기본 목표:

I. 화학방사선 요법 +/- 아테졸리주맙으로 치료받은 LS-SCLC 환자의 전체 생존(OS)을 비교하기 위함.

2차 목표:

I. 화학방사선 +/- 아테졸리주맙으로 치료받은 제한기 소세포 폐암(LS-SCLC) 환자의 무진행 생존(PFS)을 비교하기 위함.

II. 화학방사선 +/- 아테졸리주맙으로 전체 반응률(ORR), 국소 조절률, 원격 전이 없는 생존율을 결정합니다.

III. 화학방사선요법과 아테졸리주맙의 면역매개 및 비면역매개 독성을 특성화합니다.

IV. 화학방사선 요법 +/- 아테졸리주맙을 받는 환자에 대해 FACT-TOI(Functional Assessment of Cancer Therapy-Trial Outcome Index)로 측정한 삶의 질을 비교합니다.

V. LS-SCLC 환자에 대한 화학방사선 +/- 아테졸리주맙의 5단계 EuroQol 5차원 설문지(EQ-5D-5L)의 점수를 사용하여 품질 조정 생존을 평가하기 위해.

VI. 화학방사선 요법 +/- 아테졸리주맙 후 환자 보고 결과 측정 정보 시스템(PROMIS)으로 측정한 피로를 특성화합니다.

VII. 혈액 기반 종양 돌연변이 부담(bTMB) 및 조직 기반 종양 돌연변이 부담(tTMB)과 임상 결과의 연관성을 결정합니다.

탐구 목표:

I. 향후 분석을 위해 기준선, 화학방사선요법 종료 후 1일 및 3개월에 생체 표본을 수집합니다.

II. 환자 보고 결과 - 부작용에 대한 일반 용어 기준(PRO-CTCAE)에 의해 측정된 환자 보고 증상 독성을 특성화합니다.

개요: 환자는 2개의 팔 중 1개로 무작위 배정됩니다.

  • 60일 이내 병리학적으로(조직학적 또는 세포학적으로) 입증된 제한기 소세포 폐암 진단(Tx기, T1-T4, N0-3, M0, American Joint Committee on Cancer[AJCC] 병기, 8판[Ed.]) 등록하기 전에
  • 환자는 백금/에토포시드 화학요법 사전 등록(연구 등록 전)의 1주기를 받아야 합니다. 연구 등록은 화학 요법의 사전 등록 주기의 1일부터 21일 이내에 이루어져야 합니다.
  • 환자는 백금/에토포시드 화학요법의 필수 사전 등록 주기 이전에 측정 가능한 질병이 있어야 합니다(고형 종양의 반응 평가 기준[RECIST], 버전 1.1에 따라).

  • 최소 스테이징 요구 사항은 다음과 같습니다.

    • 등록 전 30일 이내 병력/신체검사
    • 등록 전 60일 이내에 병기 결정을 위한 양전자 방출 단층촬영(PET)/컴퓨터 단층촬영(CT) 스캔

    • 등록 전 60일 이내에 CT 흉부 및 CT 복부와 IV 조영제(신장 기능에 근거하여 금기 사항이 없는 한); CT 복부 대신 IV 조영제를 사용하는 자기 공명 영상(MRI) 복부

      • 참고: 조영제 알레르기가 있는 경우 조영제 CT를 얻기 전에 기관 지침에 따라 사전 투약을 수행해야 합니다. 이에 대한 유일한 예외는 기록된 생명을 위협하는 알레르기입니다.

  • 6개월 이내에 감지할 수 없는 바이러스 부하로 효과적인 항레트로바이러스 요법을 받고 있는 인간 면역결핍 바이러스(HIV) 감염 환자는 이 시험에 적합합니다.

