체세포 과돌연변이 재발성 WHO 등급 4 신경아교종 환자를 대상으로 한 관문 차단 면역 요법의 2상 연구
이 2상 시험은 재발(재발)하고 많은 수의 돌연변이(돌연변이 부담)를 수반하는 신경아교종 환자를 치료하는 데 있어 면역요법 약물(ipilimumab 및 nivolumab)의 효과를 연구합니다.
암은 세포 기능을 제어하는 유전자의 변화(돌연변이)로 인해 발생합니다.
돌연변이 수가 많은 종양은 면역 요법에 잘 반응할 수 있습니다.
이필리무맙 및 니볼루맙과 같은 단일클론항체를 사용한 면역요법은 신체의 면역체계가 암을 공격하는 데 도움이 될 수 있으며 종양 세포의 성장 및 확산 능력을 방해할 수 있습니다.
이필리무맙과 니볼루맙을 투여하면 일반적인 치료(수술 또는 화학요법)에 비해 돌연변이가 많은 재발성 교모세포종의 성장 또는 확산 가능성을 낮출 수 있습니다.

기본 목표:
I. 이필리무맙과 니볼루맙의 조합이 과돌연변이 재발성 교모세포종 환자에서 수정된 신경종양 반응 평가(RANO) 기준에 의해 평가된 종양 반응률을 증가시키는지 여부를 결정하기 위함.
2차 목표:
I. 이필리무맙 및 니볼루맙으로 치료받은 과돌연변이, 재발성 교모세포종 환자의 전체 생존 분포, 중간 생존율 및 1년 생존율을 추정합니다.
II. 이필리무맙 및 니볼루맙으로 치료받은 과돌연변이, 재발성 교모세포종 환자의 무진행 생존 분포 및 무진행 생존 중앙값을 추정합니다.
III. 이필리무맙 및 니볼루맙으로 치료받은 과돌연변이, 재발성 교모세포종 환자의 부작용 프로파일을 결정합니다.
탐구 목표:
I. 종양 미세환경에서 PD-L1 또는 면역학적 침윤이 객관적인 종양 반응, 전체 생존, 무진행 생존, 종양 돌연변이 부담 또는 3등급 이상의 부작용 비율과 관련이 있는지 여부를 테스트합니다.
II. MGMT 상태, 현미부수체 불안정성(MSI) 상태, 돌연변이 서명 또는 생식계열 분석을 포함한 종양 돌연변이 부담(TMB)의 양이 객관적인 종양 반응, 전체 생존, 무진행 생존 또는 3등급 이상의 부작용 비율과 관련이 있는지 여부를 테스트합니다. 이벤트.

개요:
환자는 제1일에 30분에 걸쳐 니볼루맙 정맥주사(IV) 및 30분에 걸쳐 이필리무맙 IV를 투여받습니다. 치료는 질병 진행 또는 허용할 수 없는 독성이 없는 경우 4주기 동안 3주마다 반복됩니다. 그런 다음 환자는 1일차에 30분에 걸쳐 니볼루맙 IV를 투여받습니다. 주기는 질병 진행이나 허용할 수 없는 독성이 없는 경우 4주마다 반복됩니다.

별아교세포종, IDH-돌연변이 CNS WHO 등급 4
- 미만성 성상세포 신경아교종 IDH-돌연변이(빈번한 ATRX 및/또는 TP53 돌연변이 및 1p/19q 결손이 없음), 괴사 및/또는 미세혈관 증식 또는 CDKN2A 및/또는 CDKN2B의 동형접합 결실을 나타내는 낮은 등급의 조직학적 특징이 있는 것
- 참고: 자격 기준은 WHO 2021 병리학 분류 변경의 새로운 진단 언어를 포함하도록 변경되었습니다. 따라서 위의 진단은 변경 사항을 반영하고 이전에 자격이 있었지만 이제 업데이트된 병리학적 분류를 수반하는 항목을 포함합니다.

활동성 자가면역 질환이 없거나 자가면역 질환의 병력이 없음
- 여기에는 면역 관련 신경계 질환, 다발성 경화증, 자가면역(탈수초성) 신경병증, 길랭-바레 증후군, 중증 근무력증의 병력이 있는 환자가 포함되나 이에 국한되지 않습니다. 전신성 홍반성 루푸스(SLE), 결합 조직 질환, 경피증, 염증성 장 질환(IBD), 크론병, 궤양성 대장염, 간염과 같은 전신 자가면역 질환; 독성 표피 괴사(TEN), 스티븐스-존슨 증후군 또는 인지질 증후군의 병력이 있는 환자는 질병의 재발 또는 악화 위험 때문에 제외되어야 합니다.
- 생리적 코르티코스테로이드를 포함한 대체 호르몬으로 관리되는 갑상선염을 포함한 백반증, 내분비 결핍증이 있는 환자가 적합합니다. 류마티스 관절염 및 기타 관절병증, 쇼그렌 증후군 및 건선이 국소 약물로 조절되는 환자 및 항핵항체(ANA), 항갑상선 항체와 같은 양성 혈청을 보이는 환자는 표적 장기 침범의 존재 및 전신 치료의 잠재적 필요성에 대해 평가되어야 합니다. 그러나 그렇지 않으면 자격이 있어야합니다

실험적: 치료(니볼루맙, 이필리무맙)
환자는 제1일에 30분에 걸쳐 니볼루맙 IV를, 30분에 걸쳐 이필리무맙 IV를 투여받습니다.
치료는 질병 진행 또는 허용할 수 없는 독성이 없는 경우 4주기 동안 3주마다 반복됩니다.
