기본 목표:

I. BRAF-V600 돌연변이를 가진 참가자 중 엔코라페닙 + 비니메티닙 + 니볼루맙의 삼중 조합 대 이필리무맙 + 니볼루맙의 이중 조합에 무작위 배정된 참가자 간의 고형 종양 반응 평가 기준(Response Evaluation Criteria in Solid Tumors, RECIST) 1.1에 따른 무진행 생존(PFS) 비교 뇌로 전이된 흑색종.

2차 목표:

I. 각 치료 부문에서 참가자의 전체 생존(OS)을 추정하기 위해. II. 각 치료군에서 RECIST 1.1에 따른 객관적 반응률(ORR)(확인 및 미확정, 완전 및 부분 반응) 추정.

III. 뇌 전이에 대한 수정된 RECIST(mRECIST)에 따라 확인 및 확인되지 않은 완전 및 부분 반응으로 정의되는 두개내 반응률(ICRR)을 추정합니다.

IV. 각 치료군에서 RECIST 1.1에 따른 반응 지속 시간 및 mRECIST에 따른 ICRR 지속 시간, 신경종양학(RANO)-뇌전이(BM) 반응 평가(및 면역 요법[i]RANO)에 따른 반응 지속 시간을 평가하기 위해.

개요: 환자는 2개의 팔 중 1개로 무작위 배정됩니다.

ARM I: 환자는 1-28일에 엔코라페닙을 1일 1회(QD) 경구로(PO), 1-28일에 1일 2회(BID) 비니메티닙 PO를, 1일에 니볼루맙을 정맥내(IV) 투여받습니다. 주기는 질병 진행이나 허용할 수 없는 독성이 없는 경우 28일마다 반복됩니다.

ARM II: 환자는 모든 주기의 1일에 니볼루맙 IV를 받고 주기 1-4의 1일에 30분에 걸쳐 이필리무맙 IV를 받습니다. 주기는 4주기 동안 21일마다 반복한 다음 질병 진행이나 허용할 수 없는 독성이 없는 경우 28일마다 반복합니다.

  • 참가자는 뇌로 전이된 조직학적 및 병리학적으로 확인된 흑색종이 있어야 합니다.
  • 모든 원발성(피부, 견봉/점막 등) 또는 알려지지 않은 기원이 허용됩니다. 단, 포도막 원발성 참가자는 자격이 없습니다.
  • 참가자는 CLIA(Clinical Laboratory Improvement Act) 인증 실험실에서 문서화한 BRAF-V600 돌연변이 흑색종이 있어야 합니다.
  • 모든 참가자는 등록 전 28일 이내에 뇌의 자기 공명 영상(MRI)을 가지고 있어야 하며 최소 한 번의 측정 가능한 뇌 전이 >= 0.5cm 크기(수정된 RECIST 1.1에 따라)가 있는 중추 신경계 전이가 있어야 합니다. 이전 방사선 치료 후 방사선 조사 또는 진행(치료 의사의 의견에 따름). 참여 사이트는 1.5mm 미만의 MRI 슬라이스 두께를 사용해야 하며 '최소' 뇌 전이 임상 시험을 위한 뇌종양 이미징 프로토콜(BTIP-BM) 준수 MRI 획득 프로토콜을 준수하는 것이 좋습니다. 머리의 컴퓨터 단층 촬영(CT)은 뇌 MRI를 대체할 수 없습니다.   • 심장 질환의 알려진 병력 또는 현재 증상이 있거나 심장 독성 물질을 사용한 치료 이력이 있는 참가자는 New York Heart Association Functional Classification을 사용하여 심장 기능의 임상적 위험 평가를 받아야 합니다. 이 시험에 참가하려면 참가자는 클래스 2B 이상이어야 합니다.
  • 자연경과 또는 치료(치료 의사의 의견에 따름)가 조사 요법의 안전성 또는 효능 평가를 방해할 가능성이 없는 이전 또는 동시 악성 종양이 있는 참가자는 이 시험에 참가할 수 있습니다.
  • 인간 면역결핍 바이러스(HIV) 감염이 알려진 참가자는 효과적인 항레트로바이러스 요법을 받고 있고 가장 최근의 바이러스 부하 테스트에서 그리고 무작위 배정 전 90일 이내에 바이러스 부하가 감지되지 않는 경우 자격이 있습니다.
  • C형 간염 바이러스(HCV) 감염 병력이 있는 참여자는 치료를 받고 치료를 받아야 합니다. 현재 치료 중인 HCV 감염 참가자는 무작위배정 전에 검출할 수 없는 HCV 바이러스 부하가 있어야 합니다.

  • 참가자는 이 연구의 연구 특성에 대해 통보를 받아야 하며 기관 및 연방 지침에 따라 서명하고 정보에 입각한 동의를 제공해야 합니다.
  • OPEN 등록 프로세스의 일부로 치료 기관의 신원이 제공되어 현재(365일 이내) 본 연구에 대한 기관 검토 위원회 승인 날짜가 시스템에 입력되었는지 확인합니다.

제외 기준:

  • 참여자는 이전에 전이성 질환에 대한 전신 요법을 받은 적이 없어야 합니다. 선행 요법 및/또는 보조 요법(예: BRAF/MEK 억제제 요법, 항-PD-1 요법 또는 항-CTLA4 요법, 알파-인터페론 등)에서만 받은 이전의 전신 요법은 허용됩니다. 환자가 이전에 신보조/보조 요법을 받은 경우 무작위 배정 전에 궁극적인 질병 재발이 있어야 합니다.
  • 참여자는 무작위 배정 전 7일 이내에 사전 방사선 요법을 받은 적이 없어야 합니다.
  • 참가자는 프로토콜 치료를 받는 동안 흑색종에 대한 전신 항종양 요법의 추가 형태를 요구할 계획이 없어야 합니다.
  • 참가자는 호르몬 피임약을 사용할 계획이 없어야 합니다.
  • 참가자는 치료 의사의 의견에 따라 무작위 배정 시점에 전신 요법이 필요한 심각한 활동성 감염이 없어야 합니다.
  • 참가자는 지난 6개월 동안 생물학적 질병 조절제(예: 인플릭시맙, 아달리무맙). 비생물학적 질병 조절제(예: 메토트렉세이트) 또는 코르티코스테로이드 = < 10 mg 프레드니손(자가 면역 질환 치료를 위해) 매일 또는 이에 상응하는 용량을 사용하거나 대체 요법(예: 티록신, 인슐린)을 사용하는 환자는 치료 의사가 환자에게 최선의 이익이라고 판단하는 경우 적격입니다.
  • 참가자는 코르티코스테로이드로 12주 이상의 면역 억제를 필요로 하는 ipilimumab 또는 nivolumab에 대한 3등급 또는 4등급의 면역 관련 부작용이 없어야 합니다.
  • 참가자는 특히 엔코라페닙 및/또는 비니메티닙과 관련된 부작용, 특히 하나 또는 두 가지 약물의 중단이 필요한 부작용이 없어야 합니다. (다른 BRAF/MEK 억제제에는 적용되지 않습니다.)
