prosesseringshastighet og eksekutiv funksjon.
Atferdsmessig: Arm 2: Datastyrt kognitiv trening-nevroplastisitetsspill
Kognitive treningsøvelser inkluderer Double Decision (målretter visuell prosesseringshastighet og nyttig synsfelt), Target Tracker (målretter visuospatial arbeidsminnekapasitet), Visual Sweeps (målretter prosesseringshastighet), Syllable Stacks (målretter auditivt minne), Eye for Detail ( målretter øyebevegelseshastighet og visuo-spatialt arbeidsminne), Mind Bender (målretter mental fleksibilitet), Divided Attention (en form-farge-mønster-hemmende kontrolløvelse som fokuserer på raskt å bestemme om blinkende farger, former eller mønstre oppfyller en forhåndsspesifisert regel), Høyresving (mål mot romlig resonnement).
