Neuromuskulární elektrická stimulace u pacientů se srdečním selháním pro zlepšení funkčního zotavení po hospitalizaci (NeuHF-Recover)

Cílem této výzkumné studie je pochopit, zda domácí cvičební program zahájený po hospitalizaci pro HFpEF a pokračující 4 týdny po propuštění z nemocnice může zachovat nebo zlepšit fyzické funkce.

Detailní popis

Srdeční selhání je hlavní příčinou hospitalizace dospělých v USA, přičemž polovinu z těchto přijetí tvoří pacienti se zachovanou ejekční frakcí (HFpEF). U mnoha pacientů se projevuje chronická únava, nesnášenlivost cvičení a neschopnost vykonávat činnosti každodenního života, což se zhoršuje hospitalizací. Cílem této výzkumné studie je pochopit, zda domácí cvičební program zahájený brzy po hospitalizaci a pokračující 4 týdny po propuštění může zlepšit funkční zotavení u pacientů s HFpEF. Dobrovolníci budou náhodně přiřazeni k tomu, aby podstoupili neuromuskulární elektrickou stimulaci (NMES) svých čtyřhlavých svalů, nebo aby nedostali NMES (kontrolní skupina). Dobrovolníci budou hodnoceni během hospitalizace a 4 týdny po propuštění. Hodnocení bude zahrnovat měření fyzické funkce pomocí 6minutového testu chůze a Short Physical Performance Battery, stejně jako hodnocení subjektivní fyzické funkční kapacity a kvality života pomocí Medical Outcomes Short form 36 a dotazníku Kansas City Cardiomyopathy.

  • bydlet do 30 mil od lékařského centra
  • schopen poskytnout informovaný souhlas

Kritéria vyloučení:

  • revmatoidní artritidu nebo jiné zánětlivé/autoimunitní onemocnění
  • rakovina, s výjimkou nemelanomové rakoviny kůže nebo rakoviny prostaty nízkého stupně
  • těžká demence/Alzheimerova choroba
  • cvičení omezující onemocnění periferních cév
  • neuromuskulární onemocnění nebo neuromuskulární dysfunkce spojené s cerebrovaskulární příhodou
  • index tělesné hmotnosti ≥40 kg/m2
  • krevní sraženina dolních končetin nebo implantabilní kardioverter-defibrilátor nebo kardiostimulátor

Lékaři obvykle používají tento nástroj k detekci problémů při provádění činností každodenního života a k plánování péče podle toho. Index hodnotí přiměřenost výkonu v šesti funkcích – koupání, oblékání, toaleta, přenášení, kontinence a krmení. Klienti jsou hodnoceni ano/ne za nezávislost v každé ze šesti funkcí. The Lawton Instrumental Activities of Daily Living Scale (IADL) is an appropriate instrument to assess independent living skills. These skills are considered more complex than the basic activities of daily living as measured by the Katz Index of ADLs. The instrument is most useful for identifying how a person is functioning at the present time, and to identify improvement or deterioration over time. There are eight domains of function measured with the Lawton IADL scale. Women are scored on all 8 areas of function; historically, for men, the areas of food preparation, housekeeping, laundering have been excluded. Since men regularly complete food preparation, housekeeping, and laundering tasks, they were scored in all 8 domains as well.

Clients are scored according to their highest level of functioning in that category. A summary score ranges from 0 (low function, dependent) to 8 (high function, independent) for women and men.
