Beschreibung
Einschlusskriterien:
- Bereitstellung einer unterschriebenen und datierten Einverständniserklärung und Zustimmung, je nach Alter.
- Erklärte Bereitschaft zur Einhaltung aller Studienabläufe und Verfügbarkeit für die Dauer der Studie
- Zum Zeitpunkt der Einschreibung 8 bis 25 Jahre alt
- Symptome einer Gp von mindestens 12 Wochen Dauer: Konstellation einer Kombination aus:
Übelkeit, Erbrechen, frühes Sättigungsgefühl, postprandiales Völlegefühl; Die Symptome können manchmal von Schmerzen im Oberbauch begleitet sein
- Sie haben sich in den letzten 12 Monaten einer szintigraphischen Untersuchung der Magenentleerung von Feststoffen unter Verwendung des 4-Stunden-Ei-Beater-Protokolls (oder einer gleichwertigen generischen flüssigen Eiweißmahlzeit) unterzogen und fallen in eine der beiden Kategorien:
- Verzögerte Magenentleerung – definiert als abnormales 2-Stunden- (>60 % Retention) und/oder 4-Stunden-Ergebnis (>10 % Retention), basierend auf einer 4-stündigen szintigraphischen Magenentleerungsstudie
- Die Magenentleerung ist nicht verzögert, aber sie haben Symptome von Gp (in dieser Studie als GLS bezeichnet) oder haben zuvor an der Pediatric Gastroparesis Registry (PGpR)-Studie teilgenommen
- Eine Ätiologie von entweder diabetischem oder idiopathischem Gp oder GLS
Ausschlusskriterien:
- Unfähigkeit, den szintigraphischen Magenentleerungstest einzuhalten oder abzuschließen (einschließlich Allergie gegen Eier)
- Schwangerschaft
- Autismus-Spektrum-Störung, erhebliche Entwicklungsverzögerung, Psychose (aufgrund der Unfähigkeit, Fragebögen auszufüllen)
- Verwendung von narkotischen Analgetika an mehr als drei Tagen pro Woche.
- Vorliegen von Erkrankungen im Zusammenhang mit Erkrankungen der Magen-Darm-Schleimhaut (z. B. entzündliche Darmerkrankung, bekannte eosinophile Gastroenteritis oder eosinophile Ösophagitis, Magengeschwür, Magengeschwür, Zöliakie)
- Vorliegen einer anderen Erkrankung, die zu einer verzögerten Magenentleerung führen könnte
- Gastrointestinale Konstruktion bestätigt durch obere Endoskopie (EGD), obere gastrointestinale Serie (UGI) oder Abdomen-CT
- Primäre neurologische Erkrankungen, die Übelkeit und Erbrechen verursachen können, wie z. B. erhöhter Hirndruck, raumfordernde oder entzündliche/infektiöse Legionen
- Akutes oder chronisches Nierenversagen (altersbedingt abnormales Kreatinin) und/oder unter Hämodialyse oder Peritonealdialyse
- Akutes Leberversagen
- Fortgeschrittene Lebererkrankung (Merkmale der portalen Hypertonie)
- Klinisch signifikante angeborene Herzkrankheit (d. h. Vaginalverletzung während der Herzreparatur)
- Vorgeschichte von Speiseröhren-, Magen- oder Darmoperationen.
- Stoffwechselerkrankungen, einschließlich Mitochondrienerkrankungen und angeborener Stoffwechselstörungen
- Chronische Lungenerkrankung (einschließlich Mukoviszidose)
- Eine schwere chronische Erkrankung (z. B. entzündliche Darmerkrankung)
- Verwendung von Medikamenten, die die Motilität während der Magenentleerungsstudie beeinträchtigen können
- Jeder andere Zustand, der nach Wahl des Prüfers die Symptome erklären oder die Studienanforderungen beeinträchtigen könnte.

Der PedsQL™-Fragebogen zur GI-Symptomskala umfasst 65 Antwortelemente, die 10 Dimensionen abdecken, wobei jedes Element als besser-schlechter-5-Punkte-Likert-Skala zur Auswahl angeboten wird: 0 = nie, 1 = fast nie, 2 = manchmal, 3 = oft. 4=fast immer.
Die Item-Antworten werden in eine schlechtere/bessere Reihenfolge umgewandelt und auf 0-100 neu skaliert: 0=fast immer, 25=häufig, 50=manchmal, 75=fast nie, 100=nie, wobei höhere Werte auf einen besseren Gesundheitszustand hinweisen Lebensqualität (HRQOL) und weniger Probleme oder Symptome
Die Gesamtpunktzahl der PedsQL™ GI-Symptomskalen pro Teilnehmer ist die Summe der 65 transformierten und neu skalierten Item-Antworten.
Das primäre Ergebnismaß ist das arithmetische Mittel der 65 transformierten Item-Antworten und wird zu Studienbeginn und nach 48 Wochen für jeden Teilnehmer wiederholt.

Zeitfenster: Grundlinie
|
Vorhandensein oder Fehlen des Carnett-Zeichens für Bauchwandschmerzen zu Studienbeginn. Beim Carnett-Zeichen handelt es sich um einen Befund bei der klinischen Untersuchung, bei dem (akute) Bauchschmerzen bei Anspannung der Bauchwandmuskulatur unverändert bleiben oder sich verstärken. Im Rahmen der Bauchuntersuchung wird der Patient aufgefordert, Kopf und Schultern vom Untersuchungstisch anzuheben, um die Bauchmuskulatur anzuspannen. Eine Alternative besteht darin, den Patienten zu bitten, beide Beine mit gestreckten Knien anzuheben. Ein positiver Test weist auf eine erhöhte Wahrscheinlichkeit hin, dass die Bauchdecke und nicht die Bauchhöhle die Schmerzquelle ist (z. B. aufgrund eines Rektusscheidenhämatoms statt einer Blinddarmentzündung). Ein negatives Carnett-Zeichen soll auftreten, wenn der Bauchschmerz nachlässt, wenn der Patient aufgefordert wird, den Kopf anzuheben; Dies deutet auf eine intraabdominale Schmerzursache hin. |

Eine negative Veränderung weist auf eine verringerte Schmerzkatastrophe hin.
Der PCS-Gesamtscore wird durch Summieren der Antworten auf alle 13 Punkte berechnet.
Die PCS-Gesamtpunktzahl liegt zwischen 0 und 52.
