Randomizovaná studie fáze II Avelumab plus Cetuximab versus samotný avelumab u pokročilého kožního spinocelulárního karcinomu kůže (cSCC)

Tato studie fáze II studuje, jak dobře funguje avelumab s cetuximabem nebo bez něj při léčbě pacientů s kožním spinocelulárním karcinomem, který se rozšířil do dalších míst v těle. Imunoterapie s monoklonálními protilátkami, jako je avelumab a cetuximab, může pomoci imunitnímu systému těla napadnout rakovinu a může narušit schopnost nádorových buněk růst a šířit se.

PRIMÁRNÍ CÍLE:

I. Zhodnotit, zda léčba avelumabem plus cetuximabem prodlužuje přežití bez progrese (PFS) ve srovnání se samotným avelumabem.

DRUHÉ CÍLE:

I. Vyhodnotit potvrzenou míru objektivní odpovědi v každém léčebném rameni. II. Vyhodnotit míru klinického přínosu každého léčebného ramene. III. Vyhodnotit PFS cetuximabu plus avelumabu u pacientů, u kterých došlo k progresi na avelumabu v monoterapii.

IV. Vyhodnotit celkové přežití (OS) pro každé léčebné rameno. V. Vyhodnotit toxicitu v léčebných ramenech avelumabu plus cetuximabu a samotného avelumabu.

Přehled: Pacienti jsou randomizováni do 1 ze 2 ramen.

ARM I: Pacienti dostávají avelumab intravenózně (IV) po dobu 60 minut ve dnech 1 a 15. Léčba se opakuje každých 28 dní po dobu až 24 cyklů v nepřítomnosti progrese onemocnění nebo nepřijatelné toxicity. Pacienti se selháním avelumabu přejdou do ramene II.

ARM II: Pacienti dostávají cetuximab IV po dobu 1-2 hodin ve dnech 1, 8, 15 a 22 a avelumab IV po dobu 60 minut ve dnech 1 a 15. Léčba se opakuje každých 28 dní po dobu až 12 cyklů pro cetuximab a 24 cyklů pro avelumab v nepřítomnosti progrese onemocnění nebo nepřijatelné toxicity.

Po dokončení studijní léčby jsou pacienti sledováni každé 3 měsíce až do progrese onemocnění, poté každých 6 měsíců po dobu až 2 let.

  • KRITÉRIA ZPŮSOBILOSTI PŘEDREGISTRACE

  • Poskytněte adekvátní tkáň pro testování PD-L1

    * Lze použít čerstvý nebo archivní papírový papír. Vzorek musí být alespoň z biopsie jádrové jehly. Aspirace jemnou jehlou není dostatečná. Předložení tohoto vzorku je povinné před registrací, protože výsledky budou použity pro stratifikaci

  • KRITÉRIA ZPŮSOBILOSTI REGISTRACE
  • • Biopticky ověřený pokročilý kožní spinocelulární karcinom. Pokročilé onemocnění je definováno buď jako metastatický kožní spinocelulární karcinom nebo lokálně pokročilý kožní spinocelulární karcinom, který nelze kurativní chirurgickou resekcí, nebo pacient chirurgickou resekci odmítne
  • Pacient musí mít alespoň jednu lézi, která je měřitelným onemocněním na základě kritérií hodnocení odpovědi u solidních nádorů (RECIST) 1.1
  • Pokud pacient podstoupil rozsáhlou operaci, musí se před zahájením léčby adekvátně zotavit z toxicity a/nebo komplikací z intervence
  • Tato studie zahrnuje zkoumanou látku, jejíž genotoxické, mutagenní a teratogenní účinky na vyvíjející se plod a novorozence nejsou známy. Proto pouze u žen ve fertilním věku je vyžadován negativní těhotenský test z moči nebo séra =< 7 dní před registrací
  • Stav výkonnosti Eastern Cooperative Oncology Group (ECOG) 0-2
  • Pokud je virus lidské imunodeficience (HIV) pozitivní, musí být virová zátěž HIV < 200 kopií/ml a počet CD4 > 200. Pokud je HIV pozitivní pacient na vysoce aktivní antiretrovirové terapii (HAART), musí být takto léčen déle než 4 týdny
  • Absolutní počet neutrofilů (ANC) >= 1500/mm^3
  • Počet krevních destiček >= 100 000/mm^3
  • Vypočtená clearance kreatininu >= 30 ml/min
  • Celkový bilirubin = < 1,5 x horní hranice normálu (ULN)
  • Aspartátaminotransferázy (AST) / alaninaminotransferázy (ALT) =< 2,5 x horní hranice normy (ULN)

