Et fase III randomiseret forsøg med kemoterapi med eller uden bevacizumab hos patienter med tilbagevendende eller metastaserende hoved- og nakkekræft

Dette randomiserede fase III-forsøg undersøger kemoterapi for at se, hvor godt det virker med eller uden bevacizumab til behandling af patienter med pladecellekræft i hoved og hals, som er vendt tilbage (tilbagevendende), eller som har spredt sig til andre dele af kroppen (metastatisk). Lægemidler brugt i kemoterapi, såsom docetaxel, cisplatin, carboplatin og fluorouracil, virker på forskellige måder for at stoppe væksten af tumorceller, enten ved at dræbe cellerne, ved at stoppe dem i at dele sig eller ved at stoppe dem i at sprede sig. Monoklonale antistoffer, såsom bevacizumab, kan interferere med tumorcellernes evne til at vokse og sprede sig. Bevacizumab kan også gøre tumorceller mere følsomme over for kemoterapi og stoppe væksten af hoved- og halskræft ved at blokere blodtilførslen til tumoren. Det vides endnu ikke, om kombinationskemoterapi er mere effektiv, når den gives med eller uden bevacizumab til behandling af patienter med planocellulært karcinom i hoved og hals.

PRIMÆRE MÅL:

I. At sammenligne den samlede overlevelse af patienter med tilbagevendende eller metastatisk hoved- og halscancer behandlet med standard platinbaseret kemoterapi med eller uden bevacizumab.

SEKUNDÆRE MÅL:

I. At vurdere toksicitet ved tilsætning af bevacizumab til hver platin-doublet (cisplatin/docetaxel, carboplatin/docetaxel, cisplatin/fluorouracil [5-FU], carboplatin/5-FU).

II. At sammenligne de objektive responsrater og den progressionsfrie overlevelse opnået med ovennævnte terapier.

III. At indsamle blodprøver før og efter behandling til fremtidige korrelative undersøgelser.

IV. At indsamle tumorvævsprøver tilgængelige ved baseline fra tidligere diagnostiske procedurer til fremtidige korrelative undersøgelser.

OVERSIGT: Efter at lægen har besluttet, hvilken kemoterapi dublet der skal bruges, randomiseres patienterne til 1 ud af 2 behandlingsarme for den kemoterapikombination.

ARM IA: Patienter modtager kemoterapi omfattende docetaxel intravenøst (IV) over 1 time og cisplatin IV over 1-2 timer på dag 1. Kurser gentages hver 21. dag i fravær af sygdomsprogression eller uacceptabel toksicitet.

ARM IB: Patienterne får bevacizumab IV over 30-90 minutter på dag 1 og docetaxel og cisplatin som i arm IA.

ARM IIA: Patienterne får docetaxel IV over 1 time og carboplatin IV over 30 minutter på dag 1. Kurser gentages hver 21. dag i fravær af sygdomsprogression eller uacceptabel toksicitet.

ARM IIB: Patienterne får bevacizumab som i arm IB og docetaxel og carboplatin som i arm IIA.

ARM IIIA: Patienter får cisplatin IV over 1-2 timer på dag 1 og fluorouracil IV kontinuerligt på dag 1-4. Kurser gentages hver 21. dag i fravær af sygdomsprogression eller uacceptabel toksicitet.

ARM IIIB: Patienterne får bevacizumab som i arm IB og cisplatin og fluorouracil som i arm IIIA.

ARM IVA: Patienterne får carboplatin IV over 30 minutter på dag 1 og fluorouracil IV kontinuerligt på dag 1-4. Kurser gentages hver 21. dag i fravær af sygdomsprogression eller uacceptabel toksicitet.

ARM IVB: Patienterne får bevacizumab som i arm IB og carboplatin og fluorouracil som i arm IVA.

Efter afsluttet undersøgelsesbehandling følges patienterne hver 3. måned i 2 år og derefter hver 6. måned i 3 år.

