  • Patienter skal have metastatisk og/eller recidiverende (fjern eller lokalt tilbagevendende) brystkræft og være HER2 ikke-overudtrykkende i henhold til 2013 American Society of Clinical Oncology (ASCO)-College of American Pathologists (CAP) retningslinjer for HER-test (0 eller 1+ pr. immunhistokemi [IHC] og/eller HER2-forhold < 2,0 og HER2-kopital < 4 signaler/celle ved in-situ hybridisering [ISH])

    • Lokal Regional Gentagelse

      • I brystet (efter konserveringsbehandling)
      • I brystvæggen (efter mastektomi)

  • Patienter med kendte hjernemetastaser skal enten opfylde de yderligere kriterier og tilmeldes som en del af Progressive Brain Metastases Cohort eller have klinisk kontrollerede neurologiske symptomer, defineret som kirurgisk excision og/eller strålebehandling efterfulgt af 14 dages stabil neurologisk funktion før registrering; patienter med tidligere tilfældigt opdagede eller asymptomatisk(e) hjernemetastaser(er) skal modtage kirurgisk excision og/eller strålebehandling før registrering; patienter med progressive hjernemetastaser efter forudgående behandling er ikke berettigede til standardkohorten, men kan overvejes til progressive hjernemetastaser kohorte
  • Patienter skal have en fuldstændig anamnese og fysisk undersøgelse inden for 28 dage før registrering
  • Patienter i den fødedygtige alder må ikke være gravide (negativ graviditetstest) eller ammende på grund af muligheden for skade på et foster eller et ammende spædbarn fra dette behandlingsregime; mænd og kvinder med reproduktionspotentiale skal have accepteret at bruge en effektiv præventionsmetode i 6 måneder efter afslutningen af ​​undersøgelsesbehandlingen; en kvinde anses for at have "reproduktionspotentiale", hvis hun har haft menstruation på noget tidspunkt i de foregående 12 på hinanden følgende måneder; ud over rutinepræventionsmetoder omfatter "effektiv prævention" også heteroseksuel cølibat og kirurgi beregnet til at forhindre graviditet (eller med en bivirkning af graviditetsforebyggelse) defineret som en hysterektomi, bilateral oophorektomi eller bilateral tubal ligering; men hvis en tidligere cølibatpatient på noget tidspunkt vælger at blive heteroseksuelt aktiv i løbet af tidsperioden for brug af præventionsforanstaltninger, der er beskrevet i protokollen, er han/hun ansvarlig for at påbegynde præventionsforanstaltninger

  • Som en del af registreringsprocessen for Oncology Patient Enrollment Network (OPEN) oplyses den behandlende institutions identitet for at sikre, at den aktuelle (inden for 365 dage) dato for institutionsbedømmelsesudvalgets godkendelse af denne undersøgelse er blevet indtastet i systemet

  • Progressive hjernemetastaser-kohorte

    • S1416 er ét studie med to kohorter; patienter, der har progressive hjernemetastaser efter kirurgisk excision og/eller intrakraniel stråling, vil være i Progressive Brain Metastases Cohort og vil kræve en baseline magnetisk resonansbilleddannelse (MRI); patienter med tidligere behandlede hjernemetastaser, stabil sygdom og stabil neurologisk funktion i 14 dage før forsøgsregistrering vil være i standardkohorten og kan få MRI af hjernen efter lægens skøn; randomisering og behandling er den samme for begge kohorter

    • Ud over alle de tidligere berettigelseskriterier skal patienter med progressive hjernemetastaser, som ikke opfylder betingelserne for at tilmelde sig standardkohorten (neurologisk stabilitet i 14 dage efter operation og/eller strålebehandling), også opfylde følgende kriterier for at blive tilmeldt som del af hjernemetastase-kohorten:

      • Patienter med progressive hjernemetastaser skal have en baseline hjerne-MR inden for 28 dage før registrering; hjernemetastaser skal være progressive og >= 10 mm i længste dimension på røntgenbilleder EFTER forudgående intrakraniel strålebehandling (IR) (dvs. strålebehandling af hele hjernen [WBRT], stereotaktisk strålekirurgi [SRS], gammakniv [GK] eller lokal ækvivalent) ; patienter må ikke have tegn på diffus leptomeningeal sygdom på hjerne-MR eller ved tidligere dokumenteret cerebrospinalvæske (CSF) cytologi; diskrete durale metastaser er tilladt; der må ikke være tegn på blødning eller forestående herniation ved baseline-hjernebilleddannelse; patienter med kontraindikation for gadolinium-forstærket MR-billeddannelse er ikke kvalificerede
      • Patienter skal have en stabil eller faldende dosis af steroider i >= 7 dage før registrering
      • Hvis patienten fik en åben hjernebiopsi, skal der være gået mindst 28 dage mellem biopsi og registrering
      • Patienter, der tilmelder sig Progressive Brain Metastases Cohort, kan have modtaget op til 3 tidligere linjer med cytotoksisk kemoterapi for metastatisk sygdom; bemærk, at for at blive optaget i standardkohorten skal patienter have haft =< 1 tidligere cytotoksisk regime for metastatisk sygdom

Ekskluderingskriterier:

  • Patienter må ikke tidligere have modtaget cisplatin eller poly (adenosin diphosphat [ADP]-ribose) polymerase (PARP) hæmmere; tidligere carboplatin i adjuverende/neoadjuverende omgivelser er tilladt, hvis afsluttet mere end 12 måneder før studiestart
  • Patienter må ikke have modtaget kemoterapi inden for 14 dage før registrering
  • Patienter må ikke have modtaget nogen immunterapi, biologisk eller andet forsøgslægemiddel inden for 28 dage før registrering; patienter må ikke have modtaget bevacizumab inden for 42 dage før registrering
  • Patienter må ikke have en klinisk relevant hørenedsættelse >= grad 2
  • Patienter må ikke have behandlingsrelateret akut myeloid leukæmi (AML) (t-AML)/myelodysplastisk syndrom (MDS) eller træk, der tyder på AML/MDS
  • Patienter må ikke tidligere have haft allogen knoglemarvstransplantation eller dobbelt navlestrengsblodtransplantation
