バックグラウンド:
喫煙は慢性気流閉塞の発症における最も重要な要因として認識されていますが、臨床疾患を発症する喫煙者はほんの少数です。 1985 年、慢性気流閉塞の発症を促進する可能性のある因子として、気道の反応性レベルの上昇とアトピーの役割に関心が集まりました。 その後、現在の疫学研究では、これらの潜在的な危険因子のうちの 1 つに関するデータしか得られていないか、関連する問題に適切に対処するには小さすぎます。 本研究では、気道の反応性とアトピーの両方を評価します。
デザインの物語:
この研究では、1963年に始まった退役軍人局の規範的高齢化研究(NAS)の人口とデータベースが使用された。 NAS の人口は、もともと健康のために選ばれた 1,900 人を超える男性とその妻で構成されています。 NAS は現在、喫煙歴に関する質問票、NHLBI-ATS の呼吸器症状と病気の質問票、および 3 年おきの定期検査で肺活量測定を実施しています。 今回の研究では、NAS参加者が次の2回の検査に戻った際に、メタコリン吸入に対する反応で測定される気道の反応性と、皮膚検査、血液好酸球増加症、血清IgEで測定されるアトピーを評価した。 このデータは、気道の反応性、アトピー、喫煙と呼吸器症状との関係を横断的および縦断的に調べるために使用されました。
この研究は、肺機能の低下速度に影響を与える因子を前向きに特定し、高齢者の母集団サンプルにおける慢性閉塞性肺疾患(COLD)と喘息の予測因子を特定するために、1996年に更新されました。 研究者らは、高齢者におけるFEV1の加速的な低下と呼吸器症状の発生は、タバコの煙による傷害と吸入空気アレルゲンに対するIgE媒介過敏症という2つの環境曝露によって決定されるという仮説を立てている。 アレルゲンへの曝露と気道の反応性が、高齢者の固定的な気流閉塞にどの程度つながるかは不明です。 対象となる主なアレルゲンには、イエダニ (Der p I、Der fI)、ゴキブリ (Bla g I、Bla g II)、ネコ (Fel d I)、および真菌が含まれます。 気管支収縮刺激に対する気道過敏性は、気道疾患の症状として、また気道疾患を発症しやすくその臨床パターンを決定する固有の宿主特性として考えられる生理学的特徴です。 彼らは、呼吸器症状と病気、喫煙、肺機能、気道の反応性、アトピーの指標 (皮膚検査、総 IgE および抗原特異的 IgE)、炎症の指標 (末梢血中の好酸球と白血球の数)、次の仮説に対処するために、NAS 参加者とその妻の塵と空気中のアレルゲン レベルを調査しました。気道反応性の増加と肺機能の縦方向の低下の加速との関連性は、性別、喫煙、FEV1 のレベル、および皮膚検査の反応性によって変化するというものです。 一般的な屋内アレルゲンへの曝露レベルが肺機能の長期的な低下速度に影響を与えること。一般的な屋内アレルゲンへの高レベルの曝露は、時間の経過とともに気道の反応性の長期的な増加につながること。そして、真菌(培養可能で数えられる生物)および抗原、ゴキブリ(Bla g l、Bla g II]、およびネコ(Fel d I)の沈降粉塵レベルは、これらの病原体の空中浮遊レベルと相関していると考えられます。
