Adjuvant Nivolumab ved reseksjonert lungekreft (ANVIL) - En randomisert fase III-studie av Nivolumab etter kirurgisk reseksjon og adjuvant kjemoterapi ved ikke-småcellet lungekreft
Denne fase III behandlingsstudien med ALCHEMIST studerer hvor godt nivolumab etter kirurgi og kjemoterapi virker ved behandling av pasienter med stadium IB-IIIA ikke-småcellet lungekreft.
Immunterapi med monoklonale antistoffer, som nivolumab, kan hjelpe kroppens immunsystem til å angripe kreften, og kan forstyrre tumorcellenes evne til å vokse og spre seg.

PRIMÆRE MÅL:
I. For å evaluere om adjuvant behandling med nivolumab vil resultere i forbedret total overlevelse (OS) og/eller sykdomsfri overlevelse (DFS) i forhold til standard observasjon hos pasienter med stadium IB >= 4 cm, II og IIIA, ikke-småcellet lunge kreft (NSCLC) etter kirurgisk reseksjon og standard adjuvant terapi.
II. For å evaluere om adjuvant behandling med nivolumab vil resultere i forbedret sykdomsfri overlevelse (DFS) i forhold til standard observasjon hos pasienter med stadium IB >= 4 cm, II og IIIA, NSCLC med høyt PD-L1-ekspresjon (>= 50 % farging) etter kirurgisk inngrep reseksjon og standard adjuvant terapi.
III. For å evaluere om adjuvant behandling med nivolumab vil resultere i forbedret total overlevelse (OS) i forhold til standard observasjon hos pasienter med stadium IB >= 4 cm, II og IIIA, NSCLC etter kirurgisk reseksjon og standard adjuvant terapi.
SEKUNDÆRE MÅL:
I. For å evaluere sikkerhetsprofilen til nivolumab når det gis som adjuvant terapi.
II. For å evaluere og sammenligne sykdomsfri og total overlevelse hos pasienter med svulster som uttrykker programmert celledødsligand (PD-L)1 i ulike mønstre assosiert med nivolumab og standard observasjon.
III. For å evaluere og sammenligne sykdomsfri og total overlevelse hos pasienter med svulster som har høy mutasjonsbelastning assosiert med nivolumab og standard observasjon.
IV. For å evaluere OS og DFS etter trinn. V. Å evaluere OS og DFS etter hver stratifiseringsfaktor.
OVERSIKT: Pasienter er randomisert til 1 av 2 behandlingsarmer.
ARM I: Pasienter får nivolumab intravenøst (IV) over 30 minutter på dag 1. Sykluser gjentas hver 4. uke i opptil 1 år i fravær av sykdomsprogresjon eller uakseptabel toksisitet.
ARM II: Pasienter følges serielt med bildediagnostikk i 1 år.
Etter avsluttet studiebehandling følges pasientene opp etter 6 uker, hver 3. måned i 2 år, hver 6. måned i 2 år, og deretter hver 12. måned i 6 år.

