Odhaduje se, že osteoartróza postihuje polovinu dospělé populace a ve věku 75 let se předpokládá, že 85 ze 100 starších pacientů bude mít nějakou formu tohoto onemocnění. Osteoartritida (OA) je charakterizována střední až silnou bolestí, která omezuje funkční schopnosti. Existuje podezření, že příčiny této oslabující bolesti zahrnují podráždění synovie, nadměrné síly subchondrální kosti a zvýšený intraarteriální tlak. Koleno je nejčastěji postiženým váhonosným kloubem. Osteoartróza má velký společenský dopad jako druhý nejčastější důvod pracovní neschopnosti ve Spojených státech. Očekává se, že prevalence OA v příštích letech vzroste v důsledku prodlužování dlouhověkosti, zvýšeného podílu populace ve věku nad 75 let a zvýšené diagnostické schopnosti.
Spíše než další hodnocení různých forem artroskopické chirurgie (tj. laváž vs. debridement vs abraze) při léčbě OA kolena považujeme za důležité stanovit účinnost této technologie ve srovnání s nejlepší nechirurgickou léčbou samotnou.
Toto je hodnotitelem zaslepená, fáze III, jednocentrová, skupinová sekvenční randomizovaná kontrolovaná studie u pacientů s osteoartrózou kolene.
Účinnost léčby je založena na kvalitě života pacientů po 2 letech specifické pro onemocnění pomocí indexu osteoartrózy Western Ontario McMaster (WOMAC). Kromě toho se posuzují fyzické funkce pacientů a efektivita nákladů. Náklady budou odhadnuty z pohledu systému zdravotní péče, pacienta, rodiny a společnosti a budou zahrnovat využití nemocničních a jiných zdrojů a také ekonomické důsledky ztracených pracovních dnů spojených s těmito alternativními způsoby léčby.
Tato randomizovaná klinická studie poskytne vysoce kvalitní důkazy k určení role této nákladné léčby v celkové léčbě OA.

Kritéria pro zařazení:
- Idiopatická nebo sekundární osteoartritida kolena diagnostikovaná klasifikačním stromem Altman et al 2 - 83 procent senzitivita, 93 procent specificita
- Stupně 2 až 4 závažnosti OA podle radiografického hodnocení - modifikovaný klasifikační systém Kellgren a Lawrence
- Věk vyšší než 18.
Kritéria vyloučení:
- Zánětlivá nebo postinfekční artritida kolena
- Předchozí artroskopie pro léčbu OA kolena
- Izolovaný mediální kompartment OA stupeň 3-4 s mechanickou varozitou větší než 5 stupňů – ideální kandidát pro vysokou tibiální osteotomii
- Izolovaný laterální kompartment OA stupeň III-IV s mechanickou valgozitou větší než 5 stupňů – ideální kandidát pro vysokou tibiální osteotomii nebo distální osteotomii femuru
- OA 4. stupně v minimálně 2 kompartmentech a starší 60 let – ideální kandidát na totální endoprotézu kolene
- Injekce kortikosteroidů během posledních tří měsíců.
- Velký neurologický deficit
- Závažné zdravotní onemocnění – očekávaná délka života kratší než 2 roky nebo s nepřijatelně vysokým operačním rizikem.
- Těhotná
- Neumí mluvit nebo číst anglicky
- Psychiatrické onemocnění, které omezuje informovaný souhlas
- Neochotný být sledován po dobu 2 let
