BAKGRUNN: Kilereseksjon eller segmentektomi kan være mindre invasive typer kirurgi enn lobektomi for ikke-småcellet lungekreft og kan ha færre bivirkninger og forbedre restitusjonen. OVERSIKT: Dette er en multisenter, randomisert studie. Pasientene er stratifisert etter tumorstørrelse (< 1 cm vs 1-1,5 cm vs > 1,5-2,0 cm) (basert på maksimal dimensjon bestemt fra den preoperative CT-skanningen), histologi (plateepitelkarsinom vs adenokarsinom vs annet), og røykestatus (aldri røykt [røykt < 100 sigaretter i løpet av livet] vs tidligere røyker [røykt > 100 sigaretter OG slutte for ≥ 1 år siden] vs nåværende røyker [slutte for < 1 år siden eller røyker for øyeblikket]). Hovedmål:

For å bestemme om DFS etter sublobar reseksjon (segmentektomi eller kile) er ikke-underordnet etter lobektomi hos pasienter med liten perifer (≤ 2 cm) NSCLC.

Sekundære mål:

  1. For å avgjøre om total overlevelse (OS) (etter sublobar reseksjon) er ikke dårligere enn etter lobektomi.
  2. For å bestemme frekvensen av loko-regional og systemisk tilbakefall (eksklusive andre primære) etter lobar og sublobar reseksjon.
  3. For å bestemme forskjellen mellom de to armene av studien i lungefunksjon som bestemt ved ekspiratoriske strømningshastigheter målt 6 måneder postoperativt.
  4. Imaging Substudie: For å utforske forholdet mellom karakteristika ved primær lungekreft, som avslørt av preoperativ CT og PET-avbildning, og utfall; en bestemmelse av den falske negative frekvensen av den preoperative PET-skanningen for identifikasjon av involverte hilarand mediastinale lymfeknuter; og en vurdering av nytten av årlig oppfølging CT-avbildning etter kirurgisk reseksjon av små stadium IA NSCLC.

Kvalifikasjonskriterier for forhåndsregistrering:

  1. Perifer lungeknute ≤ 2 cm ved preoperativ CT-skanning og antatt å være lungekreft. Sentrum av svulsten, som sett på CT, må være lokalisert i den ytre tredjedelen av lungen i enten det tverrgående, koronale eller sagittale planet. Pasienter med ren malt glassopasitet eller patologisk bekreftet N1- eller N2-sykdom er ikke kvalifisert.
  2. Svulstlokaliseringen må være egnet for enten lobar eller sublobar reseksjon (kile eller segment).
  3. ECOG-ytelsesstatus på 0-2.
  4. Ingen tidligere malignitet innen 3 år annet enn ikke-melanom hudkreft, overfladisk blærekreft eller CIS i livmorhalsen.
  5. Ingen tidligere kjemoterapi eller strålebehandling for denne maligniteten.
  6. Ingen tegn på lokalt avansert eller metastatisk sykdom.
  7. Alder ≥ 18 år

Kvalifikasjonskriterier for intraoperativ randomisering:

  1. Histologisk bekreftelse av NSCLC (hvis ikke allerede oppnådd)
  2. Bekreftelse av N0-status ved fryseseksjonsundersøkelse. Høyresidige svulster krever at nodenivåene 4, 7 og 10 tas prøver og diagnostiseres som negative på frosset snitt. Venstresidige svulster krever at nodenivåene 5 eller 6, 7 og 10 tas prøver og diagnostiseres som negative på frosset snitt. Nivå 4 og 7 noder kan prøves ved mediastinoskopi, endobronkial ultralyd (EBUS) og/eller endoskopisk ultralyd (EUS), eller på tidspunktet for torakotomi eller VATS-utforskning. Noder som tidligere er tatt med mediastinoskopi (eller EBUS og/eller EUS) enten umiddelbart før eller innen 6 uker etter den endelige kirurgiske prosedyren (torakotomi eller VATS) trenger ikke tas på nytt.
