En randomisert fase III-studie av Interferon Alfa-2B eller Interferon Alfa-2B Plus Bevacizumab hos pasienter med avansert nyrekarsinom
RASIONAL: Biologiske terapier, som interferon alfa-2b, kan forstyrre veksten av tumorceller. Bevacizumab kan stoppe veksten av tumorceller ved å stoppe blodstrømmen til svulsten. Det er ennå ikke kjent om interferon alfa-2b er mer effektivt med eller uten bevacizumab ved behandling av avansert nyrecellekarsinom (nyrekreft).
FORMÅL: Denne randomiserte fase III-studien studerer interferon alfa-2b og bevacizumab for å se hvor godt de virker sammenlignet med interferon alfa-2b alene ved behandling av pasienter med avansert nyrecellekarsinom.

- Sammenlign den totale overlevelsen for pasienter med avansert nyrecellekarsinom behandlet med interferon alfa-2b alene eller interferon alfa-2b med bevacizumab.
Sekundær
- Sammenlign tiden til sykdomsprogresjon og objektive responsrater hos pasienter behandlet med disse regimene.
- Bestem toksisiteten til interferon alfa-2b i kombinasjon med bevacizumab hos disse pasientene.
OVERSIKT: Dette er en randomisert, multisenterstudie. Pasientene er stratifisert i henhold til tidligere nefrektomi (ja vs nei) og antall risikofaktorer for sykdomsprogresjon (0 vs 1-2 vs 3 eller mer). Pasientene er randomisert til 1 av 2 behandlingsarmer.
- Arm I: Pasienter får interferon alfa-2b subkutant (SC) tre ganger i uken.
- Arm II: Pasienter får interferon alfa-2b som i arm I og bevacizumab IV over 30-90 minutter på dag 1 og 15.
I begge armer gjentas kursene hver 28. dag i fravær av sykdomsprogresjon eller uakseptabel toksisitet.
Pasientene følges hver 3. måned i 2 år og deretter årlig i opptil 10 år etter studiestart.
PROSJERT PASSERING: Totalt 700 pasienter (350 per behandlingsarm) vil bli påløpt for denne studien innen 3 år.

SYKDOMENS KARAKTERISTIKA:
Histologisk eller cytologisk bekreftet nyrecellekarsinom (RCC)
- Konvensjonell klarcellet karsinom
- Metastatisk eller uoperabel sykdom
Følgende egenskaper og cellulære typer er ekskludert:
- Ekte papillær
- Sarkomatoide funksjoner uten en klar cellekomponent
- Kromofobe
- Onkocytom
- Samlekanalsvulst
- Overgangscellekarsinom
Målbar eller ikke-målbar sykdom, inkludert noen av følgende:
- Unidimensjonalt målbar lesjon ≥ 20 mm ved konvensjonelle teknikker (f.eks. fysisk undersøkelse eller røntgen av thorax) ELLER 10 mm ved spiral CT-skanning eller MR
Følgende regnes som ikke-målbar sykdom:
- Små lesjoner
- Benlesjoner
- Leptomeningeal sykdom
- Ascites
- Pleural/pericardial effusjon
- Lymfangitt cutis/pulmonis
- Abdominale masser som ikke er bekreftet og etterfulgt av bildeteknikker
- Cystiske lesjoner
- Bestrålte lesjoner, med mindre progresjon er dokumentert etter strålebehandling
- RCC parafinvevsblokker eller ufargede objektglass må være tilgjengelige
- Ingen bevis for tidligere eller samtidige CNS-metastaser ved MR- eller CT-skanning

- Proteinuri ≥ 2+ tillatt forutsatt at protein er < 2 g/24-timers urinsamling
Kardiovaskulær
- Ingen dyp venetrombose det siste året
- Ingen cerebrovaskulær ulykke det siste året
- Ingen perifer karsykdom med claudicatio på < 1 blokk
- Ingen ukontrollert hypertensjon definert som blodtrykk ≥160 mm Hg (systolisk) og/eller ≥ 90 mm Hg (diastolisk) under medisinering
- Ingen New York Heart Association klasse II-IV kongestiv hjertesvikt
- Ingen angina pectoris som krever nitratbehandling

- Ingen annen signifikant hjerte- og karsykdom

- Ikke gravid eller ammende
- Negativ graviditetstest
- Fertile pasienter må bruke effektiv prevensjon under og i 3 måneder etter studiebehandling
- Ingen eksisterende skjoldbruskkjertelavvik der normal skjoldbruskkjertelfunksjon ikke kan opprettholdes med medisiner
- Ingen forsinket sårheling, sår eller benbrudd
- Ingen ukontrollert psykiatrisk lidelse
- Ingen annen for øyeblikket aktiv* malignitet bortsett fra ikke-melanom hudkreft MERK: *Sykdommen anses ikke for øyeblikket aktiv hvis pasienten fullførte kreftbehandling og anses å ha < 30 % risiko for tilbakefall
FØR SAMTIDIG TERAPI:
Biologisk terapi
- Ingen tidligere systemisk immunterapi for RCC
- Ingen tidligere behandling med thalidomid, anti-vaskulær endotelial vekstfaktor (VEGF), VEGF-reseptorhemmere eller antiangiogene behandling av noe slag
- Ingen samtidig profylaktisk filgrastim (G-CSF) eller sargramostim (GM-CSF)
Kjemoterapi
- Ingen tidligere systemisk kjemoterapi for RCC
- Ingen samtidig kjemoterapi
Endokrin terapi
Ingen samtidig systemisk kortikosteroidbehandling bortsett fra følgende:
- Aktuelle og inhalerte steroider
- Erstatningsterapi for binyrebarksvikt
- Ingen samtidige hormoner bortsett fra de som administreres for ikke-sykdomsrelaterte tilstander (f.eks. insulin for diabetes)
Strålebehandling

- Minst 4 uker siden tidligere strålebehandling og ble frisk
- Tidligere palliativ strålebehandling mot metastatiske lesjoner tillatt forutsatt at minst 1 målbar eller ikke-målbar lesjon forblir ubehandlet
- Ingen samtidig palliativ strålebehandling
Kirurgi
- Minst 4 uker siden forrige større operasjon og ble frisk
Annen
- Ingen annen tidligere systemisk undersøkelsesterapi for RCC
- Ingen annen tidligere adjuvant eller neoadjuvant systemisk terapi for RCC
- Ingen samtidig fulldose oral eller parenteral antikoagulasjon* MERK: *Lavdose (1 mg) warfarin for vedlikehold av kateterets åpenhet og/eller daglig profylaktisk aspirin er tillatt
