ZDŮVODNĚNÍ: Radiační terapie využívá vysokoenergetické rentgenové záření k poškození nádorových buněk, ale může způsobit podráždění a zánět kůže. Biafinový krém může být účinný při zmírňování vedlejších účinků způsobených radiační terapií.

ÚČEL: Randomizovaná studie fáze III ke stanovení účinnosti krému Biafin při snižování vedlejších účinků radiační terapie u pacientů léčených pro rakovinu hlavy a krku.

CÍLE: I. Porovnat účinnost profylaktického krému Biafine vs. standardní nejlepší podpůrná péče při snižování radiačním zářením indukované kožní toxicity pozorované během radioterapie u pacientů s pokročilým spinocelulárním karcinomem hlavy a krku. II. Zjistěte, zda je profylaktické nebo intervenční použití krému Biafine účinnější na základě snížení maximální kožní toxicity vyplývající z radiační léčby u této populace pacientů. III. Porovnejte kvalitu života těchto pacientů s touto léčbou. IV. Posuďte toxicitu krému Biafine u těchto pacientů.

PŘEHLED: Toto je randomizovaná studie. Pacienti jsou stratifikováni podle stavu lymfatických uzlin (negativní vs. pozitivní), typu léčby (ozařování plus chemoterapie vs. samotné ozařování), radiační frakcionace (standardní vs. souběžný boost) a dávky záření (50-60 Gy vs. větší než 60 Gy). Pacienti jsou randomizováni do 1 ze 3 léčebných ramen. Rameno I: Pacienti dostávají radioterapii jednou denně 5 dní v týdnu po dobu 5-8 týdnů, s nebo bez standardní aplikace kožního krému (bez krému Biafine) na ozařovanou pokožku. Rameno II: Pacienti dostávají radioterapii jako v rameni I, s Biafinovým krémem aplikovaným na kůži 3x denně 7 dní v týdnu při zahájení radioterapie a pokračující 2 týdny po posledním ozařování. Krém Biafine se aplikuje nejméně 4 hodiny před ošetřením a nejméně 4 hodiny mezi aplikacemi. Rameno III: Pacienti dostávají radioterapii jako v ramenech I a II, přičemž krém Biafine se aplikuje jako v rameni II pouze poté, co se kůže stane symptomatickou (tj. zarudnutí, suchost, svědění nebo citlivost). Kvalita života se hodnotí před léčbou, týdně během léčby a poté jednou týdně po dobu 4 týdnů po ukončení léčby. Pacienti jsou sledováni týdně po dobu 4 týdnů.

PŘEDPOKLADANÝ AKRUÁLNÍ AKRUÁLNÍ: Do této studie bude během 17 měsíců získáno přibližně 500 pacientů.
