Uno studio di TAK-341 nel trattamento dell'atrofia multisistemica
Uno studio di fase 2 randomizzato, in doppio cieco, controllato con placebo per valutare l'efficacia, la sicurezza, la tollerabilità, la farmacocinetica e la farmacodinamica del TAK-341 per via endovenosa in soggetti con atrofia multisistemica
L'obiettivo principale è vedere come funziona TAK-341 dopo 52 settimane nei partecipanti con atrofia multisistemica misurata dalla scala di valutazione dell'atrofia multisistemica unificata parte I (UMSARS).
Lo studio arruolerà circa 138 pazienti. I partecipanti riceveranno un totale di 13 infusioni endovenose ogni 4 settimane circa, queste possono essere di TAK-341 o placebo, dopo ogni infusione verranno prelevati alcuni prelievi di sangue e completate altre valutazioni.
Questo processo sarà condotto in Nord America, Europa e Asia.

Il farmaco in fase di test in questo studio si chiama TAK-341. Lo studio valuterà l'efficacia, la sicurezza, la tollerabilità, la farmacocinetica (PK) e la farmacodinamica del TAK-341 per via endovenosa (IV) nei partecipanti con atrofia multisistemica (MSA).
Lo studio arruolerà circa 138 partecipanti. Lo studio comprende un periodo di screening fino a 42 giorni (6 settimane), un periodo di trattamento in doppio cieco di 52 settimane e una visita di sicurezza di follow-up. I partecipanti verranno assegnati in modo casuale (per caso, come lanciare una moneta) a uno dei programmi di trattamento, che rimarrà nascosto al partecipante, all'operatore sanitario e allo sperimentatore durante lo studio:
- Coorte PK precoce: TAK-341 Dose 1
- Coorte PK precoce: Placebo
- Coorte principale: TAK-341 Dose 2
- Gruppo principale: placebo
La variazione rispetto al basale di UMSARS sarà misurata alla settimana 52 post-dose.
Questo studio multicentrico sarà condotto in tutto il mondo. La durata del trattamento in questo studio sarà di 52 settimane. I partecipanti effettueranno una visita di follow-up al sito dopo circa 90 giorni dall'ultima dose del trattamento in studio. I partecipanti con risoluzione anticipata non effettueranno una visita di sicurezza di follow-up.

Criterio di inclusione:
Diagnostico:
- Il partecipante ha una diagnosi di MSA possibile o probabile utilizzando i criteri diagnostici modificati di Gilman et al, 2008.
- L'insorgenza dei primi sintomi di MSA da parte del partecipante si è verificata ≤4 anni prima dello screening, come valutato dallo sperimentatore.
- Evidenza di sintomi e deficit specifici di MSA misurati dalla scala UMSARS.
Criteri di esclusione:
Storia medica:
1. Il partecipante ha alcuna controindicazione alle procedure di studio.
Valutazioni diagnostiche:
- Presenza di diagnosi confondenti e/o condizioni che potrebbero influenzare la sicurezza dei partecipanti durante lo studio secondo il giudizio dello sperimentatore.
- La partecipazione del partecipante a un precedente studio di una terapia modificante la malattia (con comprovato ricevimento di un trattamento attivo) comprometterà l'interpretabilità dei dati del presente studio, previa consultazione con il monitor medico o designato.
Altro:
1. Il partecipante ha partecipato a un altro studio sull'immunizzazione attiva o passiva contro l'α-sinucleina (αSYN) per malattia progressiva (PD) o MSA, o ha ricevuto una terapia con immunoglobuline G, entro 6 mesi prima dello screening.

Participants received TAK-341, IV Q4W for 52 weeks.
An early set of participants initially received 2400 milligrams (mg) to determine pharmacokinetic (PK) parameters.
Following the early participants, a dose of 2000 mg was given until the data from the PK participants became available.
All participants then received 2400 mg until the end of treatment at 52 weeks.

Change From Baseline in Modified Unified Multiple System Atrophy Rating Scale (UMSARS) Part I Total Score at Week 52

UMSARS Part I (historical review) is a 12-item scale that was adapted from the Unified Parkinson's Disease Rating Scale (UPDRS) and is used to assess activities related to motor disability and autonomic dysfunction.
In this study, the UMSARS was modified to exclude the sexual function item.
Thus, total 11 items were assessed.
Each item was initially scored on a scale from 0 (normal) to 4 (severe); ratings of normal (0) and mild (1) were then combined and recorded as 0, making minimum score 0 and maximum score 3. The investigator rated the average functional situation for the past 2 weeks according to findings from the participant and caregiver interview and indicated the score that best fit with the participant's status.
The total score is a sum of scores from all domains and range from 0 to 33.
Higher scores indicate worse impairment.

Each item was scored on a scale from 0 (normal) to 4 (severe); total score ranges from 0 to 44, higher scores indicated worse impairment.

The SCOPA-AUT is a participant-reported outcome that assesses autonomic function.
Autonomic function is a critical symptom domain for MSA.
The scale was completed by participants and consisted of 25 items assessing the following domains: gastrointestinal (7 items), urinary (6 items), cardiovascular (3 items), thermoregulatory (4 items), pupillomotor (1 item), and sexual (2 items for men and 2 items for women).
The score for each item ranged from 0 (never experiencing the symptom) to 3 (often experiencing the symptom).
The total composite score including all domains was reported.
The score range was 0 (no symptoms) to 69 (highest burden of symptoms).
A higher score indicates worse impairment.
