ZDŮVODNĚNÍ: Pochopení vztahu mezi hustotou prsou, hladinami hormonů v krvi a hladinami protirakovinných léků v krvi může pomoci zlepšit schopnost plánovat účinnou léčbu u žen s invazivní rakovinou prsu.

Hlavní

  • Posuďte změnu procentuální hustoty prsu a hustoty plochy v reakci na terapii inhibitorem aromatázy z předběžné léčby na 1 rok, 1 rok na 2 roky a předléčbu na 2 roky u žen s invazivním karcinomem prsu.
  • Určete změny v procentu hustoty prsou a hustoty plochy v reakci na terapii inhibitorem aromatázy od předběžné léčby do 1 roku a korelujte změny se změnami plazmatických hormonů (např. estron, estron-sulfát, estradiol a globulin vázající pohlavní hormony) a hladinami léků (anastrozol nebo exemestan) ve stejném období u žen s invazivním karcinomem prsu.
  • Určete změnu v průběhu času v procentech hustoty prsu a husté oblasti (od předléčení do časového období před lokální recidivou) v kontralaterálním prsu v korelaci s lokální recidivou karcinomu prsu.

  • Určete změnu v průběhu času v procentech hustoty prsu a husté oblasti v kontralaterálním prsu v korelaci s rozvojem kontralaterálního karcinomu prsu.
  • Zjistěte, zda ženy s vysokou procentuální hustotou před léčbou (horní tertil) zažívají větší pokles procentuální hustoty prsou po 1 a 2 letech léčby inhibitorem aromatázy než ženy s nízkou procentuální hustotou před léčbou (dolní tertil).
  • Zjistěte, zda ženy s vysokou hustou oblastí před ošetřením (horní tertil) zaznamenají větší pokles husté oblasti po 1 a 2 letech léčby inhibitorem aromatázy než ženy s nízkou hustotou oblastí před ošetřením (spodní tertil).
  • Porovnejte haplotypově označené jednonukleotidové polymorfismy v genech v aromatázové dráze (identifikované prostřednictvím projektů Mayo Clinic a Indiana University Pharmacogenomics Research Network) se změnami v procentech a plošné hustotě prsů, hladinách hormonů v plazmě a hladinách léků za 1 rok.

Pacientky vyplní 10minutový dotazník o faktorech, které mohou ovlivnit změny hustoty prsu na začátku studie, a další dotazník 1 rok a 2 roky po registraci. Vzorky krve se odebírají na začátku (před zahájením léčby) a 1 rok po registraci pro analýzu hladiny hormonů a léků. Mamogramy pořízené před registrací (do 12 měsíců před zařazením) a přibližně 1 a 2 roky po registraci do této studie jsou získány a digitalizovány pro stanovení procenta hustoty prsou a husté plochy.

PŘEDPOKLÁDANÝ AKRUÁLNÍ AKRUÁLNÍ: Do této studie bude nashromážděno celkem 550 pacientů.

Pacientky s diagnostikovanou rakovinou prsu.

Popis

CHARAKTERISTIKA ONEMOCNĚNÍ:

  • Histologicky potvrzený invazivní karcinom prsu
  • Kompletně resekovaná nemoc

  • Jeden intaktní, nezhoubný prs bez předchozí operace prsu kromě biopsie prsu

    • K dispozici je mamograf pořízený do 12 měsíců před zápisem, který zahrnuje pohledy ze strany a shora dolů na intaktní, nezhoubný prs
  • Nádor pozitivní na estrogenový a/nebo progesteronový receptor

CHARAKTERISTIKA PACIENTA:

  • ženský
  • Postmenopauzální
  • Souhlasí s vyhledáním a digitalizací mamografů pořízených před registrací (do 12 měsíců před vstupem do studia) a přibližně 1 a 2 roky po registraci do této studie
  • Souhlasí s odběrem dalšího vzorku krevní banky současně s odběrem vzorků před léčbou pro nadřazený protokol CAN-NCIC-MA27
  • Souhlasí s odebráním vzorku krve 12 měsíců po registraci v této studii

PŘEDCHOZÍ SOUČASNÁ TERAPIE:

  • Viz Charakteristika onemocnění
  • Více než 6 měsíců od předchozí hormonální substituční terapie, perorální antikoncepce, tamoxifenu, raloxifenu, jiných selektivních modulátorů estrogenových receptorů nebo analogů hormonu uvolňujícího gonadotropin před předregistračním mamografem

Pacientky vyplní 10minutový dotazník o faktorech, které mohou ovlivnit změny hustoty prsu na začátku studie, a další dotazník 1 rok a 2 roky po registraci. Vzorky krve se odebírají na začátku (před zahájením léčby) a 1 rok po registraci pro analýzu hladiny hormonů a léků. Mamogramy pořízené před registrací (do 12 měsíců před zařazením) a přibližně 1 a 2 roky po registraci do této studie jsou získány a digitalizovány pro stanovení procenta hustoty prsou a husté plochy.
