Zlepšení adherence k lékům u metastatického karcinomu prsu pomocí připojené přizpůsobené léčebné platformy (CONCURxP)

Tato klinická studie porovnává použití připojené přizpůsobené léčebné platformy (CONCURxP), která se skládá z používání zařízení pro monitorování léků nazvaného WiseBag spolu s textovými upozorněními na zmeškané nebo nadbytečné léky, s rozšířenou obvyklou péčí (EUC), kde pacienti používají pouze Wisebag, ke sledování adherence k léčbě u pacientek s metastatickým karcinomem prsu, které užívají inhibitor CKD4/6. Aby bylo zajištěno, že inhibitory CDK4/6 dosáhnou plného klinického přínosu, musí je pacienti užívat tak, jak je předepsáno, podle komplexního léčebného plánu. Zapomnětlivost byla nejčastější příčinou nedodržování léků. Použití WiseBag spolu s CONCURxP nebo zvýšenou obvyklou péčí může zlepšit adherenci k léčbě u pacientek s metastatickým karcinomem prsu, které užívají inhibitor CKD4/6.

PRVNÍ CÍL:

I. Porovnat adherenci inhibitorů CDK4/6 (CDK4/6i) 12 měsíců po dokončení základního průzkumu zachyceného pomocí elektronického monitorování mezi rameny EUC (Arm A) a CONCURxP (Arm B).

DRUHÉ CÍLE:

I. Porovnat adherenci k CDK4/6i 12 měsíců po dokončení základního průzkumu zachyceného prostřednictvím self-reportu mezi rameny EUC (Arm A) a CONCURxP (Arm B).

II. Porovnat přetrvávání CDK4/6i 12 měsíců po dokončení základního průzkumu zachycené pomocí elektronického monitorování mezi rameny EUC (Arm A) a CONCURxP (Arm B).

III. Porovnat symptomovou zátěž po 12 měsících mezi rameny EUC (rameno A) a CONCURxP (rameno B).

IV. Porovnat kvalitu života po 12 měsících mezi rameny EUC (rameno A) a CONCURxP (rameno B).

V. Porovnat komunikaci mezi pacientem a poskytovatelem po 12 měsících mezi rameny EUC (rameno A) a CONCURxP (rameno B).

VI. Porovnat vlastní účinnost při zvládání symptomů po 12 měsících mezi rameny EUC (rameno A) a CONCURxP (rameno B).

VII. Porovnat finanční obavy za 12 měsíců mezi rameny EUC (Arm A) a CONCURxP (Arm B).

PRŮZKUMNÉ CÍLE:

I. Posoudit dlouhodobé změny výsledků hlášených pacientem (sama hlášená adherence, zátěž symptomů, kvalita života a finanční starosti) z ramen EUC (rameno A) a CONCURxP (rameno B).

II. Porovnat využití zdravotní péče po 12 měsících mezi rameny EUC (Arm A) a CONCURxP (Arm B).

III. Porovnat přežití bez progrese po 12 měsících mezi rameny EUC (Arm A) a CONCURxP (Arm B).

IV. Porovnat celkové přežití po 12 měsících mezi rameny EUC (rameno A) a CONCURxP (rameno B).

V. Popsat zkušenosti pacientů s CONCURxP (rameno B) a jejich poskytovatelů s různými výsledky implementace.

Přehled: Pacienti jsou randomizováni do 1 ze 2 ramen. Netrpěliví účastníci jsou přiřazeni do ramene C.

ARM A: Pacienti používají dávkovač léků Wisebag a mají přístup ke vzdělávacím materiálům každé 4 týdny po dobu 12 měsíců.

ARM B: Pacienti používají dávkovač léků Wisebag a dostávají personalizované upomínky pomocí textových zpráv, sledování léků a sledování poskytovatelů zdravotní péče jako součást platformy CONCURxP po dobu 12 měsíců. Pacienti mohou absolvovat rozhovor v délce 20–30 minut do 6 měsíců od ukončení studie.

ARM C: Účastníci dokončí rozhovor v průběhu 20–30 minut 15–39 měsíců po prvním zařazení pacienta.

Po dokončení studijní intervence mohou být pacienti sledováni až 6 měsíců.

Kritéria pro zařazení:

  • NEPACIENT: Účastníci musí být poskytovatelem onkologické zdravotní péče (tj. onkologem, poskytovatelem pokročilé praxe nebo onkologickou sestrou)
  • NEPACIENT: Účastníci se museli starat alespoň o jednoho pacienta randomizovaného do ramene B (CONCURxP), který měl po 12 měsících míru adherence nižší než 85 %, jak bylo měřeno pomocí WiseBag.
  • NEPACIENT: Účastník musí mluvit anglicky
  • NEPACIENT: Účastník musí být zaměstnán v místě NCORP (National Cancer Institute Community Oncology Research Program) po dobu nejméně 6 měsíců.
  • NEPACIENT: Účastník musí být schopen poskytnout informovaný souhlas s účastí v této studii
  • PACIENT KROK 0: Pacientovi musí být >= 18 let
  • KROK 0 PACIENTA: Pacient musí plynule mluvit psané a mluvené angličtině NEBO pacient musí plynule mluvit španělsky
  • PACIENTSKÝ KROK 0: Pacient musí mít v době kroku 0 nový nebo prokázaný patologicky prokázaný hormonální receptor (HR)+ HER2- metastatický karcinom prsu

  • PACIENT KROK 0: Pacient musí zahájit léčbu některým z inhibitorů CKD4/6 (palbociclib nebo Ibrance, ribociclib nebo Kisqali, abemaciclib nebo Verzenio) do 30 dnů před souhlasem s krokem 0 nebo musí obdržet předpis na předpis s uvedeným úmyslem zahájit do 30 dní po souhlasu v kroku 0

    • POZNÁMKA: Pacienti, kteří byli dříve léčeni jinou protinádorovou léčbou než inhibitory CDK4/6, jsou způsobilí
    • POZNÁMKA: Inhibitory CDK4/6 musí být poskytnuty/dodány jako jednočinné blistrové balení. Pokud je lék dodáván jako kapsle v lahvičce na pilulky (např. kapsle Ibrance), pacient není způsobilý
    • POZNÁMKA: Ribociclib (Kisgali) a abemaciclib (Verzenio) jsou dostupné pouze v blistrech. Palbociclib (Ibrance) je jediný inhibitor CDK4/6, který může být dostupný ve formě kapslí. Toto je však zastaralá formulace a zřídka se předepisuje jako nový začátek. Formát objednaného palbociklibu lze určit na základě objednávky na předpis
  • KROK 0 PACIENTA: Pacienti nesmí být dříve léčeni žádným z následujících inhibitorů CDK4/6: Palbociclib nebo Ibrance, ribociclib nebo Kisqali a abemaciclib nebo Verzenio
  • PACIENTSKÝ KROK 0: Pacienti již nesmí být zařazeni do terapeutické klinické studie, která sleduje inhibitory CDK4/6
  • PACIENT KROK 0: Pacient musí potvrdit, že má v úmyslu získat péči nebo sledování v místě NCORP

  • PACIENT KROK 0: Pacient musí mít osobní mobilní telefon, ve kterém je schopen a ochoten posílat a přijímat textové zprávy
