ODŮVODNĚNÍ: Radiační terapie využívá k poškození nádorových buněk vysokoenergetické rentgenové záření. Chemoterapie používá různé způsoby, jak zastavit dělení nádorových buněk, takže přestanou růst nebo zemřou. Kombinace radioterapie s chemoterapií může zabít více nádorových buněk.

ÚČEL: Randomizovaná studie fáze III pro studium účinnosti radiační terapie a karmustinu při léčbě pacientů, kteří mají supratentoriální multiformní glioblastom.

CÍLE: I. Zjistit, zda použití radiochirurgie (stereotaktické ozáření zevním paprskem) před konvenční radioterapií karmustinem (BCNU) zlepšuje celkové přežití ve srovnání s konvenční radioterapií plus BCNU samotnou u pacientů se supratentoriálním multiformním glioblastomem. II. Určete a porovnejte frekvenci a závažnost toxicit spojených s těmito režimy. III. Porovnejte účinky těchto dvou režimů na neurologické funkce a kvalitu života.

PŘEHLED: Randomizovaná studie. Rameno I: Radioterapie plus jednočinná chemoterapie. Ozáření nádoru pomocí megavoltážního zařízení (alespoň 4 MV fotony); plus Carmustine, BCNU, NSC-409962. Rameno II: Radiochirurgie následovaná radioterapií plus jednočinná chemoterapie. Stereotaktické ozařování nádorů; následované ozářením nádoru jako v rameni I; plus BCNU.

PŘEDPOKLADANÝ AKRUÁLNÍ AKRUÁLNÍ: V průběhu přibližně 3 let bude přihlášeno 200 pacientů.

CHARAKTERISTIKA ONEMOCNĚNÍ: Histopatologicky potvrzený supratentoriální multiformní glioblastom nebo gliosarkom Požaduje se diagnostika chirurgickou biopsií nebo resekcí Následující typy nádorů jsou specificky vyloučeny: Astrocytom s atypickými nebo anaplastickými rysy Dobře diferencované astrocytomy Nádory pocházející z mozkového kmene Infratenní nádory 10 mm nádorů nádory Multifokální maligní gliom Recidivující maligní gliom Dobře ohraničený nádor zvyšující kontrast na předoperačním a pooperačním kontrastním CT nebo MRI s maximálním průměrem (v libovolném směru) 40 mm Požadováno Nádory, které se nezlepšují při pooperačním CT nebo MRI vyloučeno Současné zařazení na RTOG-9308 podporován

CHARAKTERISTIKA PACIENTA: Věk: 18 a více Výkonnostní stav: Karnofsky 60-100 % Předpokládaná délka života: Nejméně 3 měsíce Hematopoetický: ANC nejméně 1 500 Krevní destičky nejméně 100 000 Hb nejméně 10 g/dl (může podat transfuzi) Jaterní: ne vyšší než bilirubin 2,0 mg/dl SGOT nebo SGPT ne větší než 2 x normální Ledvina: Kreatinin ne větší než 1,5 mg/dl BUN ne větší než 25 mg/dl Plicní: Rentgen hrudníku normální Pokud je abnormální nebo pokud je v anamnéze plicní onemocnění, studie plicních funkcí (včetně DLCO) musí dosahovat alespoň 75 % předpokládané hodnoty Jiné: Stav neurologické funkce 0-3 Žádné AIDS Žádné závažné lékařské nebo psychiatrické onemocnění, které by vylučovalo protokolární léčbu nebo sledování Žádná druhá malignita během 5 let kromě: Nemelanomatózní kůže rakovina In situ karcinom děložního čípku
