상세 설명
미국에서 매년 응급실(ED)에 흉통을 호소하는 대략 8-1000만 명의 환자가 있습니다. 흉통 평가의 총 비용은 연간 50억 ~ 100억 달러로 추산되지만 이러한 환자의 10%만이 궁극적으로 급성 관상 동맥 증후군으로 진단됩니다. American College of Cardiology/American Heart Association(ACC/AHA) 가이드라인에서는 급성 관상동맥 증후군의 위험이 낮은 환자가 일련의 심장 마커를 받은 후 객관적인 심장 검사(부하 검사 또는 심장 영상 촬영)를 받아야 한다고 권장합니다. 그러나 많은 사람들이 모든 저위험 환자에서 객관적인 심장 검사의 가치에 의문을 제기했습니다. 급성 관상 동맥 증후군(ACS)에 대한 저위험 환자에 대한 심장 검사는 의료 품질이나 경제적 관점에서 지속 가능하지 않습니다. 비용 증가 외에도 객관적인 심장 검사는 상당 수의 위양성 및 비진단 검사와 관련되어 불필요하고 종종 침습적인 추가 절차로 이어집니다.
불필요한 심장 검사를 제거하도록 설계된 정확한 위험 계층화 치료 경로를 구현하면 위양성/비진단 검사, 방사선 및 비용을 줄임으로써 치료의 효율성과 품질을 향상시킬 수 있습니다. 임상 결정 규칙, HEART 점수 및 두 개의 일련의 트로포닌 측정을 결합한 HEART 경로는 객관적인 테스트 없이 조기 퇴원을 위해 흉통 환자를 식별하도록 설계된 최근 개발된 치료 경로입니다.
기본 가설: HEART Pathway는 일반적인 치료와 비교할 때 환자의 안전을 유지하면서 30일의 객관적인 심장 검사, 입원 기간 및 비용을 줄일 것입니다.
방법: ACS 위험이 있는 참가자(n=282)를 Wake Forest Baptist Medical Center(WFBMC) ED에서 임상 시험에 모집합니다. 환자는 HEART Pathway 또는 일반 치료에 동등하게 무작위 배정됩니다. HEART Pathway 참가자의 치료 의사는 심장 검사를 위해 병원이나 관찰실에 입원하거나 검사 없이 집으로 퇴원할 환자를 식별하기 위해 HEART Pathway 지침을 따릅니다. Usual Care 참가자의 치료 의사는 HEART 경로를 사용하지 않고 ACC/AHA 지침에 따라 환자를 집으로 퇴원할지, 병원 또는 관찰 단위에 입원할지 여부를 결정합니다.

- 가능한 ACS와 일치하는 흉부 불편 또는 기타 증상
- 치료 의사는 심장 질환이 제외되면 환자를 집으로 퇴원시킬 수 있다고 생각합니다.
제외 기준:
- 임의의 심전도(>/= 1mV)에서 연속 리드의 새로운 ST 세그먼트 상승
- 불안정한 바이탈 징후: 등록 시 증상이 있는 저혈압(수축기 < 90mmHg), 빈맥(HR>120), 서맥(HR<40) 및 저산소혈증(실내 공기 또는 정상적인 가정 산소 흐름에서 <90% 맥박 산소 측정) 비율)
- 기대 수명이 1년 미만인 말기 진단
- 입원이 필요하거나, 객관적인 심장 검사의 위험이 증가하거나, 음성 검사 후 즉시 퇴원을 방지하기 위해 서비스 제공자가 결정한 비심장성 내과적, 외과적 또는 정신 질환.
