Wirksamkeitsbewertung des HEART Pathway bei Patienten in der Notaufnahme mit akuten Brustschmerzen (HEART Pathway)

Detaillierte Beschreibung
Etwa 8-10 Millionen Patienten, die sich über Brustschmerzen beschweren, kommen jährlich in den Vereinigten Staaten in eine Notaufnahme (ED). Die Gesamtkosten für Brustschmerzuntersuchungen werden auf jährlich 5 bis 10 Milliarden US-Dollar geschätzt, doch nur bei 10 % dieser Patienten wird letztendlich ein akutes Koronarsyndrom diagnostiziert. Die Richtlinien des American College of Cardiology/der American Heart Association (ACC/AHA) empfehlen, dass Patienten mit niedrigem Risiko für ein akutes Koronarsyndrom serielle Herzmarker erhalten sollten, gefolgt von objektiven Herztests (Belastungstests oder kardiologische Bildgebung). Viele haben jedoch den Wert objektiver Herztests bei allen Patienten mit niedrigem Risiko in Frage gestellt. Herztests für alle Patienten mit niedrigem Risiko für akutes Koronarsyndrom (ACS) sind aus qualitativer oder wirtschaftlicher Sicht der Gesundheitsversorgung nicht nachhaltig. Zusätzlich zu steigenden Kosten sind objektive Herztests mit einer beträchtlichen Anzahl falsch positiver und nicht diagnostischer Tests verbunden, die zu zusätzlichen unnötigen und oft invasiven Verfahren führen.
Die Implementierung von Versorgungspfaden zur genauen Risikostratifizierung, die darauf ausgelegt sind, unnötige Herztests zu vermeiden, könnte die Effizienz und Qualität der Versorgung verbessern, indem falsch positive/nicht diagnostische Tests, Bestrahlung und Kosten verringert werden. Der HEART-Pfad, der eine klinische Entscheidungsregel, den HEART-Score und zwei serielle Troponinmessungen kombiniert, ist ein kürzlich entwickelter Behandlungspfad, der entwickelt wurde, um Brustschmerzpatienten für eine frühzeitige Entlassung ohne objektive Tests zu identifizieren.
Haupthypothese: Der HEART Pathway wird im Vergleich zur üblichen Versorgung die 30-tägigen objektiven Herztests, die Krankenhausaufenthaltsdauer und die Kosten reduzieren und gleichzeitig die Patientensicherheit gewährleisten.
Methoden: Teilnehmer (n=282) mit ACS-Risiko werden für eine klinische Studie des Wake Forest Baptist Medical Center (WFBMC) ED rekrutiert. Die Patienten werden gleichermaßen auf HEART Pathway oder die übliche Versorgung randomisiert. Der behandelnde Arzt des HEART Pathway-Teilnehmers befolgt die HEART Pathway-Richtlinien zur Identifizierung von Patienten, die für Herztests in das Krankenhaus oder die Beobachtungseinheit aufgenommen oder ohne Tests nach Hause entlassen werden sollen. Die behandelnden Ärzte der Teilnehmer an der üblichen Versorgung entscheiden, ob die Patienten nach Hause entlassen oder auf der Grundlage der ACC/AHA-Richtlinien ohne Verwendung des HEART-Signalwegs in das Krankenhaus oder die Beobachtungseinheit aufgenommen werden sollen.

Einschlusskriterien:
- Alter größer oder gleich 21 Jahre
- Brustbeschwerden oder andere Symptome, die mit einem möglichen ACS vereinbar sind
- Der behandelnde Arzt meint, der Patient könne nach Hause entlassen werden, wenn eine Herzerkrankung ausgeschlossen würde
Ausschlusskriterien:
- Neue ST-Strecken-Hebung in zusammenhängenden Ableitungen in jedem Elektrokardiogramm (>/= 1 mV)
- Instabile Vitalzeichen: symptomatische Hypotonie zum Zeitpunkt der Einschreibung (systolisch < 90 mm Hg), Tachykardie (HF > 120), Bradykardie (HF < 40) und Hypoxämie (< 90 % Pulsoximetrie bei Raumluft oder normalem Sauerstofffluss zu Hause). Rate)
- Enddiagnose mit einer Lebenserwartung von weniger als 1 Jahr
- Eine nicht kardiale medizinische, chirurgische oder psychiatrische Erkrankung, die nach Feststellung des Anbieters eine Aufnahme erfordert, das Risiko objektiver Herztests erhöht oder eine sofortige Entlassung nach einem negativen Test verhindert.

Der HEART Pathway wird für die klinische Entscheidungsfindung in Echtzeit verwendet.
Ärzte erhalten Behandlungsempfehlungen gemäß dem HEART Pathway, basierend auf dem HEART-Score und seriellen kardialen Biomarkern.
Dies wird Ärzten helfen, Patienten mit geringem Risiko für eine vorzeitige Entlassung ohne objektive Herztests zu identifizieren.
