Skuteczność algorytmu rozrusznika serca w promowaniu wewnętrznej aktywności serca.

Szczegółowy opis
Stymulacja elektryczna koniuszka prawej komory (stymulacja komorowa) zwykle poprawia czynność serca u pacjentów z rozrusznikiem serca, a nawet może uratować życie. Jednak gromadzą się dowody na to, że stymulacja komorowa może mieć również niepożądane długoterminowe skutki sercowe. Dlatego sensowne jest ograniczenie stymulacji komorowej do bezwzględnego wymaganego minimum. Funkcja RVP (Refined Ventricular Pacing) w rozrusznikach serca drugiej generacji serii C firmy Vitatron B.V. Arnhem w Holandii została zaprojektowana w celu ograniczenia stymulacji komorowej.
Po wszczepieniu implantu Vitatron C50 D model C50A2 (rozrusznik) lub Vitatron C60 DR model C60A2 (rozrusznik) i okresie stabilizacji trwającym 4-6 tygodni sprawdza się poprawność działania stymulatora i elektrod (parametry stymulacji i detekcji). Stymulatory zostaną zaprogramowane zgodnie z wcześniej zdefiniowanymi ustawieniami.
W ciągu następnych 4 tygodni okresu bazowego dane diagnostyczne (obciążenie migotaniem przedsionków i procent stymulacji komorowej (% VP)) są gromadzone w pamięci stymulatora. Na podstawie tych danych pacjenci zostaną wykluczeni z dalszego udziału (pacjenci z migotaniem przedsionków powyżej 15%) lub podzieleni na trzy grupy: (a) < 30% VP (grupa 30-VP), (b) >30% VP, Zespół chorego węzła zatokowego i przewodnictwo prawidłowe (grupa SSS), (c) >30% VP, blok przedsionkowo-komorowy I lub II stopnia. Pacjenci w tych trzech grupach będą leczeni przez 4 tygodnie na przemian z włączoną lub wyłączoną funkcją RVP. Kolejność zostanie ustalona w drodze losowania. Na koniec tych dwóch okresów przejściowych zostanie oceniony % VP i ocena pacjentów z ostatniego okresu. Zdarzenia niepożądane będą rejestrowane od momentu włączenia do badania.

Kryteria przyjęcia:
- Pacjenci są chętni do podpisania Świadomej Zgody Pacjenta na to badanie
- Pacjenci powinni mieć co najmniej jedno z następujących wskazań do wszczepienia stymulatora serca: - Zespół chorego węzła zatokowego z prawidłowymi zespołami QRS
- Blok przedsionkowo-komorowy pierwszego stopnia z odstępem PR <_220 ms dla pacjentów w wieku <_ 70 lat lub <_ 260 u pacjentów w wieku powyżej 70 lat
- Blok AV II stopnia, mobitz I (wenckebach) lub mobitz II
- Pacjenci będą dostępni do obserwacji przez cały okres ich uczestnictwa.
Kryteria wyłączenia:
- Pacjenci biorący udział w innym badaniu badawczym prowadzonym równolegle do tego badania
- Pacjenci w wieku poniżej 18 lat i/lub pacjenci, którzy NIE spełniają innych lokalnych wymagań dotyczących uczestnictwa
- Pacjentki w ciąży
- Pacjenci z problemami z integralnością elektrody (a elektroda nie jest wymieniana)
- Pacjenci z przetrwałym AF
- Pacjenci z całkowitym blokiem AV
- Pacjenci z klasą III i IV wg NYHA (New York Heart Association0).
- Pacjenci, którzy przeszli operację klatki piersiowej w ciągu ostatnich trzech miesięcy lub będą mieli taką operację w najbliższej przyszłości
- Pacjenci z blokiem 2:1
- Pacjenci, których oczekiwana długość życia jest krótsza niż pół roku
