En åpen, 2 x 2 faktoriell, randomisert kontrollert, klinisk studie for å evaluere sikkerheten til apixaban vs. vitamin K-antagonist og aspirin vs. aspirin placebo hos pasienter med atrieflimmer og akutt koronarsyndrom eller perkutan koronar intervensjon
Hensikten med denne studien er å finne ut om Apixaban er tryggere enn en vitamin K-antagonist gitt i 6 måneder når det gjelder blødning hos pasienter med uregelmessig hjerterytme (atrieflimmer) og nylig hjerteinfarkt eller en nylig prosedyre for å åpne opp et blod kar i hjertet.
Alle pasienter vil også ta en klasse med medisiner kalt P2Y12-hemmere (som klopidogrel/Plavix) og bli behandlet i opptil 6 måneder.
Hovedfokuset vil være en sammenligning av blødningsrisikoen for Apixaban, med eller uten aspirin, versus en vitamin K-antagonist, som warfarin, med eller uten aspirin.

Inklusjonskriterier:
- Voksne med enten aktiv eller tidligere ikke-valvulær atrieflimmer eller flagrer med planlagt eller eksisterende bruk av et oralt antikoagulant for profylakse av tromboemboli. - Planlagt bruk av blodplatehemmere i minst 1 til 6 måneder
- Menn og kvinner ≥ 18 år
- Kvinner i fertil alder må ha en negativ serum- eller uringraviditetstest innen 24 timer før starten av studiemedikamentet
Ekskluderingskriterier:
- Andre tilstander enn atrieflimmer som krever kronisk antikoagulasjon. (f.eks. protetisk mekanisk hjerteklaff)
- Alvorlig nyresvikt (serumkreatinin > 2,5 mg/dL eller en beregnet kreatininclearance < 30 ml/min.
- Pasienter med en historie med intrakraniell blødning
- Pasienter har hatt eller vil gjennomgå koronar arteriell bypass graft (CABG) for sin indeks akutte koronar syndrom (ACS) hendelse
- Pasienter med kjent pågående blødning og pasienter med kjente koagulopatier
- Eventuelle kontraindikasjoner eller allergier mot VKA, apixaban eller tiltenkte P2Y12-antagonister eller mot aspirin
