Postoperativ kjemoterapi med eller uten bevacizumab for pasienter med stadium II eller III rektal kreft

Intergruppe randomisert fase III studie av postoperativ oksaliplatin, 5-fluorouracil og leucovorin vs oksaliplatin, 5-fluorouracil, leucovorin og bevacizumab for pasienter med stadium II eller III rektal kreft som får preoperativ kjemoradiasjon

Legemidler som brukes i kjemoterapi, som oxaliplatin, leukovorin og fluorouracil, virker på forskjellige måter for å stoppe veksten av tumorceller, enten ved å drepe cellene eller ved å stoppe dem fra å dele seg. Monoklonale antistoffer, som bevacizumab, kan blokkere tumorvekst på forskjellige måter. Noen blokkerer tumorcellenes evne til å vokse og spre seg. Andre finner svulstceller og hjelper til med å drepe dem eller bære svulstdrepende stoffer til dem. Bevacizumab kan også stoppe veksten av tumorceller ved å blokkere blodstrømmen til svulsten. Å gi mer enn ett legemiddel (kombinasjonskjemoterapi) sammen med bevacizumab etter operasjonen kan drepe eventuelle tumorceller som er igjen etter operasjonen. PRIMÆRE MÅL:

I. Evaluer toleranse for behandling, sviktmønstre og sykdomsfri overlevelse hos pasienter behandlet med disse regimene

UNDERSØKENDE MÅL:

I. Vurder langvarig rektalfunksjon ved å bruke spørreskjemaet Pasient Tarmfunksjon/Uniscale og underskalaen Functional Assessment of Cancer (FACT)-Diarrhea hos pasienter behandlet med disse regimene.

II. Valider FACT-Diarrhea subscale. III. Vurder langsiktige symptomer på oksaliplatin-relatert nevrotoksisitet ved å bruke FACT/Gynecologic Oncology Group (GOG)-nevrotoksisitetssubskalaen hos pasienter behandlet med disse regimene.

IV. V. Korreler tumormolekylære prognostiske markører (kromosom 18q allelisk tap og MSI) med overlevelse hos pasienter behandlet med dette regimet.

OVERSIKT:

Dette er en randomisert studie. Pasientene er stratifisert i henhold til Eastern Cooperative Oncology Group (ECOG) ytelsesstatus (0 vs 1), klinisk stadieinndeling (høy risiko [T3, N+, M0 eller T4, enhver N, M0] vs lav risiko [T1-2, N+, M0 eller T3, N0, M0]), tidligere preoperativ oksaliplatin (ja vs nei) og tidligere strålebehandlingsdose (40-50 Gy vs > 50-55,8 Gy). Pasientene blir randomisert til 1 av 2 behandlingsarmer i forholdet 1:1.

ARM I: Pasienter får oksaliplatin intravenøst (IV) over 2 timer, leucovorin kalsium IV over 2 timer, og fluorouracil IV på dag 1 etterfulgt av fluorouracil IV kontinuerlig over 46 timer på dag 1 og 2. Behandlingen gjentas hver 2. uke i opptil 12. kurs* i fravær av sykdomsprogresjon eller uakseptabel toksisitet.

ARM II: Pasienter får bevacizumab** IV over 30-90 minutter på dag 1. Pasienter får også oksaliplatin, leukovorin kalsium og fluorouracil som i arm I*.

Pasienter fyller ut 10-15 minutters spørreskjema om tarmfunksjon ved randomisering, behandlingsslutt, 12 måneder etter behandling og deretter årlig til 5 år etter behandling.

Etter avsluttet studiebehandling følges pasientene periodisk i ca. 10 år.

