상세 설명
우심실 정점의 전기 자극(심실 조율)은 일반적으로 심박 조율기를 사용하는 환자의 심장 기능을 향상시키고 생명을 구할 수도 있습니다. 그러나 심실 조율이 바람직하지 않은 장기적인 심장 영향을 미칠 수 있다는 증거가 축적되고 있습니다. 따라서 심실 조율을 절대적으로 필요한 최소값으로 제한하는 것이 좋습니다. 네덜란드 Vitatron B.V. Arnhem의 C 시리즈 2세대 심박 조율기의 기능 RVP(정제된 심실 조율)는 심실 조율을 감소시키도록 설계되었습니다.
Vitatron C50 D 모델 C50A2(심박 조율기) 또는 Vitatron C60 DR 모델 C60A2(심박 조율기)를 이식하고 4~6주의 안정화 기간을 거친 후 심박 조율기 및 리드(자극 및 감지 매개변수)의 적절한 기능을 확인합니다. 심박 조율기는 사전 정의된 설정에 따라 프로그래밍됩니다.
다음 4주 동안 기본 기간 진단 데이터(심방 세동 부하 및 심실 조율 비율(% VP))가 심박조율기 메모리에 수집됩니다. 이러한 데이터를 기반으로 환자는 추가 참여에서 제외되거나(심방세동이 15% 이상인 환자) 세 그룹으로 세분됩니다: (a) < 30% VP(30-VP 그룹), (b) >30% VP, Sick Sinus Syndrome 및 정상 전도성(SSS 그룹), (c) >30% VP, 1도 또는 2도 방실 차단. 이 세 그룹의 환자는 RVP 기능을 켜거나 끈 상태에서 교대로 4주 동안 치료를 받게 됩니다. 순서는 무작위로 결정됩니다. 이 두 교차 기간이 끝날 때 %VP와 마지막 기간의 환자 판단이 평가됩니다. 부작용은 연구 등록 시점부터 기록됩니다.

- 환자는 본 연구에 대한 환자 고지 동의서에 기꺼이 서명해야 합니다.
- 환자는 심박조율기에 대한 다음 적응증 중 적어도 하나를 가지고 있어야 합니다.
- 70세 미만 환자의 경우 PR 간격이 220ms 미만이거나 70세 이상 환자의 경우 260ms 미만인 1도 방실 차단
- 2도 AV 블록, mobitz I(wenckebach) 또는 mobitz II
- 환자는 참여 기간 동안 후속 조치를 받을 수 있어야 합니다.
