小児胃不全麻痺レジストリ 2 の目的は、胃不全麻痺および胃不全麻痺様症候群 (胃不全麻痺の症状はあるが正常な胃内容排出) を持つ小児、青少年、および若年成人の全国的な前向き登録を作成することです。

胃内容排出の遅延または正常な胃不全麻痺の症状(吐き気、嘔吐、腹痛、膨満感、膨満など)を伴う小児、青少年、若年成人の自然史と臨床経過を調査する観察研究。
これらの障害をより深く理解するために、このレジストリは人口統計学的、臨床的、生理学的、アンケート、および患者の転帰データを収集して、参加者とその臨床経過を特徴づけます。

研究対象者は、米国在住のGPの症状のある8~25歳の参加者216名となります。
説明
包含基準:
- 年齢に応じて、署名と日付を記入したインフォームドコンセントフォームと同意書を提供します。
- すべての研究手順に従う意思と研究期間中の利用可能性の表明
- 入学時の年齢が8歳以上25歳以下であること
- 少なくとも 12 週間続く GP の症状: 以下の組み合わせの症状:
吐き気、嘔吐、早期の満腹感、食後の膨満感;症状は上腹部の痛みを伴うこともあります
- 過去 12 か月以内に 4 時間のエッグビータープロトコル(または同等の一般的な液体卵白粉)を使用した固形物の胃排出シンチグラフィー検査を受け、次の 2 つのカテゴリーのいずれかに該当する者。
- 胃内容排出の遅延 - 4 時間のシンチグラフィー胃内容排出研究に基づいて、異常な 2 時間 (>60% 保持) および/または 4 時間 (>10% 保持) の結果として定義されます。
- 胃内容排出が遅れていないが、Gp(この研究ではGLSと呼ばれる)の症状がある、または以前に小児胃不全麻痺登録(PGpR)研究に参加したことのある患者
- 糖尿病性または特発性 Gp または GLS の病因
除外基準:
- シンチグラフィーによる胃内容排出検査に従うことができない、または検査を完了できない(卵に対するアレルギーを含む)
- 妊娠
- 自閉症スペクトラム障害、著しい発達遅延、精神病(アンケートに回答できないため)
- 麻薬性鎮痛薬を週に 3 日を超えて使用する。
- 消化管粘膜疾患に関連する症状の存在(例、炎症性腸疾患、既知の好酸球性胃腸炎または好酸球性食道炎、胃潰瘍、消化性潰瘍、セリアック病)
- 胃内容排出の遅延を引き起こす可能性のあるその他の疾患の存在
- 上部内視鏡検査(EGD)、上部消化管シリーズ(UGI)、または腹部CTによって確認された消化管の構造
- 頭蓋内圧の上昇、空間の占有、炎症性/感染性の病巣など、吐き気や嘔吐を引き起こす可能性のある一次的な神経学的症状
- 急性または慢性腎不全(年齢によるクレアチニン異常)および/または血液透析または腹膜透析を受けている
- 急性肝不全
- 進行性肝疾患(門脈圧亢進症の特徴)
- 臨床的に重大な先天性心疾患(心臓修復中の膣損傷など)
- 食道、胃、腸の手術歴。
- ミトコンドリア病や先天性代謝異常などの代謝疾患
- 慢性肺疾患(嚢胞性線維症を含む)
- 重篤な慢性病状(炎症性腸疾患など)
- 胃排出試験中の運動性に影響を与える可能性のある薬剤の使用

小児QOLインベントリ(PedsQL)Gastrointestinal Symptoms Scales™からの合計スコアの変化を使用した、胃腸症状の平均症状重症度の変化

|
PedsQL™ 胃腸症状スケールのアンケートには、10 の側面をカバーする 65 の回答項目があり、各項目は、良い方から悪い方への 5 段階のリッカート スケールの選択肢として提供されます: 0= まったくない、1= ほとんどない、2= 時々、3= 頻繁に、 4=ほぼ常に。
項目の回答は、悪い方から良い方へ変換され、0 ~ 100 に再スケールされます。0= ほぼ常に、25= 頻繁に、50= 時々、75= ほとんどなし、100= まったくなし。スコアが高いほど健康関連が良好であることを示します。生活の質 (HRQOL) が向上し、問題や症状が軽減されます。
参加者ごとの PedsQL™ GI 症状スケールの合計スコアは、変換および再スケールされた 65 項目の回答の合計です。
主要評価項目は、変換された 65 項目の回答の算術平均であり、各参加者についてベースラインと 48 週間に繰り返されます。

腹部検査によるカーネット徴候の有無

|
ベースラインでの腹壁痛のカーネット徴候の有無。 カーネット徴候は、腹壁の筋肉が緊張したときに (急性) 腹痛が変化しない、または増加するという臨床検査の所見です。 腹部検査の一環として、患者は腹部の筋肉を緊張させるために検査台から頭と肩を持ち上げるように求められます。 別の方法としては、患者に両脚を膝を伸ばした状態で上げるように依頼することもできます。 検査結果が陽性の場合は、腹腔ではなく腹壁が痛みの原因である可能性が高いことを示します(たとえば、虫垂炎ではなく直筋鞘血腫による)。 陰性カーネット徴候は、患者に頭を持ち上げるように指示すると腹痛が軽減するときに発生するといわれています。これは腹腔内に痛みの原因があることを示しています。 |
