Si stima che l'osteoartrosi colpisca la metà della popolazione adulta e, all'età di 75 anni, si prevede che 85 pazienti anziani su 100 avranno una qualche forma di questa malattia. L'osteoartrosi (OA) è caratterizzata da dolore da moderato a severo che limita la capacità funzionale. Si sospetta che le cause di questo dolore debilitante includano irritazione della sinovia, eccessive forze ossee subcondrali e aumento della pressione intra-arteriosa. Il ginocchio è l'articolazione portante più comunemente colpita. L'artrosi ha un grande impatto sociale, essendo la seconda causa più comune di disabilità lavorativa negli Stati Uniti. Si prevede che la prevalenza di OA aumenterà nei prossimi anni a causa della maggiore longevità, dell'aumento della percentuale della popolazione di età superiore ai 75 anni e della maggiore capacità diagnostica.
Piuttosto che ulteriori valutazioni di diverse forme di chirurgia artroscopica (es. lavaggio vs debridement vs abrasione) nel trattamento dell'OA del ginocchio, riteniamo importante stabilire l'efficacia di questa tecnologia rispetto al solo miglior trattamento non chirurgico.
Si tratta di uno studio controllato, randomizzato, sequenziale, randomizzato, di fase III, monocentrico, in cieco con valutatore in pazienti con osteoartrosi del ginocchio.
L'efficacia del trattamento si basa sulla qualità di vita specifica della malattia dei pazienti a 2 anni utilizzando il Western Ontario McMaster (WOMAC) Osteoarthritis Index. Sono inoltre in fase di valutazione la funzione fisica dei pazienti e l'efficacia in termini di costi. I costi saranno stimati dal punto di vista del sistema sanitario, del paziente, della famiglia e della società e includeranno l'uso dell'ospedale e di altre risorse, nonché le implicazioni economiche dei giorni di lavoro persi associati a questi trattamenti alternativi.
Questo studio clinico randomizzato fornirà prove di alta qualità per determinare quale sia il ruolo di questo costoso trattamento nella gestione complessiva dell'OA.

Criterio di inclusione:
- Artrosi idiopatica o secondaria del ginocchio diagnosticata da Altman et al. 2 albero di classificazione -83% di sensibilità, 93% di specificità
- Gradi da 2 a 4 di gravità dell'OA secondo la valutazione radiografica - sistema di classificazione Kellgren e Lawrence modificato
- Età maggiore di 18 anni.
Criteri di esclusione:
- Artrite infiammatoria o post-infettiva del ginocchio
- Precedente artroscopia per il trattamento dell'artrosi del ginocchio
- Compartimento mediale isolato OA Grado 3-4 con varo meccanico superiore a 5 gradi - candidato ideale per l'osteotomia tibiale alta
- Compartimento laterale isolato OA Grado III-IV con valgismo meccanico superiore a 5 gradi - candidato ideale per osteotomia tibiale alta o osteotomia femorale distale
- Grado 4 OA in almeno 2 compartimenti e oltre 60 anni di età - candidato ideale per l'artroplastica totale del ginocchio
- Iniezione di corticosteroidi negli ultimi tre mesi.
- Deficit neurologico maggiore
- Malattia medica maggiore: aspettativa di vita inferiore a 2 anni o con rischio operatorio inaccettabilmente elevato.
- Incinta
- Incapace di parlare o leggere l'inglese
- Malattia psichiatrica che limita il consenso informato
- Non disposto a essere seguito per 2 anni