Histologisk bekreftet adenokarsinom i endetarmen som oppfyller 1 av følgende kliniske (f.eks. før neoadjuvant terapi) eller patologiske stadiekriterier:
- T3, N+, M0
- T3, N0, M0
- T4, N0, M0
- Enhver T, N1-2, M0
T4, N0-2, M0 sykdom må oppfylle 1 av følgende kriterier:
- Klinisk fiksert tumor ved rektal undersøkelse med tumor adherent til bekkensideveggen eller korsbenet
- Hydronefrose på computertomografi (CT) skanning eller intravenøst pyelogram (IVP)
- Ureterisk eller blæreinvasjon som dokumentert ved cystoskopi og cytologi eller biopsi
- Invasjon i prostata
- Vaginal eller livmorpåvirkning
Må ha gjennomgått fullstendig tumorreseksjon for >= 28 dager siden og kunne begynne behandlingen innen dag 56
Må ha gjennomgått samtidig neoadjuvant kjemoradioterapi*
- MERK: *Neoadjuvant kjemoradioterapi mottatt etter protokoll NSABP-R-04 tillatt forutsatt at den oppfyller disse kriteriene
Må ha gjennomgått tidligere strålebehandling ved 40-55,8 Gy** OG fått 1 av følgende kjemoterapiregimer:
- Kontinuerlig infusjon av fluorouracil med eller uten oksaliplatin; fluorouracil og leucovorin kalsium
- Capecitabin med eller uten oksaliplatin; capecitabin med eller uten oksaliplatin ELLER en kontinuerlig infusjon av fluorouracil med eller uten oksaliplatin mottatt på protokoll NSABP-R-04
- MERK: **Intensitetsmodulert strålebehandling tillatt
ECOG ytelsesstatus 0-1
Blodplateantall >= 100 000/mm^3
Absolutt antall granulocytter >= 1500/mm^3
Bilirubin normalt (med mindre kronisk grad 1 bilirubinøkning på grunn av Gilberts sykdom eller lignende syndrom på grunn av langsom konjugering av bilirubin)
Alkalisk fosfatase (AP) < 2,5 ganger øvre normalgrense (ULN) og aspartataminotransferase (AST) < 1,5 ganger ULN
Hepatitt B og C negativ (for pasienter med AP > normal) med mindre de er vaksinert tidligere
Serumkreatinin =< 1,5 ganger ULN
Urinprotein:kreatinin (UPC)-forhold < 1,0 ELLER urinprotein < 1 g ved 24-timers urinsamling
International Normalized Ratio (INR) =< 1,5
INR > 1,5 tillatt forutsatt at pasienten er på full dose antikoagulantia OG oppfyller alle følgende kriterier:
- INR i området (dvs. mellom 2 og 3) på en stabil dose warfarin eller lavmolekylært heparin
- Ingen aktiv blødning eller patologisk tilstand som er forbundet med høy risiko for blødning
Negativ graviditetstest
Fertile pasienter må bruke effektiv prevensjon under og i minst 3 måneder etter studiebehandling
Ingen annen tidligere eller samtidig malignitet bortsett fra ikke-melanom hudkreft, brystkreft in situ, karsinom in situ i livmorhalsen eller tidligere behandlet ikke-bekkenkreft som har vært sykdomsfri i > 5 år
Pasienter med en historie med brystkreft (uten tegn på sykdom) som forblir på hormonbehandling i > 5 år er kvalifisert
Pasienter med hypertensjon i anamnesen må ha blodtrykk < 150/90 mm Hg OG være på et stabilt regime med antihypertensiv behandling
Ingen annen tidligere kjemoterapi eller bekkenstrålebehandling bortsett fra som neoadjuvant behandling for nåværende diagnose av endetarmskreft
Samtidig deltakelse på protokoll NSABP-R-04 tillatt

Ekskluderingskriterier:

Gravid eller ammende
Bevis på metastatisk sykdom ved kirurgisk/intraoperativ undersøkelse
Bevis på metastatisk sykdom bekreftet ved CT-skanning, magnetisk resonansavbildning (MRI), eller ultralyd av leveren eller CT-skanning av brystet eller røntgen av thorax i løpet av de siste 6 månedene
Bevis på svulst utenfor bekkenet, inkludert levermetastaser, peritoneal seeding eller metastatisk inguinal lymfadenopati
Samtidig større operasjon
Aktiv blødning som ikke er relatert til den primære rektaltumoren i løpet av de siste 6 månedene
