기본 목표:

I. 아벨루맙 + 세툭시맙으로의 치료가 아벨루맙 단독에 비해 무진행 생존(PFS)을 연장하는지 여부를 평가하기 위함.

2차 목표:

I. 각 치료군의 확인된 객관적 반응률을 평가하기 위함. II. 각 치료 부문의 임상적 혜택 비율을 평가합니다. III. 단일 제제 아벨루맙으로 진행된 환자에서 세툭시맙 + 아벨루맙의 무진행생존(PFS)을 평가합니다.

IV. 각 치료군에 대한 전체 생존(OS)을 평가하기 위해. V. 아벨루맙 플러스 세툭시맙 및 아벨루맙 단독의 치료 아암 전반에 걸쳐 독성을 평가하기 위함.

개요: 환자는 2개의 팔 중 1개로 무작위 배정됩니다.

ARM I: 환자는 1일과 15일에 60분에 걸쳐 정맥 주사(IV)로 아벨루맙을 투여받습니다. 치료는 질병 진행 또는 허용할 수 없는 독성이 없는 경우 최대 24주기 동안 28일마다 반복됩니다. 아벨루맙 실패 환자는 II군으로 넘어갈 것입니다.

  • • 생검으로 입증된 진행성 피부 편평 세포 암종. 진행성 질환은 전이성 피부 편평 세포 암종 또는 근치적 수술적 절제가 불가능한 국소 진행성 피부 편평 세포 암종 또는 환자가 수술적 절제를 거부하는 것으로 정의됩니다.
  • 환자는 RECIST(Response Evaluation Criteria in Solid Tumors) 1.1에 따라 측정 가능한 질병인 병변이 하나 이상 있어야 합니다.

제외 기준:

  • 진행성 피부 편평 세포 암종(cSCC)에 대한 완화 치료로서 이전에 세툭시맙으로 치료를 받은 환자는 제외됩니다. 국소 진행성 질환에 대한 이전 치료로 세툭시맙 기반 화학방사선 요법(최종 또는 보조)을 받은 환자는 등록 전 >= 6개월 전에 마지막 용량을 투여한 경우 자격이 있습니다.
  • 이전에 cetuximab을 투여받았고 cetuximab의 중단을 요하는 심각한 주입 반응이 있었던 환자는 제외됩니다.
  • 이전에 항 PD-1 또는 항 PD-L1 단클론 항체(mAb)로 치료받은 환자는 제외됩니다.

A patient will be declared a success for RR if they achieve a partial response (PR) or better on two consecutive evaluations (at least 8 weeks apart). The number of successes will be divided by the number of evaluable patients to estimate the RR for each treatment arm and exact 95% confidence intervals will be calculated based on the properties of the binomial distribution.

Clinical Benefit Rate (CBR)

The CBR will be estimated as the number of successes divided by the total number of evaluable patients. An exact 95% confidence interval will be constructed around the CBR using the properties of the binomial distribution.

PFS in Patients Who Have Progressed on Avelumab Alone

Will explore the PFS for those patients that crossover to combination therapy (avelumab + cetuximab) after progressing on avelumab alone (baseline time being the time of crossover). The Kaplan-Meier method will be used to calculate a median PFS as well as a 95% confidence interval.
