Et randomiseret dobbeltblindet fase III-studie, der sammenligner gemcitabin, cisplatin og bevacizumab med gemcitabin, cisplatin og placebo hos patienter med avanceret overgangscellekarcinom
Dette randomiserede fase III-studie undersøger gemcitabinhydrochlorid, cisplatin og bevacizumab for at se, hvor godt de virker sammenlignet med gemcitabinhydrochlorid og cisplatin ved behandling af patienter med urinvejskræft, der har spredt sig til andre steder i kroppen.
Lægemidler, der bruges i kemoterapi, såsom gemcitabinhydrochlorid og cisplatin, virker på forskellige måder for at stoppe væksten af tumorceller, enten ved at dræbe cellerne, ved at stoppe dem i at dele sig eller ved at stoppe dem i at sprede sig.
Immunterapi med bevacizumab kan inducere ændringer i kroppens immunsystem og kan interferere med tumorcellers evne til at vokse og sprede sig.
Det vides endnu ikke, om gemcitabinhydrochlorid og cisplatin er mere effektive, når de gives med eller uden bevacizumab til behandling af patienter med urinvejskræft.

Detaljeret beskrivelse
PRIMÆRE MÅL:
I. For at bestemme, om patienter med fremskreden overgangscellekarcinom behandlet med bevacizumab, gemcitabinhydrochlorid (gemcitabin) og cisplatin vil have øget samlet overlevelse sammenlignet med patienter behandlet med gemcitabin, cisplatin og placebo.
SEKUNDÆRE MÅL:
I. At sammenligne den progressionsfrie overlevelse af disse to regimer hos patienter med fremskreden overgangscellekarcinom.
II. At sammenligne andelen af patienter, der oplever en objektiv respons på hvert regime.
III. At sammenligne grad 3 og større toksicitet hos patienter behandlet på de to regimer.
OVERSIGT: Patienterne randomiseres til 1 ud af 2 arme.
ARM I: Patienter får gemcitabinhydrochlorid intravenøst (IV) over 30 minutter på dag 1 og 8, cisplatin IV og placebo IV over 30-90 minutter på dag 1. Behandlingen gentages hver 21. dag i 6 cyklusser i fravær af sygdomsprogression eller uacceptabel toksicitet. Patienterne får derefter placebo IV over 30-90 minutter hver 21. dag i fravær af sygdomsprogression eller uacceptabel toksicitet.
ARM II: Patienterne får gemcitabinhydrochlorid og cisplatin som i arm I. Patienterne får også bevacizumab IV over 30-90 minutter på dag 1. Behandlingen gentages hver 21. dag i 6 cyklusser i fravær af sygdomsprogression eller uacceptabel toksicitet. Patienterne får derefter bevacizumab IV over 30-90 minutter hver 21. dag i fravær af sygdomsprogression eller uacceptabel toksicitet.
Efter afsluttet undersøgelsesbehandling følges patienterne periodisk i op til 7 år.

Inklusionskriterier:
Patienter skal have histologisk eller cytologisk dokumenteret metastatisk eller inoperabel overgangscellekarcinom (urothelial) i urinvejene (nyrebækken, urinleder, blære, prostata eller urinrør) med metastatisk eller lokalt fremskreden sygdom (T4b, N2, N3 eller M1) ; patienter må ikke være kandidater til potentielt helbredende kirurgi eller strålebehandling
- For patienter, der har fået foretaget kirurgisk resektion før studieindskrivningen, skal resterende eller ikke-reseceret sygdom (målbar og/eller ikke-målbar) være tydelig på post-kirurgiske scanninger
Tidligere behandling for overgangscellekarcinom (TCC)
- Patienter har muligvis ikke modtaget systemisk kombinationskemoterapi for metastatisk sygdom
- I denne undersøgelses formål betragtes radiosensibiliserende enkeltstof-kemoterapi ikke som forudgående systemisk terapi
- Forudgående neoadjuverende eller adjuverende systemisk kemoterapi er tilladt, forudsat at intervallet fra slutningen af behandlingen til diagnosen metastatisk sygdom er mindst 1 år
- >= 4 uger siden nogen tidligere stråling (inklusive palliativ) eller større operation og fuldt restitueret

Patienterne får gemcitabinhydrochlorid IV over 30 minutter på dag 1 og 8 og cisplatin IV og placebo IV over 30-90 minutter på dag 1.
Behandlingen gentages hver 21. dag i 6 cyklusser i fravær af sygdomsprogression eller uacceptabel toksicitet.

Samlet overlevelsestid er defineret som tiden fra randomisering til død på grund af enhver årsag.
Medianen og 95 % konfidensintervaller estimeres ved hjælp af Kaplan-Meier-estimatoren. Den stratificerede log-rang-statistik vil være den primære analyse til at sammenligne de to behandlingsarme på OS med stratificeringsfaktorerne: tilstedeværelse af viscerale metastaser (nej, ja) og forudgående kemoterapi (nej, ja).
Derudover vil proportional hazards-modellen blive brugt til at vurdere vigtigheden af, at behandlingsarmen justerer på patientkarakteristika, stratifikationsvariabler og andre vigtige kovariater til forudsigelse af OS.

Den primære analyse af PFS vil være en tosidet stratificeret log-rank test, der sammenligner arm A og arm B. Stratifikationsfaktorerne vil bestå af de to stratifikationsfaktorer, der bruges til patientrandomisering: forudgående nefrektomi (ja vs. nej) og Motzer score ( 0 vs. 1-2 vs. 3+).
Resultater fra ustratificerede log-rank tests vil også blive leveret.
Kaplan-Meier metodologi vil blive brugt til at estimere median PFS for hver behandlingsarm.

Objektiv respons er defineret som bekræftede fuldstændige og delvise responser ved brug af kriterier for responsevaluering i solide tumorer. En komplet respons (CR) er defineret som en forsvinden af alle mållæsioner. En partiel respons (PR) er defineret som at have mindst 30 % reduktion i summen af den længste diameter af mållæsioner, idet der tages udgangspunkt i baseline-summen. I hver arm blev antallet af patienter, der rapporterede en CR eller PR, divideret med antallet af patienter, der kunne evalueres i hver arm for at opnå andelen. |
