第一目的：

I. 4サイクルの治療以内で、いずれかのトリプレットレジメンの測定残存病変陰性（MRDneg）完全寛解（CR）の達成率を、アザシチジンおよびベネトクラクス単独と比較する。

第二の目的:

I. 治療4サイクル以内のいずれかのトリプレットレジメンのMRDneg CR/不完全なカウント回復を伴う完全寛解（CRi）/部分血液学的回復を伴う完全寛解（CRh）の達成率を、アザシチジンおよびベネトクラクス単独と比較する。

II. ギルテリチニブ、アザシチジン、ベネトクラクスの併用療法の両方がアザシチジンとベネトクラクスの併用療法よりも優れている場合に、ギルテリチニブ、アザシチジン、ベネトクラクスの併用の安全性と忍容性を判定する。

Ⅲ． 両方のトリプレットレジメンがアザシチジンとベネトクラクスレジメンよりも優れている場合に、アザシチジンおよびベネトクラクスにギルテリチニブを追加した場合の最適な投与順序と投与期間を決定するため。

IV. 完全寛解（CR）、不完全なカウント回復を伴う完全寛解（CRi）、部分的な血液学的回復を伴う完全寛解（CRh）、形態学的白血病のない状態（MLFS）、無イベント生存（EFS）、およびギルテリチニブ、アザシチジン、ベネトクラクスの併用とアザシチジンとベネトクラクスの単独投与の全生存期間（OS）。

  • 患者は60歳以上、または臨床状態（すなわち、パフォーマンスステータス、年齢＞75歳以上）に基づいて、集中的なシタラビンベースの導入療法よりもアザヌクレオシドベースの療法の方が効果があると主治医が判断した60歳以下の成人でなければならない。 ）、臓器機能不全、または疾患生物学
  • 患者は、世界保健機関（WHO）の2016年の分類に従って形態学的にAMLの診断を受けている必要がある。ただし、PML-RARAによる急性前骨髄球性白血病（APL）、RUNX1-RUNX1T1によるAML、またはCBFB-MYH11によるAMLは除く。
  • 患者は、ヒドロキシウレアおよびオールトランスレチノイン酸（ATRA）、または白血球除去療法を除いて、AML の治療歴がないことが条件となります。 この試験の治療開始前にシタラビンベースの緊急治療を受けた患者が対象となる

  • 慢性 B 型肝炎ウイルス (HBV) 感染の証拠がある患者の場合、抑制療法が必要な場合、HBV ウイルス量が検出不能でなければなりません。
  • C型肝炎ウイルス（HCV）感染歴のある患者は、治療を受けて治癒している必要があります。 現在治療中の HCV 感染患者については、HCV ウイルス量が検出不能であれば対象となります。
  • 患者は、フレデリシア補正 (QTcF) を使用してベースライン補正された QT 間隔が 480 ミリ秒以上であってはなりません
  • 患者は、強力なCYP3A4誘導薬による継続的な治療の医学的必要性を有してはなりません
  • 悪性腫瘍の既往または併発があり、自然歴や治療が治験の安全性や有効性の評価を妨げる可能性がない患者は、この試験の対象となります。

Rate of measured residual disease (MRD) negative complete remission (CR)
時間枠：Up to 4 cycles of treatment (1 cycle = 28 days)
Will be assessed by multiparameter flow cytometry at a level of ≤ 1 residual blast / 1,000 leukocytes (≤ 10^-3). Patients achieved MRD negative CR at any time up to 4 cycles will count as a responder. The MRD negative CR frequencies will be compared between each triplet regimen and the control regimen using Fisher's exact test with one-sided alpha of 0.05 for each comparison. Test results with one-sided p-value < 0.05 will be considered statistically significant.
