PERUSTELUT: Kemoprevention terapiassa käytetään tiettyjä lääkkeitä, joilla pyritään estämään syövän kehittyminen tai uusiutuminen. Vielä ei tiedetä, estääkö seleeni tehokkaasti uusien kasvainten kasvua potilailla, joilla on aiemmin leikattu ei-pienisoluinen keuhkosyöpä.

TARKOITUS: Tässä satunnaistetussa vaiheen III tutkimuksessa seleeniä tutkitaan sen selvittämiseksi, kuinka hyvin se toimii lumelääkkeeseen verrattuna estämään toisten primääristen keuhkokasvainten kehittymistä potilailla, joille on tehty vaiheen I ei-pienisoluisen keuhkosyövän poistamiseksi leikkaus.

TAVOITTEET:

Ensisijainen tavoite:

  • Seleenin tehokkuuden vähentäminen toisten primaaristen keuhkokasvainten esiintyvyyden vähentämisessä osallistujilla, joilla on aiemmin leikattu vaiheen I ei-pienisoluinen keuhkosyöpä.

Toissijaiset tavoitteet:

  • Arvioi seleenin laadullinen ja kvantitatiivinen toksisuus näillä potilailla.
  • Vertaa tiettyjen syöpien ilmaantuvuutta, syöpäkuolleisuutta ja seleenihoitoa saaneiden potilaiden kokonaiseloonjäämistä lumelääkettä saaneisiin.

OUTLINE: Tämä on satunnaistettu, kaksoissokkoutettu, lumekontrolloitu monikeskustutkimus. Osallistujat ositetaan tupakoinnin tilan mukaan (aktiivisesti tupakointi tai lopetettu alle vuosi sitten vs. lopetettu vähintään vuosi sitten vs. ei koskaan polttanut tai enintään 100 savuketta koskaan), sukupuolen sekä vaiheen ja aiemman terapian mukaan (vaihe IA vs. vaihe IB aikaisemmalla hoidolla vs. vaihe IB ilman aikaisempaa hoitoa). Osallistujat satunnaistetaan suhteessa 1:2 haaroihin I ja II.

  • Käsivarsi I: Osallistujat saavat oraalista hiivaa lumelääkettä kuten haarassa II.
  • Käsivarsi II: Osallistujat saavat oraalista seleenihiivaa päivittäin 6 kuukauden ajan. Hoito toistetaan 6 kuukauden välein 8 hoitojakson ajan yhteensä 4 vuoden ajan, mikäli myrkyllisyyttä ei voida hyväksyä.

Osallistujia seurataan vuosittain (12 kuukauden välein) 10 vuoden ajan.

ARVIOTETTU KERTYMINEN: Yhteensä 1 960 osallistujaa kertyy tähän tutkimukseen neljän vuoden sisällä.

  • Histologisesti vahvistettu, täysin resektoitu vaiheen IA (pT1, N0) tai IB (pT2, N0) ei-pieni keuhkosyöpä (paitsi karsinoidi)*

    • Vaiheen I keuhkosyövän hoidon päättyminen viimeisten 6–36 kuukauden aikana ja tällä hetkellä sairausvapaa

Progression-Free Survival (PFS) määriteltiin ajaksi satunnaistamisesta toiseen primaariseen keuhkosyöpään tai uusiutumiseen. Tapaukset, joissa ei ole tapahtumia, on sensuroitu, kun viimeksi tiedettiin elossa. Viiden vuoden PFS-asteen arvioimiseen käytettiin Kaplan-Meier-menetelmää.

On ratkaisevan tärkeää määrittää tarkka syöpä, onko kyseessä uusiutuminen vai onko kyseessä toinen ensisijainen sairaus. Kaikki kliinisesti ja/tai radiografisesti tunnistetut epäilyttävät leesiot varmistettiin histologisesti.
