전방 십자인대(ACL) 재건술 후 환자의 대퇴사두근 강화에 대한 니합의 효과 (NMES)
전방 십자인대(ACL)의 외과적 재건술 후 재활을 받는 환자에 대한 3상 연구
전방 십자 인대 재건술 후 재활을 받는 환자를 대상으로 한 전향적, 통제, 무작위, 단일 맹검 연구. 96명의 환자가 세 그룹 중 하나에 무작위로 배정되었습니다. 그룹 그룹 KH(Kneehab n=33)는 12주 동안 1일 3회, 주 5일, 20분 동안 새로운 유형의 통합 NMES로 교육을 받았습니다. 그룹 PS(Poli-Stim n=29)는 12주 동안 하루 20분, 주당 5일 동안 표준 신경근 전기 자극(NMES)으로 훈련을 받았습니다. 근육 자극을 사용하지 않은 그룹 CO(대조군 - n=34)는 수의 등척성 대퇴사두근 근육 수축을 수행했습니다. 두 가지 형태의 NMES 치료를 받는 환자는 각각의 전기 근육 자극으로 대퇴사두근을 자발적으로 등척성 수축하도록 지시받았습니다. 세 그룹 모두 클리닉에서 사용되는 표준 ACL 후 재건 재활 프로토콜에 이 훈련 일정을 중첩했습니다.
연구 설계는 3개 그룹과 4개의 검사 시간(기준선, ACL 재건 후 6주, 12주 및 24주)의 양방향 분산 분석에 해당했습니다. 유의수준은 5%로 설정하였다. 대퇴사두근 훈련 개입의 주요 지표는 부상당한 다리의 신근의 등속성 근력 발달이 초당 90도 및 초당 180도에서 참가자가 싱글 레그 홉, 셔틀 런을 수행하여 달성한 거리였습니다. 목표는 싱글 레그 홉과 등속력 테스트에서 상대 점프 거리에서 10%의 차이를 보이고 셔틀 런을 완료하는 데 걸리는 시간에서 1.2초의 차이를 보이는 것이었습니다. 3주의 목표는 참가자들이 보행 보조기를 사용하여 완전한 체중 부하 능력을 달성하도록 설정되었습니다. 일상적인 작업 활동으로의 복귀를 위한 시간 그룹 간의 비교가 계획되었습니다.

- 미세골절이 있거나 무릎에 추가 부담이 있는 ACL 환자
- 수술 3일 후 IKDC 주관적 평가 통증 척도 문항에서 3단계 이상의 통증을 호소한 자
- 지난 90일 이내에 자원봉사 연구에 참여한 참가자
- 심각한 심장 부정맥 또는 기타 이식 장치가 있는 사람
- 심장 박동기를 사용하는 사람
- 신경계 또는 정신 장애가 있는 사람
- 임신 중이거나 모유 수유 중인 사람
- 신경근계를 방해하는 약물 섭취
- 간질 환자
