La stimolazione elettrica nell'apice del ventricolo destro (stimolazione ventricolare) di solito migliora la funzione cardiaca dei pazienti portatori di pacemaker e può persino salvare la vita. Tuttavia, si stanno accumulando prove che la stimolazione ventricolare può anche avere effetti cardiaci indesiderati a lungo termine. Pertanto, ha senso limitare la stimolazione ventricolare al minimo assoluto richiesto. La funzionalità RVP (Refined Ventricular Pacing) nei pacemaker di seconda generazione della serie C di Vitatron B.V. Arnhem, Paesi Bassi è progettata per ridurre la stimolazione ventricolare.
Dopo l'impianto del Vitatron C50 D modello C50A2 (pacemaker) o Vitatron C60 DR modello C60A2 (pacemaker) e un periodo di stabilizzazione di 4-6 settimane, viene verificato il corretto funzionamento del pacemaker e degli elettrocateteri (parametri di stimolazione e sensing). I pacemaker verranno programmati in base a impostazioni predefinite.
Nelle successive 4 settimane, i dati diagnostici del periodo basale (carico di fibrillazione atriale e percentuale di stimolazione ventricolare (% VP)) vengono raccolti nella memoria del pacemaker. Sulla base di questi dati, i pazienti saranno esclusi da un'ulteriore partecipazione (pazienti con più del 15% di fibrillazione atriale) o suddivisi in tre gruppi: (a) <30% VP (gruppo 30-VP), (b) >30% VP, Sindrome del nodo del seno e conduttività normale (gruppo SSS), (c) >30% VP, blocco AV di 1o o 2o grado. I pazienti in questi tre gruppi saranno trattati per 4 settimane alternativamente con la funzionalità RVP attivata o disattivata. L'ordine sarà determinato dalla randomizzazione. Al termine di questi due periodi di cross-over verrà valutata la % VP e il giudizio dei pazienti dell'ultimo periodo. Gli eventi avversi saranno registrati dal momento dell'iscrizione allo studio.

Descrizione
Criterio di inclusione:
- I pazienti devono essere disposti a firmare il consenso informato del paziente per questo studio
- I pazienti devono avere almeno una delle seguenti indicazioni per un pacemaker: - Sindrome del seno malato con complessi QRS normali
- Blocco AV di primo grado con intervallo PR <_220 ms per pazienti di età <_ 70 anni o <_ 260 per pazienti di età superiore a 70 anni
- Blocco AV di secondo grado, mobitz I (wenckebach) o mobitz II
- I pazienti devono essere disponibili per il follow-up per tutta la durata della loro partecipazione.
Criteri di esclusione:
- Pazienti coinvolti in un altro studio di indagine condotto parallelamente a questo studio
- Pazienti di età inferiore ai 18 anni e/o pazienti che NON soddisfano altri requisiti locali per la partecipazione
- Pazienti in gravidanza
- Pazienti con problemi di integrità dell'elettrocatetere (e l'elettrocatetere non viene sostituito)
- Pazienti con fibrillazione atriale persistente
- Pazienti con blocco AV completo
- Pazienti con NYHA (New York Heart Association0 classe III e IV
- Pazienti che sono stati sottoposti a chirurgia toracica negli ultimi tre mesi o che si prevede lo faranno nel prossimo futuro
- Pazienti con un blocco 2:1
- Pazienti con un'aspettativa di vita inferiore a mezzo anno
