En randomisert dobbeltblindet fase III-studie som sammenligner gemcitabin, cisplatin og bevacizumab med gemcitabin, cisplatin og placebo hos pasienter med avansert overgangscellekarsinom
Denne randomiserte fase III-studien studerer gemcitabinhydroklorid, cisplatin og bevacizumab for å se hvor godt de virker sammenlignet med gemcitabinhydroklorid og cisplatin ved behandling av pasienter med urinveiskreft som har spredt seg til andre steder i kroppen.
Legemidler som brukes i kjemoterapi, som gemcitabinhydroklorid og cisplatin, virker på forskjellige måter for å stoppe veksten av tumorceller, enten ved å drepe cellene, ved å stoppe dem fra å dele seg eller ved å hindre dem i å spre seg.
Immunterapi med bevacizumab kan indusere endringer i kroppens immunsystem og kan forstyrre tumorcellenes evne til å vokse og spre seg.
Det er ennå ikke kjent om gemcitabinhydroklorid og cisplatin er mer effektive når de gis med eller uten bevacizumab ved behandling av pasienter med urinveiskreft.

Detaljert beskrivelse
PRIMÆRE MÅL:
I. For å bestemme om pasienter med avansert overgangscellekarsinom behandlet med bevacizumab, gemcitabinhydroklorid (gemcitabin) og cisplatin vil ha økt total overlevelse sammenlignet med pasienter behandlet med gemcitabin, cisplatin og placebo.
SEKUNDÆRE MÅL:
I. Å sammenligne den progresjonsfrie overlevelsen til disse to regimene hos pasienter med avansert overgangscellekarsinom.
II. For å sammenligne andelen pasienter som opplever en objektiv respons på hvert regime.
III. For å sammenligne grad 3 og høyere toksisitet hos pasienter behandlet på de to regimene.
OVERSIGT: Pasienter randomiseres til 1 av 2 armer.
ARM I: Pasienter får gemcitabinhydroklorid intravenøst (IV) over 30 minutter på dag 1 og 8, cisplatin IV og placebo IV over 30-90 minutter på dag 1. Behandlingen gjentas hver 21. dag i 6 sykluser i fravær av sykdomsprogresjon eller uakseptabel toksisitet. Pasienter får deretter placebo IV over 30-90 minutter hver 21. dag i fravær av sykdomsprogresjon eller uakseptabel toksisitet.
ARM II: Pasienter får gemcitabinhydroklorid og cisplatin som i arm I. Pasienter får også bevacizumab IV over 30-90 minutter på dag 1. Behandlingen gjentas hver 21. dag i 6 sykluser i fravær av sykdomsprogresjon eller uakseptabel toksisitet. Pasienter får deretter bevacizumab IV over 30-90 minutter hver 21. dag i fravær av sykdomsprogresjon eller uakseptabel toksisitet.
Etter avsluttet studiebehandling følges pasientene opp periodisk i inntil 7 år.

Inklusjonskriterier:
Pasienter må ha histologisk eller cytologisk dokumentert metastatisk eller ikke-opererbart overgangscellekarsinom (urotelialt) i urinveiene (nyrebekken, urinleder, blære, prostata eller urinrør), med metastatisk eller lokalt avansert sykdom (T4b, N2, N3 eller M1) ; Pasienter må ikke være kandidater for potensielt kurativ kirurgi eller strålebehandling
- For pasienter som har hatt kirurgisk reseksjon før studieregistrering, må gjenværende eller ureseksjonert sykdom (målbar og/eller ikke-målbar) være tydelig på post-kirurgiske skanninger
Tidligere behandling for overgangscellekarsinom (TCC)
- Pasienter kan ikke ha mottatt systemisk kombinasjonskjemoterapi for metastatisk sykdom
- For formålet med denne studien regnes ikke radiosensibiliserende kjemoterapi med enkeltmiddel som tidligere systemisk terapi
- Tidligere neoadjuvant eller adjuvant systemisk kjemoterapi er tillatt forutsatt at intervallet fra avsluttet behandling til diagnose av metastatisk sykdom er minst 1 år

Samlet overlevelsestid er definert som tiden fra randomisering til død på grunn av en hvilken som helst årsak.
Median- og 95 % konfidensintervall er estimert ved å bruke Kaplan-Meier-estimatoren. Den primære analysen av PFS vil være en tosidig stratifisert log-rank test som sammenligner arm A og arm B. Stratifiseringsfaktorene vil bestå av de to stratifiseringsfaktorene som brukes for pasientrandomisering: tidligere nefrektomi (ja vs. nei) og Motzer score ( 0 vs. 1-2 vs. 3+).
Resultater fra ustratifiserte log-rank tester vil også bli gitt.
Kaplan-Meier metodikk vil bli brukt for å estimere median PFS for hver behandlingsarm.

Objektiv respons

Objektiv respons er definert som bekreftede komplette og delvise responser ved bruk av responsevalueringskriterier i solide svulster. En fullstendig respons (CR) er definert som en forsvinning av alle mållesjoner. En partiell respons (PR) er definert som å ha minst 30 % reduksjon i summen av den lengste diameteren til mållesjonene med utgangspunkt i summen. I hver arm ble antallet pasienter som rapporterte en CR eller PR delt på antall pasienter som kunne evalueres i hver arm for å få andelen. |
