Pilot Project on Enhancing Support for Hidden Elderly in the Three-nil Buildings and Old Tenement Buildings: Capacity Building & Impact Evaluation
The aims of this interventional study are of two-fold. First, the study aims to develop the capacity to facilitate the collaborating NGOs to address the social determinants of aging well for 3-N seniors. The second aim of the study is to evaluate the impact of the transitional support for 3-N elders. The main question is aims to answer is:
Does the transitional support facilitate a better physical and psychological wellbeing in 3-N seniors.
Participants will be categorized into transitional-support group or non-transitional-support group based on their existing community service. They will join evaluation at four timepoints (baseline 3-, 6-, and 12-month) over the 1-year follow-up period.

Detailed Description
The project will set up the following digital platform:
- Neighbor Link for Healthy Living
- Healthy Lifestyle Hubs without Walls
- Service Connect Compass Central to the concept of prompting 'aging well' through addressing the social determinants of health, the evaluation not only focuses on the direct impact on the social cohesion of the beneficiaries with their immediate community context, but also captures the indirect impact on the broader dimensions reflecting 'aging well'.