Kritéria vyloučení:

  • Pacienti, kteří byli předtím léčeni cetuximabem jako paliativní léčba pokročilého kožního spinocelulárního karcinomu (cSCC), jsou vyloučeni. Pacienti, kteří dostávali chemoradiaci na bázi cetuximabu (ať už definitivní nebo adjuvantní) jako předchozí léčbu lokálně pokročilého onemocnění, jsou způsobilí, pokud byla poslední dávka podána >= 6 měsíců před registrací
  • Pacienti, kteří dříve dostávali cetuximab a měli závažnou reakci na infuzi vyžadující přerušení podávání cetuximabu, jsou vyloučeni
  • Pacienti léčení předchozími anti-PD-1 nebo anti-PD-L1 monoklonálními protilátkami (mAb) jsou vyloučeni
  • Pacienti nemohou během 2 týdnů od registrace podstoupit léčbu ozařováním nebo chemoterapií včetně jiného hodnoceného činidla. Kromě výše uvedeného pro cetuximab neexistují žádná omezení na počet linií jiných terapií podávaných pro pokročilé cSCC
  • Pacienti s „aktuálně aktivní“ druhou malignitou budou vyloučeni s výjimkou jiných nemelanomových kožních karcinomů nebo karcinomu děložního čípku in situ. Pacienti se nepovažují za „aktuálně aktivní“ maligní onemocnění, pokud dokončili léčbu a jsou bez onemocnění po dobu 3 let

  • Žádná historie následujících položek:

    • Autoimunitní onemocnění (včetně zánětlivého onemocnění střev) s výjimkou pacientů s diabetem typu I, vitiligem, psoriázou nebo onemocněním hypo- nebo hypertyreózy nevyžadujícím imunosupresivní léčbu
    • Neinfekční pneumonitida, která vyžadovala steroidy do 5 let
    • Transplantace orgánů včetně předchozí transplantace kmenových buněk
    • Příjem živé vakcíny =< 4 týdny
    • Cévní mozková příhoda/mrtvice do 6 měsíců od zařazení
    • Infarkt myokardu do 6 měsíců od zařazení
    • Aktivní nestabilní angina pectoris
    • Městnavé srdeční selhání (>= třída II klasifikace New York Heart Association)
    • Závažná srdeční arytmie vyžadující léky. Zda je arytmie považována za „vážnou“, je na uvážení vyšetřovatele
  • Aktivní infekce vyžadující systémovou léčbu

  • Užívání imunosupresivních léků =< 7 dní od registrace, S VÝJIMKOU:

    • Intranazální, inhalační, topické steroidy nebo lokální injekce steroidů (např. intraartikulární injekce)
    • Systémové kortikosteroidy ve fyziologických dávkách = < 10 mg/den prednisonu nebo ekvivalentu
    • Steroidy jako premedikace hypersenzitivních reakcí (např. počítačová tomografie [CT] premedikace)"
  • Jiné závažné akutní nebo chronické zdravotní stavy, včetně, ale bez omezení, imunitní kolitidy, plicní fibrózy nebo psychiatrických stavů včetně nedávných (během posledního roku) nebo aktivních sebevražedných myšlenek nebo chování; nebo laboratorní abnormality, které mohou zvýšit riziko spojené s účastí ve studii nebo podáváním léčby ve studii nebo mohou narušovat interpretaci výsledků studie a podle úsudku zkoušejícího by způsobily, že pacient není vhodný pro vstup do této studie

Pacienti dostávají avelumab IV po dobu 60 minut ve dnech 1 a 15. Léčba se opakuje každých 28 dní po dobu až 24 cyklů v nepřítomnosti progrese onemocnění nebo nepřijatelné toxicity. Pacienti se selháním avelumabu přejdou do ramene II.

Pacienti dostávají cetuximab IV po dobu 1-2 hodin ve dnech 1, 8, 15 a 22 a avelumab IV po dobu 60 minut ve dnech 1 a 15. Léčba se opakuje každých 28 dní po dobu až 12 cyklů pro cetuximab a 24 cyklů pro avelumab v nepřítomnosti progrese onemocnění nebo nepřijatelné toxicity.

A patient will be declared a success for RR if they achieve a partial response (PR) or better on two consecutive evaluations (at least 8 weeks apart). The number of successes will be divided by the number of evaluable patients to estimate the RR for each treatment arm and exact 95% confidence intervals will be calculated based on the properties of the binomial distribution.
