Eine Studie von TAK-341 zur Behandlung von Multisystematrophie
Eine randomisierte, doppelblinde, placebokontrollierte Phase-2-Studie zur Bewertung der Wirksamkeit, Sicherheit, Verträglichkeit, Pharmakokinetik und Pharmakodynamik von intravenösem TAK-341 bei Patienten mit multipler Systematrophie
Das Hauptziel besteht darin, zu sehen, wie TAK-341 nach 52 Wochen bei Teilnehmern mit multipler Systematrophie wirkt, gemessen anhand der Unified Multiple System Atrophy Rating Scale Part I (UMSARS).
In die Studie werden etwa 138 Patienten aufgenommen. Die Teilnehmer erhalten ungefähr alle 4 Wochen insgesamt 13 intravenöse Infusionen, diese können entweder TAK-341 oder Placebo sein, nach jeder Infusion werden einige Blutproben entnommen und andere Untersuchungen durchgeführt.
Diese Studie wird in Nordamerika, Europa und Asien durchgeführt.

Detaillierte Beschreibung
Das in dieser Studie getestete Medikament heißt TAK-341. Die Studie wird die Wirksamkeit, Sicherheit, Verträglichkeit, Pharmakokinetik (PK) und Pharmakodynamik von intravenösem (IV) TAK-341 bei Teilnehmern mit multipler Systematrophie (MSA) bewerten.
An der Studie werden etwa 138 Teilnehmer teilnehmen. Die Studie umfasst einen Screening-Zeitraum von bis zu 42 Tagen (6 Wochen), einen 52-wöchigen doppelblinden Behandlungszeitraum und einen Sicherheits-Follow-up-Besuch. Die Teilnehmer werden nach dem Zufallsprinzip (zufällig, wie beim Werfen einer Münze) einem der Behandlungspläne zugewiesen, die dem Teilnehmer, dem Betreuer und dem Prüfarzt während der Studie nicht bekannt gegeben werden:
- Frühe PK-Kohorte: TAK-341 Dosis 1
- Frühe PK-Kohorte: Placebo
- Hauptkohorte: TAK-341 Dosis 2
- Hauptkohorte: Placebo
Die Veränderung von UMSARS gegenüber dem Ausgangswert wird in Woche 52 nach der Verabreichung gemessen.
Diese multizentrische Studie wird weltweit durchgeführt. Die Behandlungsdauer in dieser Studie beträgt 52 Wochen. Die Teilnehmer werden ungefähr 90 Tage nach der letzten Dosis der Studienbehandlung einen Folgebesuch am Standort machen. Teilnehmer, die vorzeitig kündigen, werden keinen weiteren Sicherheitsbesuch machen.

Einschlusskriterien:
Diagnose:
- Der Teilnehmer hat eine Diagnose einer möglichen oder wahrscheinlichen MSA unter Verwendung der modifizierten diagnostischen Kriterien von Gilman et al, 2008.
- Der Beginn der ersten MSA-Symptome des Teilnehmers trat nach Einschätzung des Prüfarztes ≤4 Jahre vor dem Screening auf.
- Nachweis von MSA-spezifischen Symptomen und Defiziten, gemessen anhand der UMSARS-Skala.
Ausschlusskriterien:
Krankengeschichte:
1. Der Teilnehmer hat eine Kontraindikation für Studienverfahren.
Diagnostische Beurteilungen:
- Vorhandensein einer verwirrenden Diagnose und/oder von Zuständen, die die Sicherheit des Teilnehmers während der Studie nach Ermessen des Prüfarztes beeinträchtigen könnten.
- Die Teilnahme des Teilnehmers an einer früheren Studie einer krankheitsmodifizierenden Therapie (mit nachgewiesenem Erhalt einer aktiven Behandlung) wird die Interpretierbarkeit der Daten aus der vorliegenden Studie nach Rücksprache mit einem medizinischen Monitor oder Beauftragten beeinträchtigen.
Andere:
1. Der Teilnehmer hat innerhalb von 6 Monaten vor dem Screening an einer anderen Studie teilgenommen, in der die aktive oder passive Immunisierung gegen α-Synuclein (αSYN) bei progressiver Erkrankung (PD) oder MSA untersucht wurde, oder hat eine Immunglobulin-G-Therapie erhalten.

Participants received TAK-341, IV Q4W for 52 weeks.
An early set of participants initially received 2400 milligrams (mg) to determine pharmacokinetic (PK) parameters.
Following the early participants, a dose of 2000 mg was given until the data from the PK participants became available.
All participants then received 2400 mg until the end of treatment at 52 weeks.

Change From Baseline in Modified Unified Multiple System Atrophy Rating Scale (UMSARS) Part I Total Score at Week 52

UMSARS Part I (historical review) is a 12-item scale that was adapted from the Unified Parkinson's Disease Rating Scale (UPDRS) and is used to assess activities related to motor disability and autonomic dysfunction.
In this study, the UMSARS was modified to exclude the sexual function item.
Thus, total 11 items were assessed.
Each item was initially scored on a scale from 0 (normal) to 4 (severe); ratings of normal (0) and mild (1) were then combined and recorded as 0, making minimum score 0 and maximum score 3. The investigator rated the average functional situation for the past 2 weeks according to findings from the participant and caregiver interview and indicated the score that best fit with the participant's status.
The total score is a sum of scores from all domains and range from 0 to 33.
Higher scores indicate worse impairment.

The SCOPA-AUT is a participant-reported outcome that assesses autonomic function.
Autonomic function is a critical symptom domain for MSA.
The scale was completed by participants and consisted of 25 items assessing the following domains: gastrointestinal (7 items), urinary (6 items), cardiovascular (3 items), thermoregulatory (4 items), pupillomotor (1 item), and sexual (2 items for men and 2 items for women).
The score for each item ranged from 0 (never experiencing the symptom) to 3 (often experiencing the symptom).
The total composite score including all domains was reported.
The score range was 0 (no symptoms) to 69 (highest burden of symptoms).
A higher score indicates worse impairment.
