Descrição
Critério de inclusão:
- Fornecimento de formulário de consentimento informado assinado e datado e consentimento, conforme a idade apropriada.
- Vontade declarada de cumprir todos os procedimentos do estudo e disponibilidade para a duração do estudo
- 8 a 25 anos de idade no momento da inscrição
- Sintomas de GP com pelo menos 12 semanas de duração: constelação de alguma combinação de:
náuseas, vômitos, saciedade precoce, plenitude pós-prandial; os sintomas às vezes podem ser acompanhados por dor abdominal superior
- Foram submetidos a um estudo cintilográfico de esvaziamento gástrico de sólidos usando o protocolo de batedores de ovos de 4 horas (ou farinha de clara de ovo líquida genérica equivalente) nos últimos 12 meses que se enquadram em uma das duas categorias:
- Esvaziamento gástrico retardado - definido como um resultado anormal de 2 horas (>60% de retenção) e/ou 4 horas (>10% de retenção) com base em um estudo cintilográfico de esvaziamento gástrico de 4 horas
- Esvaziamento gástrico que não está atrasado, mas que apresenta sintomas de Gp (designado neste estudo como GLS) ou participou anteriormente do estudo Pediatric Gastroparesis Registry (PGpR)
- Uma etiologia de Gp ou GLS diabético ou idiopático
Critério de exclusão:
- Incapacidade de cumprir ou completar o teste de esvaziamento gástrico cintilográfico (incluindo alergia a ovos)
- Gravidez
- Transtorno do espectro do autismo, atraso significativo no desenvolvimento, psicose (devido à incapacidade de preencher questionários)
- Uso de analgésicos narcóticos mais de três dias por semana.
- Presença de condições associadas à doença da mucosa GI (por exemplo, doença inflamatória intestinal, gastroenterite eosinofílica conhecida ou esofagite eosinofílica, úlcera gástrica, úlcera péptica, doença celíaca)
- Presença de qualquer outra condição que possa causar retardo no esvaziamento gástrico
- Construção gastrointestinal confirmada por endoscopia digestiva alta (EGD), série gastrointestinal superior (UGI) ou TC abdominal
- Condições neurológicas primárias que podem causar náuseas e vômitos, como aumento da pressão intracraniana, ocupação de espaço ou lesões inflamatórias/infecciosas
- Insuficiência renal aguda ou crônica (creatinina anormal para a idade) e/ou em hemodiálise ou diálise peritoneal
- Insuficiência hepática aguda
- Doença hepática avançada (características de hipertensão portal)
- Doença cardíaca congênita clinicamente significativa (ou seja, lesão vaginal durante o reparo cardíaco)
- História de cirurgia esofágica, gástrica ou intestinal.
- Doença metabólica, incluindo doença mitocondrial e erros inatos do metabolismo
- Doença pulmonar crônica (incluindo fibrose cística)
- Uma condição médica crônica grave (por exemplo, doença inflamatória intestinal)
- Uso de medicamentos que podem afetar a motilidade durante o estudo do esvaziamento gástrico
- Qualquer outra condição que, a critério do investigador, possa explicar os sintomas ou interferir nos requisitos do estudo.

Mudança na gravidade média dos sintomas gastrointestinais usando a mudança na pontuação total do Inventário de Qualidade de Vida Pediátrica (PedsQL) Escalas de Sintomas Gastrointestinais™

O questionário PedsQL™ GI Sintomas Scales tem 65 itens de resposta cobrindo 10 dimensões, com cada item oferecido como uma escolha de escala Likert de 5 pontos de melhor para pior: 0=nunca, 1=quase nunca, 2=às vezes, 3=frequentemente, 4=quase sempre.
As respostas dos itens são transformadas em uma ordem de pior para melhor e reescalonadas para 0-100: 0=quase sempre, 25= frequentemente, 50=às vezes, 75=quase nunca, 100= nunca, com escores mais altos indicando melhor saúde qualidade de vida (HRQOL) e menos problemas ou sintomas.
A pontuação total da escala de sintomas gastrointestinais PedsQL™ por participante é a soma das 65 respostas de itens transformadas e reescalonadas.
A medida de resultado primário é a média aritmética das 65 respostas de itens transformados e é repetida na linha de base e 48 semanas para cada participante.

Presença ou ausência do sinal de Carnett avaliado por exame abdominal

|
Presença ou ausência do sinal de Carnett para dor na parede abdominal no início do estudo. O sinal de Carnett é um achado no exame clínico no qual a dor abdominal (aguda) permanece inalterada ou aumenta quando os músculos da parede abdominal estão tensos. Como parte do exame abdominal, pede-se ao paciente que levante a cabeça e os ombros da mesa de exame para contrair os músculos abdominais. Uma alternativa é pedir ao paciente para levantar ambas as pernas com os joelhos retos. Um teste positivo indica o aumento da probabilidade de que a parede abdominal e não a cavidade abdominal seja a fonte da dor (por exemplo, devido a hematoma da bainha do reto em vez de apendicite). Diz-se que o sinal de Carnett negativo ocorre quando a dor abdominal diminui quando o paciente é solicitado a levantar a cabeça; isso aponta para uma causa intra-abdominal da dor. |