Patienter skal have histologisk eller cytologisk bekræftet pladecellekræft i hoved og hals (SCCHN) fra et hvilket som helst primært sted, inklusive ukendte primære kræftformer i hoved og hals; patienten må ikke have nasopharynxcarcinom af histologisk type Verdenssundhedsorganisationen (WHO) 2 eller 3 eller planocellulært karcinom, der stammer fra huden
Patienter skal have SCCHN, der enten er (a) tilbagevendende, vurderes uhelbredelig ved kirurgi eller stråling eller (b) metastatisk; BEMÆRK: Patienter, der nægter radikal resektion for tilbagevendende sygdom, er kvalificerede; BEMÆRK: Et andet primært planocellulært carcinom i hoved og hals er tilladt, hvis berettigelse er baseret på et tilbagevendende eller metastatisk første primært planocellulært karcinom i hoved og hals
Ingen forudgående kemoterapi eller biologisk/molekylær målrettet behandling for tilbagevendende eller metastatisk SCCHN
- Patienter kan have modtaget et regime med induktion, samtidig kemoradioterapi og/eller adjuverende kemoterapi som en del af indledende potentiel helbredende behandling, men må ikke have modtaget tidligere kemoterapi for tilbagevendende eller metastatisk sygdom
- Der kræves minimum 4 måneder mellem sidste dosis kemoterapi eller kemoradioterapi og undersøgelsesbehandling; desuden skal patienter være progressionsfrie i mindst 4 måneder efter afslutning af kemoterapi eller kemoradioterapi eller stråling plus cetuximab givet med en helbredende hensigt; (cetuximab-behandling: 4 måneder er påkrævet mellem sidste dosis kemoterapi eller kemoradioterapi og undersøgelsesbehandling, hvis en del af samtidig regime, 8 uger, hvis en del af adjuverende regime efter stråling)
- Patienter med progression efter 2 cyklusser med induktionskemoterapi er ikke kvalificerede til undersøgelsen

Maksimalt én forudgående strålebehandling, helbredende eller palliativ, til hoved og hals er tilladt; hvis strålingen kombineres med kemoterapi og/eller cetuximab, skal der gå minimum 4 måneder mellem afslutning af strålebehandling og registrering; hvis strålingen gives alene, skal der gå minimum 8 uger mellem afslutning af strålebehandling og registrering; Der skal minimum gå 3 uger mellem forudgående stråling til andre områder og registrering
Patienter må ikke modtage andre forsøgsmidler, mens de er i undersøgelsen
Eastern Cooperative Oncology Group (ECOG) præstationsstatus på 0-1
Patienter skal være restitueret til grad 1 eller bedre efter eventuelle akutte virkninger af tidligere operation, kemoterapi eller strålebehandling og bør være > 4 uger efter operationen; kronisk sen xerostomi, tale- og synkeabnormiteter som følge af tidligere stråling eller operation er tilladt, hvis ernæringstilstanden er stabil
Patienter skal have målbar sygdom baseret på responsevalueringskriterier i solide tumorer (RECIST); baseline målinger og evalueringer af alle sygdomssteder skal opnås =< 4 uger før randomisering; sygdom på tidligere bestrålede steder anses for at kunne måles, hvis der har været en utvetydig sygdomsprogression eller biopsi-bevist restkarcinom efter strålebehandling; vedvarende sygdom efter strålebehandling skal biopsi bevises mindst 8 uger efter afslutning af strålebehandling; (radiografiske fund er acceptable, forudsat at der kan foretages entydige målinger)

- Anamnese med grov hæmotyse (lyserødt blod på 1/2 tsk eller mere pr. hosteepisode) =< 3 måneder før indskrivning

Patienter bør ikke have en historie med trombose (f. lungeemboli eller dyb venetrombose), der i øjeblikket kræver terapeutisk antikoagulering (profylaktisk brug af warfarin 1 mg pr. dag er tilladt) og international normaliseret ratio (INR) bør være < 1,5 ved registrering

Ingen større kirurgisk indgreb, åben biopsi eller væsentlig traumatisk skade inden for 28 dage før studietilmelding eller forventning om behov for større kirurgisk indgreb i løbet af undersøgelsen

Kvinder må ikke være gravide eller amme; gravide kvinder er udelukket fra denne undersøgelse; kvinder i den fødedygtige alder og mænd skal acceptere total afholdenhed eller at anvende passende hormon- eller barrieremetode til prævention før studiestart og under undersøgelsens deltagelse; hvis en kvinde bliver gravid eller har mistanke om, at hun er gravid under denne undersøgelse, skal hun straks informere sin behandlende læge; alle kvinder i den fødedygtige alder skal have en blodprøve eller urinundersøgelse inden for 2 uger før randomisering for at udelukke graviditet
Human immundefektvirus (HIV)-positive patienter, der modtager antiretroviral kombinationsterapi, er udelukket fra undersøgelsen; passende undersøgelser vil blive udført hos patienter, der får antiretroviral kombinationsbehandling, når det er indiceret
