Gerandomiseerd onderzoek om de werkzaamheid en veiligheid van Finerenone op de morbiditeit en mortaliteit te bepalen bij hartfalenpatiënten met een linkerventrikelejectiefractie groter dan of gelijk aan 40% in het ziekenhuis vanwege een episode van acuut gedecompenseerd hartfalen (REDEFINE-HF)

Finerenone zal worden vergeleken met placebo om de werkzaamheid en veiligheid van de behandeling te bepalen bij patiënten die in het ziekenhuis zijn opgenomen met acuut gedecompenseerd hartfalen (HF) en een licht verminderde of behouden linkerventrikelejectiefractie.

Inclusiecriteria:

  • Geef elektronische of schriftelijke geïnformeerde toestemming, persoonlijk of via een wettelijk bevoegde vertegenwoordiger
  • Leeftijd ≥18 jaar
  • Huidige ziekenhuisopname of onlangs ontslagen met de primaire diagnose hartfalen
  • Tekenen en symptomen van hartfalen op het moment van ziekenhuisopname
  • Beeldvormend bewijs van licht verminderde of behouden linkerventrikelejectiefractie (EF) (40% of hoger)
  • Verhoogd N-terminaal pro B-type natriuretisch peptide (NTproBNP) ≥1000 pg/ml of B-type natriuretisch peptide (BNP) ≥250 pg/ml voor patiënten zonder atriumfibrilleren (AF); of verhoogde NTproBNP ≥2000 pg/ml of BNP ≥500 pg/ml voor patiënten met AF

Uitsluitingscriteria:

  • Behandeling met een mineralocorticoïdreceptorantagonist (MRA)
  • Gedocumenteerde voorgeschiedenis van ernstige hyperkaliëmie in de setting van MRA-gebruik
  • Geschatte glomerulaire filtratiesnelheid (eGFR) <25 ml/min/1,73m² of serum/plasmakalium >5,0 mmol/l bij screening
  • Acuut myocardinfarct, coronaire revascularisatie, klepvervanging/reparatie of implantatie van een apparaat voor hartresynchronisatietherapie binnen 30 dagen
  • Hemodynamisch significante (ernstige) ongecorrigeerde primaire hartklepziekte

  • Waarschijnlijke alternatieve oorzaak van de symptomen van hartfalen van de deelnemer
