BEGRUNDELSE: Kjemoprevensjonsterapi er bruk av visse medikamenter for å prøve å forhindre utvikling eller tilbakefall av kreft. Det er ennå ikke kjent om selen er effektivt for å forhindre vekst av nye svulster hos pasienter med tidligere resekert ikke-småcellet lungekreft.

Hovedmål:

Bestem effekten av selen når det gjelder å redusere forekomsten av andre primære lungetumorer hos deltakere med tidligere resekert stadium I ikke-småcellet lungekreft.

Sekundære mål:

Evaluer den kvalitative og kvantitative toksisiteten til selen hos disse pasientene.
Sammenlign forekomsten av spesifikke kreftformer, dødelighet av kreft, og total overlevelse av deltakere behandlet med selen kontra de behandlet med placebo.

OVERSIKT: Dette er en randomisert, dobbeltblindet, placebokontrollert, multisenterstudie. Deltakerne er stratifisert etter røykestatus (røyker aktivt eller sluttet for mindre enn 1 år siden vs. sluttet for minst 1 år siden vs. aldri røykt eller ikke mer enn 100 sigaretter noen gang), kjønn og stadium og tidligere terapi (stadium IA vs. stadium IB med tidligere terapi versus stadium IB uten tidligere terapi). Deltakerne blir randomisert i forholdet 1:2 til arm I og arm II.

Arm I: Deltakerne får oral gjærplacebo som i arm II.
Arm II: Deltakerne får oral selengjær daglig i 6 måneder. Behandlingen gjentas hver 6. måned i 8 kurer i totalt 4 år i fravær av uakseptabel toksisitet.

Deltakerne følges årlig (hver 12. måned) i 10 år.

Histologisk bekreftet, fullstendig resekert stadium IA (pT1, N0) eller IB (pT2, N0) ikke-liten lungekreft (unntatt karsinoid)*
- Fullført behandling for stadium I lungekreft i løpet av de siste 6 til 36 månedene og for tiden sykdomsfri
- Minst én mediastinal lymfeknute tatt ved reseksjon MERK: *Southwest Oncology Group (SWOG) og kreft og leukemi gruppe B (CALGB) pasienter må være T1, N0; CALGB-pasienter kan være T2, N0 forutsatt at sykdommen ble fullstendig resektert før 1. juni 2001 og deltakelse i CALGB 9633 ble nektet hvis tilbudt
18 år og over
Eastern Cooperative Oncology Group resultatstatus 0-1
Bilirubin ikke større enn øvre normalgrense (ULN)
Serumglutamin-oksaloeddiksyretransaminase (SGOT) eller serumglutamin-pyrodruesyretransaminase (SGPT) ikke høyere enn ULN
Tidligere mineral-, urte-, fytokjemiske eller vitamintilskudd tillatt
Samtidig ikke-selenholdig mineral-, urte-, fytokjemikalie- eller vitamintilskudd tillatt hvis tidsplan og tilskudd før studien forblir uendret

Ekskluderingskriterier:

Bevis på ny eller tilbakevendende lungekreft på røntgen av thorax i løpet av de siste 8 ukene
Synkrone lunge eller ikke-lunge lesjoner eller metastaser, selv om de er resektable
Historie med mer enn én primær lungekreft til enhver tid
Samtidig eller annen tidligere kreft innen de siste 5 årene unntatt lokalisert ikke-melanom hudkreft