제외 기준:

  • 전이성 질환의 확실한 임상적 또는 방사선학적 증거
  • 소세포폐암의 확정적 외과적 절제술
  • 최소 2년 동안 질병이 없는 경우(유방, 구강 또는 자궁경부의 상피내암종은 모두 허용)
  • 등록 전 SCLC에 대한 이전 백금 기반 화학 요법의 1주기 초과; 다른 암에 대한 사전 화학 요법은 허용됩니다.
  • 이전의 모든 아테졸리주맙 또는 기타 면역요법제
  • 폐 또는 종격동에 대한 사전 방사선 요법으로 인해 방사선 요법 분야가 임상적으로 유의하게 중복됩니다. 연구 주임 조사자(PI)의 승인에 따라 표적 부피와 최소 중첩이 있는 유방암에 대한 사전 접선 필드가 허용됩니다.
  • 세포학적으로 양성인 흉막 또는 심낭액이 있는 환자는 자격이 없습니다.
  • 활동성, 알려지거나 의심되는 자가면역 질환. 환자는 백반증, 제1형 진성 당뇨병, 호르몬 대체만 필요한 자가면역 상태로 인한 잔여 갑상선 기능 저하증, 전신 치료가 필요하지 않은 건선 또는 외부 요인 없이 재발할 것으로 예상되지 않는 상태인 경우 등록이 허용됩니다.
  • 활동성 또는 이전에 기록된 염증성 장 질환(예: 크론병, 궤양성 대장염)
  • 동종 장기이식의 역사
  • 원발성 면역결핍의 병력

  • 다음과 같이 정의된 심각한 활동성 동반이환:

    • 활동성 바이러스성, 알코올성 또는 기타 간염, 간경화, 지방간 및 유전성 간 질환을 포함하여 임상적으로 유의한 간 질환이 알려진 경우
    • 시험약의 사용을 금하거나 결과 해석에 영향을 미치거나 환자를 치료 합병증의 고위험에 놓이게 할 수 있는 질병 또는 상태에 대해 합리적으로 의심되는 모든 기타 질병, 대사 기능 장애, 신체 검사 소견 또는 임상 실험실 소견
    • 활동성 결핵

    • 활동성 B형 간염(만성 또는 급성) 또는 C형 간염 감염. 간염 상태를 알 수 없는 경우 B/C형 간염 검사가 필요합니다.

      • 과거 또는 해결된 B형 간염 감염(B형 간염 표면 항원[HBsAg] 음성으로 정의됨) 검사, 양성 항-HBc(B형 간염 핵심 항원에 대한 항체) 및 음성 바이러스 데옥시리보핵산(DNA) 검사(단지 HBsAg가 양성으로 확인된 경우 획득)에 적합합니다.
      • C형 간염 바이러스(HCV) 항체에 대해 양성인 환자는 중합효소 연쇄 반응(PCR)이 HCV 리보핵산(RNA)에 대해 음성인 경우에만 적격입니다. (HCV RNA 검사는 HCV 항체 검사에서 양성인 환자에게 시행해야 합니다.)
    • 골수 이식 또는 만성 림프구성 백혈병(CLL) 병력과 같은 알려진 면역억제 질환
    • 만성 폐쇄성 폐질환(COPD)은 등록 시 일일 > 10mg 프레드니손 또는 이에 상응하는 용량의 만성 경구 스테로이드 요법을 필요로 합니다. 흡입용 코르티코스테로이드는 배제되지 않습니다.
    • 지난 3개월 이내에 입원을 요하는 불안정 협심증 및/또는 울혈성 심부전
    • 최근 3개월 이내 경벽성 심근경색
    • 임상적으로 유의한 간질성 폐질환
  • 연구 약물 투여 14일 이내에 코르티코스테로이드(> 10 mg 일일 프레드니손 등가물) 또는 기타 면역억제 약물을 사용한 전신 치료가 필요한 상태. 흡입 또는 국소 스테로이드 및 부신 대체 용량 > 10 mg 일일 프레드니손 등가물은 활동성 자가면역 질환이 없는 경우 허용됩니다.
  • 연구 치료 기간 동안 및 연구 약물의 마지막 투여 후 150일 동안 임신 또는 가임기 여성 및 성적으로 왕성하고 의학적으로 허용되는 형태의 피임법을 사용할 의사/능력이 없는 남성(Arm 2); 이 연구에 관련된 치료가 상당히 기형을 유발할 수 있기 때문에 이러한 배제가 필요합니다.
