Randomizovaná studie fáze III 5-fluoruracilu, mitomycinu-C a radioterapie versus 5-fluoruracil, cisplatina a radioterapie u karcinomu análního kanálu

ODŮVODNĚNÍ: Léky používané v chemoterapii používají různé způsoby, jak zastavit dělení nádorových buněk, takže přestanou růst nebo zemřou. Kombinace více než jednoho léku nebo kombinace radiační terapie s chemoterapií může zabít více nádorových buněk. Dosud není známo, zda fluorouracil a mitomycin plus radiační terapie jsou účinnější než fluorouracil a cisplatina plus radiační terapie u rakoviny konečníku.

ÚČEL: Tato randomizovaná studie fáze III studuje fluorouracil a mitomycin plus radiační terapii, aby se zjistilo, jak dobře funguje ve srovnání s fluorouracilem a cisplatinou plus radiační terapií při léčbě pacientů s rakovinou konečníku stadia II nebo stadia III.

CÍLE:

Porovnejte počáteční a celkovou míru lokálního a vzdáleného selhání u pacientů s karcinomem análního kanálu léčených buď fluorouracilem (5-FU) plus mitomycin souběžně s radioterapií nebo 5-FU plus cisplatina následovaná 5-FU plus cisplatinou souběžně s radioterapií.
Identifikujte jakékoli rozdíly v lokální kontrole a četnosti kolostomií po 2 letech u pacientů léčených těmito režimy.
Určete jakýkoli rozdíl v přežití bez kolostomie, bez onemocnění nebo v celkovém přežití u pacientů léčených těmito režimy.
Porovnejte toxické účinky těchto režimů u těchto pacientů.
Vyhodnoťte prognostické účinky nadměrné exprese nádorových markerů P53, stavu lidského papilomaviru a enzymu HAP1 u pacientů léčených těmito režimy.

PŘEHLED: Toto je randomizovaná studie. Pacienti jsou stratifikováni podle pohlaví, stavu uzlin (pozitivní vs. negativní) a velikosti primárního tumoru (větší než 2 cm až 5 cm vs. větší než 5 cm). Pacienti jsou randomizováni do 1 ze 2 léčebných ramen.

Rameno I: Pacienti dostávají fluorouracil (5-FU) IV nepřetržitě po dobu 96 hodin počínaje 1. a 29. dnem a mitomycin IV 1. a 29. den se současnou radioterapií.
Rameno II: Pacienti dostávají indukční chemoterapii obsahující 5-FU IV nepřetržitě po dobu 96 hodin počínaje dnem 1, 29, 57 a 85 a cisplatinou IV po dobu 1 hodiny ve dnech 1, 29, 57 a 85. Počínaje dnem 57 dostávají pacienti souběžnou radioterapii.

V obou ramenech je radioterapie podávána denně, 5 dní v týdnu, po dobu 5–6,5 týdne. Pacienti s lézemi T3, T4 nebo N+ nebo lézemi T2 s reziduálním onemocněním dostávají další radioterapii do redukovaného pole.

Pacienti jsou sledováni každé 3 měsíce po dobu 1 roku, každých 6 měsíců po dobu 1 roku a poté každý rok.

PŘEDPOKLADANÝ AKRUÁLNÍ AKRUÁLNÍ: Do této studie bude během 5 let získáno celkem 650 pacientů.

Histologicky potvrzený primární skvamózní, bazaloidní nebo kloakogenní karcinom análního kanálu, jiný než karcinom in situ
- T2-4, libovolný N, M0 (fáze II nebo III)
Žádná lokální nebo regionální recidiva po lokální excizi nebo abdominální peritoneální resekci
