Badanie TAK-341 w leczeniu zaniku wielu układów
Randomizowane, podwójnie zaślepione, kontrolowane placebo badanie fazy 2 oceniające skuteczność, bezpieczeństwo, tolerancję, farmakokinetykę i farmakodynamikę dożylnego TAK-341 u pacjentów z zanikiem wieloukładowym
Głównym celem jest sprawdzenie, jak TAK-341 działa po 52 tygodniach u uczestników z atrofią wielonarządową, mierzoną za pomocą Skali oceny zunifikowanej atrofii wieloukładowej, część I (UMSARS).
Do badania zostanie włączonych około 138 pacjentów. Uczestnicy otrzymają w sumie 13 wlewów dożylnych co około 4 tygodnie, może to być TAK-341 lub placebo, po każdym wlewie zostaną pobrane próbki krwi i przeprowadzone zostaną inne oceny.
Ta próba zostanie przeprowadzona w Ameryce Północnej, Europie i Azji.

Szczegółowy opis
Lek testowany w tym badaniu nosi nazwę TAK-341. Badanie oceni skuteczność, bezpieczeństwo, tolerancję, farmakokinetykę (PK) i farmakodynamikę dożylnego (IV) TAK-341 u uczestników z atrofią wieloukładową (MSA).
W badaniu weźmie udział około 138 uczestników. Badanie obejmuje okres przesiewowy trwający do 42 dni (6 tygodni), 52-tygodniowy okres leczenia metodą podwójnie ślepej próby oraz wizytę kontrolną. Uczestnicy zostaną losowo przydzieleni (przypadkowo, jak rzut monetą) do jednego ze schematów leczenia, który pozostanie nieujawniony uczestnikowi, opiekunowi i badaczowi podczas badania:
- Wczesna kohorta PK: TAK-341 Dawka 1
- Wczesna kohorta PK: placebo
- Kohorta główna: TAK-341 Dawka 2
- Główna kohorta: placebo
Zmiana w stosunku do wartości początkowej w skali UMSARS będzie mierzona w 52. tygodniu po podaniu dawki.
To wieloośrodkowe badanie zostanie przeprowadzone na całym świecie. Czas trwania leczenia w tym badaniu wyniesie 52 tygodnie. Uczestnicy odwiedzą ośrodek po około 90 dniach od przyjęcia ostatniej dawki badanego leku. Uczestnicy z wcześniejszym zakończeniem nie będą przeprowadzać wizyty kontrolnej w celu zapewnienia bezpieczeństwa.

Kryteria przyjęcia:
Diagnostyczny:
- Uczestnik ma rozpoznanie możliwego lub prawdopodobnego MSA przy użyciu zmodyfikowanych kryteriów diagnostycznych Gilmana i in., 2008.
- Według oceny badacza, u uczestnika wystąpiły pierwsze objawy MSA ≤4 lata przed badaniem przesiewowym.
- Dowody na objawy specyficzne dla MSA i deficyty mierzone za pomocą skali UMSARS.

Historia medyczna:
1. Uczestnik ma jakiekolwiek przeciwwskazania do studiowania procedur.
Oceny diagnostyczne:
- Obecność mylącej diagnozy i/lub warunków, które mogłyby wpłynąć na bezpieczeństwo uczestnika podczas badania zgodnie z oceną badacza.
- Udział uczestnika we wcześniejszym badaniu dotyczącym terapii modyfikującej przebieg choroby (z udowodnionym przyjmowaniem aktywnego leczenia) zagrozi możliwości interpretacji danych z obecnego badania, w ramach konsultacji z monitorem medycznym lub wyznaczoną osobą.
Inne:
1. Uczestnik brał udział w innym badaniu dotyczącym czynnej lub biernej immunizacji przeciwko α-synukleinie (αSYN) w przypadku choroby postępującej (PD) lub MSA lub przeszedł terapię immunoglobuliną G w ciągu 6 miesięcy przed badaniem przesiewowym.

Participants received TAK-341 matching placebo intravenous (IV) infusions, once every 4 weeks (Q4W) for 52 weeks.

Participants received TAK-341, IV Q4W for 52 weeks.
An early set of participants initially received 2400 milligrams (mg) to determine pharmacokinetic (PK) parameters.
Following the early participants, a dose of 2000 mg was given until the data from the PK participants became available.
All participants then received 2400 mg until the end of treatment at 52 weeks.

UMSARS Part I (historical review) is a 12-item scale that was adapted from the Unified Parkinson's Disease Rating Scale (UPDRS) and is used to assess activities related to motor disability and autonomic dysfunction.
In this study, the UMSARS was modified to exclude the sexual function item.
Thus, total 11 items were assessed.
Each item was initially scored on a scale from 0 (normal) to 4 (severe); ratings of normal (0) and mild (1) were then combined and recorded as 0, making minimum score 0 and maximum score 3. The investigator rated the average functional situation for the past 2 weeks according to findings from the participant and caregiver interview and indicated the score that best fit with the participant's status.
The total score is a sum of scores from all domains and range from 0 to 33.
Higher scores indicate worse impairment.

The SCOPA-AUT is a participant-reported outcome that assesses autonomic function.
Autonomic function is a critical symptom domain for MSA.
The scale was completed by participants and consisted of 25 items assessing the following domains: gastrointestinal (7 items), urinary (6 items), cardiovascular (3 items), thermoregulatory (4 items), pupillomotor (1 item), and sexual (2 items for men and 2 items for women).
The score for each item ranged from 0 (never experiencing the symptom) to 3 (often experiencing the symptom).
The total composite score including all domains was reported.
The score range was 0 (no symptoms) to 69 (highest burden of symptoms).
A higher score indicates worse impairment.
