National Myelodysplastic Syndromes Natural History Study

Multisenterstudie som inkluderer pasienter mistenkt eller nylig diagnostisert med myelodysplastiske syndromer (MDS), myelodysplastiske syndromer/myeloproliferative neoplasmer (MDS/MPN) overlappende lidelse eller idiopatisk cytopeni av ubestemt betydning (ICUS). Multisenter, prospektiv kohortstudie som registrerer pasienter fra sentre i National Cancer Institute (NCI) National Clinical Trials Network (NCTN) og NCI Community Oncology Research Program (NCORP). Opptjeningsperioden er 5+ år. Etter sentral patologigjennomgang av registrerte deltakere, vil omtrent 2000 tilfeller av MDS eller MDS/MPN overlappende lidelser, og 500 tilfeller av ICUS bli identifisert for den longitudinelle studiekohorten og opptil 1000 tilfeller vil bli identifisert for tverrsnittskohorten. Det vil ikke bli registrert mer enn 3500 totalt deltakere. Deltakere i den langsgående kohorten kan følges hele livet.

Målet med den nasjonale MDS-studien er å etablere en offentlig tilgjengelig ressurs for å lette studiet av MDS naturhistorie. Studiepopulasjon

Pasienter med mistanke om MDS eller MDS/MPN overlappingsforstyrrelser som gjennomgår diagnostisk opparbeiding og benmargsvurderinger, og pasienter diagnostisert med MDS innen 6 måneder etter registrering og gjennomgår klinisk evaluering og benmargsvurderinger for å bekrefte MDS eller evaluere sykdomsstatus

Beskrivelse

Inklusjonskriterier:

  • Mistenkt (f.eks. vedvarende uforklarlig cytopeni, sirkulerende perifere blaster osv.) MDS eller MDS/MPN overlappingsforstyrrelser og gjennomgår diagnostisk opparbeidelse med planlagte benmargsvurderinger, eller diagnostisert med de novo eller terapirelatert MDS innen 12 måneder etter påmelding pr. Verdens helseorganisasjons (WHO) kriterier1 og gjennomgår klinisk evaluering og planlagte benmargsvurderinger for å bekrefte MDS eller for å evaluere sykdomsstatus
  • Benmargsaspirasjon forventes utført innen 1 uke etter registrering, og må i alle tilfeller utføres senest 4 uker etter innmelding
  • Alder 18 eller eldre
  • Hvis anemisk uten forutgående MDS-diagnose, B12-nivå, serumfolat, gjennomsnittlig korpuskulært volum (MCV), røde blodlegemers distribusjonsbredde (RDW), ferritin- og jernstudier (jern, total jernbindingskapasitet (TIBC) test og prosent metning) må utføres innen 6 måneder før.

Ekskluderingskriterier:

  • Tidligere behandling for MDS ved inngang og gjennom tidspunktet for inntreden benmargsaspirasjon
  • Behandling med hematopoetiske vekstfaktorer i de foregående 6 måneder
  • Diagnostisering av en solid svulst eller hematologisk malignitet innen to år før innmelding med unntak av in situ kreft i huden (basal eller plateepitel), livmorhals, blære, bryst eller prostata
  • Behandling med strålebehandling i de to årene før registrering
  • Ikke-hormonell behandling for malignitet innen to år før registrering
  • Etablert arvelig benmargssviktsyndrom
  • Kjent primærdiagnose av aplastisk anemi, klassisk paroksysmal nattlig hemoglobinuri, amegakaryocytisk trombocytopenisk purpura eller stor granulær lymfocytt leukemi
  • Registrert i Connect® MDS/AML Disease Registry (NCT01688011)
