De belangrijkste doelen van deze studie zijn om te leren over de veiligheid van Sacituzumab Tirumotecan met Bevacizumab en als mensen het verdragen; En als mensen die Sacituzumab Tirumotecan nemen met of zonder bevacizumab langer leven zonder dat de kanker erger wordt dan degenen die standaard zorgbehandeling krijgen.

  • Heeft 4 of meer cycli van op platina gebaseerde doubletchemotherapie ontvangen in eerstelijns en in totaal 6 cycli van op carboplatine gebaseerde doubletchemotherapie in tweedelijnsomgeving voor eierstokkanker (OC).
  • Heeft platinagevoelige epitheliale OC,
  • Heeft weefsel geleverd van een tumorlaesie die niet eerder was bestraald
  • Human Immunodeficiency Virus (HIV)-geïnfecteerde deelnemers moeten goed gecontroleerde HIV hebben op antiretrovirale therapie
  • Deelnemers die hepatitis B -oppervlakte -antigeen positief zijn, komen in aanmerking als ze gedurende ten minste 4 weken een hepatitis B -virus (HBV) antivirale therapie hebben ontvangen en niet -detecteerbare HBV -virale belasting hebben voorafgaand aan de toewijzing (deel 1) of randomisatie (deel 2)
  • Deelnemers met een geschiedenis van hepatitis C -virus (HCV) -infectie komen in aanmerking als HCV -virale belasting niet detecteerbaar is bij screening
  • Heeft een ECOG -prestatiestatus van 0 tot 1 beoordeeld binnen 7 dagen vóór toewijzing (deel 1) of randomisatie (deel 2)

Uitsluitingscriteria:

  • Heeft geen pitheliale kankers (kiemceltumoren en seksnoer-stromale tumoren), borderline-tumoren (laag kwaadaardig potentieel), slijmvlies, seromucineus die voornamelijk mucineus is, malignant Brenner's tumor en ongedifferentieerd carcinoom
  • Heeft platinaresistente OC of platina-refractaire OC
  • Heeft de geschiedenis van gedocumenteerd ernstig droge oogsyndroom, ernstige meibomische klierziekte en/of blefaritis, of hoornvliesziekte die hoornvliesgenezing voorkomt/vertraagt.
  • Heeft actieve inflammatoire darmaandoeningen die immunosuppressieve medicatie of eerdere geschiedenis van inflammatoire darmaandoeningen vereisen (bijv. De ziekte van Crohn, colitis ulcerosa of chronische diarree)
  • Heeft ongecontroleerde, significante hart- en vaatziekten of cerebrovasculaire aandoeningen.
  • Heeft een geschiedenis van (niet -infectieuze) pneumonitis/interstitiële longziekte die steroïden vereiste of huidige pneumonitis/interstitiële longziekte heeft.
  • HIV-geïnfecteerde deelnemers met een geschiedenis van Kaposi's sarcoom- en/of multicentrische Castleman's Disease
  • Heeft meer dan 2 eerdere lijnen van systemische therapie ontvangen voor OC.
  • Heeft eerdere systemische antikankertherapie ontvangen
  • Heeft eerdere radiotherapie ontvangen binnen 2 weken na allocatie (deel 1) of randomisatie (deel 2), of heeft straling gerelateerde toxiciteiten, waarvoor corticosteroïden nodig zijn
  • Heeft een extra maligniteit die vordert of in de afgelopen 3 jaar een actieve behandeling vereist
  • Heeft een actief centraal zenuwstelsel (CNS) metastasen en/of carcinomateuze meningitis
  • Heeft een actieve infectie die systemische therapie vereist

Part 2: Progression-Free Survival (PFS)

PFS is defined as the time from randomization to the first documented progressive disease (PD) or death from any cause, whichever occurs first. Per RECIST 1.1, PD is defined as at least a 20% increase in the sum of diameters of target lesions. In addition to the relative increase of 20%, the sum must also demonstrate an absolute increase of at least 5 mm. The appearance of one or more new lesions is also considered PD. PFS as assessed by blinded independent central review (BICR) will be presented.

The EORTC-QLQ-C30 is a 30-item questionnaire developed to assess the QoL of cancer patients. Participant responses to Item 30 (""How would you rate your overall quality of life during the past week?") is scored on a 7-point scale (1=Very Poor to 7=Excellent). Using linear transformation, raw scores are standardized, so that scores range from 0 to 100. A higher value indicates a better level of function. The change from baseline in EORTC QLQ-C30 Item 30 score will be reported.

Participant responses to the questions "Were you limited in doing either your work or other daily activities during the past week?" and " Were you limited in pursuing your hobbies or other leisure time activities during the past week?" will be scored on a 4-point scale (1=Not at All to 4=Very Much). Using linear transformation, raw scores are standardized, so that scores range from 0 to 100. Higher scores indicate a more impaired level of role functioning. Change from baseline in the role functioning score will be presented.

Part 2: Change From Baseline in EORTC Quality of Life Questionnaire-Ovarian Cancer Module 28 (QLQ-OV28) abdominal/gastrointestinal (GI) symptom scale

The EORTC QLQ-OV28 is an OC-specific, and psychometrically and clinically validated module to supplement the EORTC QLQ-C30. The EORTC QLQ-OV28 abdominal/GI symptom scale is scored on a 4-point scale (1=not at all, 2=a little, 3=quite a bit, 4=very much).