Inklusjonskriterier:
- Pasienter må ha gjennomgått fullstendig kirurgisk reseksjon av stadium IB (>= 4 cm), II eller IIIA NSCLC i henhold til American Joint Committee on Cancer (AJCC) 7. utgave og ha hatt negative kirurgiske marginer
- Baseline brysttomografi (CT) må utføres innen 1 måned (30 dager) etter randomisering for å sikre at det ikke er tegn på sykdom; hvis det er klinisk indisert, må ytterligere bildediagnostiske studier utføres for å utelukke metastatisk sykdom
- Eastern Cooperative Oncology Group (ECOG) ytelsesstatus 0-1
- Pasienter må registreres til ALCHEMIST-SCREEN (ALLIANCE A151216) studien før randomisering
Ikke-plateepitel-svulster må ikke være positive for epidermal vekstfaktorreseptor (EGFR) exon 19-delesjon eller exon 21 L858R-mutasjon (sentralt som en del av ALCHEMIST-SCREEN-protokollen) og omorganisering av anaplastisk lymfomreseptor tyrosinkinase (ALK) (sentralt som en del av ALCHEMIST-SCREEN og/eller lokalt)
- MERK: hvis resultatene av den sentrale EGFR-testingen er negative, men ALK-testingen ikke var i stand til å fullføres av ALCHEMIST sentrallaboratoriet, vil ALK-statusen bli ansett som negativ (med mindre lokalt positiv for ALK-omorganisering) og pasienten kan vurderes for påmelding til EA5142, når PD-L1-resultater er mottatt og alle andre kvalifikasjonskrav er oppfylt
- Tumorer må ha PD-L1-status testet sentralt som en del av ALCHEMIST-SCREEN-protokollen
- Kvinner må ikke være gravide eller ammende på grunn av ukjente og potensielt skadelige effekter av nivolumab på fosteret eller barnet under utvikling
- Alle kvinner i fertil alder må ha en blodprøve eller urinstudie innen 2 uker før registrering for å utelukke graviditet; en kvinne i fertil alder er enhver kvinne, uavhengig av seksuell legning eller om de har gjennomgått tubal ligering, som oppfyller følgende kriterier: 1) ikke har gjennomgått en hysterektomi eller bilateral ooforektomi; eller 2) ikke har vært naturlig postmenopausal i minst 24 påfølgende måneder (dvs. har hatt mens når som helst i de foregående 24 påfølgende månedene)
- Kvinner i fertil alder og seksuelt aktive menn må anbefales på det sterkeste å bruke en akseptert og effektiv prevensjonsmetode eller å avstå fra samleie under behandlingsperioden og i 31 uker etter siste nivolumab-infusjon.
- Pasienter må IKKE ha ukontrollert interkurrent sykdom inkludert, men ikke begrenset til, alvorlig pågående eller aktiv infeksjon, symptomatisk kongestiv hjertesvikt, ustabil angina pectoris, ukontrollert hjertearytmi eller psykiatrisk sykdom/sosial situasjon som vil begrense overholdelse av studiekravene
- Ingen tidligere behandling med en immunkontrollpunkthemmer (anti-programmert celledød [PD]-1, anti-PD-L1, anti-cytotoksisk T-lymfocytt-assosiert protein 4 [CTLA4] monoklonalt antistoff)
Pasienter må ha restituert tilstrekkelig fra kirurgi og eventuell administrert kjemoterapi/strålebehandling på tidspunktet for randomisering (MERK: adjuvant kjemoterapi og/eller stråling er ikke nødvendig)
- Minimumstid mellom operasjonsdato og randomisering er 4 uker (28 dager)
Maksimal tid tillatt mellom operasjon og randomisering:
- 3 måneder (90 dager) hvis ingen kjemoterapi gis
- 8 måneder (240 dager) hvis adjuvant kjemoterapi ble gitt
- 10 måneder (300 dager) hvis adjuvant kjemoterapi og strålebehandling ble gitt
- Pasienter må ha fullført og restituert fra eventuell adjuvant kjemoterapi 2 eller flere uker før randomisering (6 uker for mitomycin og nitrosoureas; 4 uker for postoperativ strålebehandling) (MERK: adjuvant kjemoterapi og/eller stråling er ikke nødvendig)
- Serumaspartattransaminase (aspartataminotransferase [AST]) og serumalanintransaminase (alaninaminotransferase [ALT]) =< 2,5 x øvre normalgrense (innen 2 uker før randomisering)
- Total bilirubin =< 1,5 x øvre normalgrense (ULN) (unntatt hos personer med Gilbert syndrom som må ha en total bilirubin < 3,0 x ULN) (innen 2 uker før randomisering)
- Hvite blodlegemer (WBC) >= 2000/uL (innen 2 uker før randomisering)
- Nøytrofiler >= 1000/uL (innen 2 uker før randomisering)
- Blodplater >= 100 x 10^3/uL (innen 2 uker før randomisering)
- Hemoglobin >= 8 g/dL (innen 2 uker før randomisering)
- Serumkreatinin =< 2 x ULN (innen 2 uker før randomisering)
- Før randomisering må pasienter med ikke-hematologisk toksisitet fra kirurgi, kjemoterapi og strålebehandling ha kommet seg til grad =< 1 med unntak av alopecia, ototoksisitet og nevropati
- Pasienter må ikke få andre undersøkelsesmidler mot kreft under studien
- Pasienter må ikke ha kjent eller mistenkt autoimmun sykdom; personer med type I diabetes mellitus, hypotyreose som krever hormonerstatning, eller hudsykdommer som ikke krever systemisk behandling, har tillatelse til å melde seg på
- Pasienter må ikke ha en tilstand som krever systemiske kortikosteroider tilsvarende > 10 mg prednison per dag eller andre immunsuppressive medisiner innen 2 uker etter randomisering; inhalerte, intraartikulære og epidurale steroider er tillatt
- Pasienter må ikke ha kjent interstitiell lungesykdom som er symptomatisk eller kan forstyrre påvisning eller håndtering av mistenkt legemiddelrelatert lungetoksisitet
- Pasienter må ikke ha en kjent historie med humant immunsviktvirus (HIV), hepatitt B eller hepatitt C-infeksjon som er ubehandlet og/eller med en påvisbar viral belastning
- Pasienter må ikke ha en historie med allergiske reaksjoner tilskrevet forbindelser med lignende kjemisk eller biologisk sammensetning som nivolumab

Pasientene følges serielt med CT- og/eller PET/CT-avbildning i opptil 1 år og deretter under oppfølging.
Pasienter gjennomgår også blodprøvetaking under screening og oppfølging.
Pasienter kan gjennomgå en ECHO som klinisk indisert i studien.

Eksperimentell: Arm I (nivolumab)
Pasienter får nivolumab IV over 30 minutter på dag 1 i hver syklus.
Sykluser gjentas hver 4. uke i opptil 1 år i fravær av sykdomsprogresjon eller uakseptabel toksisitet.
Pasienter gjennomgår også CT og/eller PET/CT gjennom hele forsøket og blodprøvetaking under screening og oppfølging.
Pasienter kan gjennomgå en ECHO som klinisk indisert i studien.

Tidsramme: Tid fra randomisering til den tidligste hendelsen definert som tilbakefall av sykdom, eventuell ny lungekreft (selv i motsatt lunge), eller død av en hvilken som helst årsak på et kjent tidspunkt, vurdert opp til 10 år

DFS-fordelinger vil bli estimert ved bruk av Kaplan-Meier-metoden, og Cox proporsjonale faremodeller vil bli brukt for å estimere risikoforhold for behandling.
De primære sammenligningene av DFS vil bruke en logrank tester stratifisert på randomiseringsstratifiseringsfaktorene med en ensidig type I feilrate tilsvarende signifikansnivået assosiert med populasjonen som analyseres; det vil si 1,5 % for den totale befolkningen og 1 % for >= 50 % befolkningen.
Sistnevnte test vil kun bli utført i tilfelle primærtesten hos allekommere ikke er statistisk signifikant.
Andre sammenligninger av grupper vil bli gjort ved å bruke logrank-testen og Cox-modellering.
