Et åbent, 2 x 2 faktorielt, randomiseret kontrolleret, klinisk forsøg til evaluering af sikkerheden af ​​apixaban vs. vitamin K-antagonist og aspirin vs. aspirin placebo hos patienter med atrieflimren og akut koronarsyndrom eller perkutan koronar intervention

Formålet med denne undersøgelse er at bestemme, om Apixaban er sikrere end en vitamin K-antagonist givet i 6 måneder med hensyn til blødning hos patienter med uregelmæssig hjerterytme (atrieflimren) og et nyligt hjerteanfald eller en nylig procedure for at åbne et blod kar i hjertet. Alle patienter ville også tage en klasse medicin kaldet P2Y12-hæmmere (såsom clopidogrel/Plavix) og blive behandlet i op til 6 måneder. Inklusionskriterier:

  • Voksne med enten aktiv eller tidligere ikke-valvulær atrieflimren eller flagrer med den planlagte eller eksisterende brug af et oralt antikoagulant til profylakse af tromboemboli. Derudover skal forsøgspersoner have haft et akut koronarsyndrom eller perkutan koronar intervention med en stent inden for de foregående 14 dage
  • Planlagt brug af blodpladehæmmende midler i mindst 1 til 6 måneder
  • Mænd og kvinder ≥ 18 år
  • Kvinder i den fødedygtige alder skal have en negativ serum- eller uringraviditetstest inden for 24 timer før starten af ​​studielægemidlet

Ekskluderingskriterier:

  • Andre tilstande end atrieflimren, der kræver kronisk antikoagulering. (f.eks. protese mekanisk hjerteklap)
  • Alvorlig nyreinsufficiens (serumkreatinin > 2,5 mg/dL eller en beregnet kreatininclearance < 30 ml/min.
  • Patienter med en historie med intrakraniel blødning
  • Patienter har haft eller vil gennemgå koronar arteriel bypass graft (CABG) for deres indeks akut koronar syndrom (ACS) hændelse
  • Patienter med kendt igangværende blødning og patienter med kendte koagulopatier
