기본 목표:

I. 치료 전 경요도 절제술을 받은 환자에서 최종 젬시타빈 염산염(젬시타빈) 및 시스플라틴(표준 또는 용량 밀도) 화학요법 후 침습성 또는 전이성 재발이 없는 환자의 비율로 정의되는 3년 사건 없는 생존을 결정하기 위해 방광 종양(TURBT) 종양은 유해한 DDR 유전자 변형을 품고 있으며 화학 요법에 대한 < cT1 반응을 달성합니다.

2차 목표:

I. 조밀한 젬시타빈 및 시스플라틴 투여 후 유해한 DDR 유전자 변형이 있는 환자에 대한 임상 반응률(< cT1)을 결정합니다.

II. cT1 미만인 DDR 변경 환자의 방광 손상 및 전체 생존을 결정합니다.

III. < cT1에도 불구하고 근치 방광 절제술을 선택한 DDR 유전자 변형 환자의 경우, 이 환자 모집단에서 pT0 비율을 결정합니다.

IV. 이 시험에 등록된 DDR 돌연변이가 없는 환자의 방광 절제술 및 3년 무재발 및 전체 생존에서 병리학적 반응률을 결정합니다.

V. 방광보존군에서 시간 경과에 따른 국소 치료 부담(Bacillus Calmette-Guerin[BCG] 요법, 비침습성 질환의 절제, 체크포인트 봉쇄)을 평가하기 위함.

VI. DDR 돌연변이 그룹 및 DDR 야생형 그룹에서 화학방사선 요법을 받기로 선택한 환자의 무방광 무병 생존을 결정합니다.

개요:

참가자는 1일에 30분에 걸쳐 젬시타빈 염산염을 정맥 주사(IV), 1일과 2일에 시스플라틴 IV, 3일에 페그필그라스팀을 피하 주사(SC)받습니다. 치료는 질병 진행이 없거나 또는 허용되지 않는 독성. 그런 다음 참가자는 2개의 팔 중 1개에 할당됩니다.

  • 조직학적으로 확인된 방광의 근육 침윤성 요로상피암. 조직학적 근육 침범 없이 전립선 간질을 침습하는 요로상피암은 허용되며, 단 질병의 범위가 영상 및/또는 마취 하 검사(EUA)를 통해 확인됩니다. 진단용 TURBT 샘플은 등록 전 60일 이내에 획득해야 합니다.
  • 포르말린 고정 파라핀 포매(FFPE) 전처리 진단 경요도 절제술(TUR) 표본의 염색되지 않은 슬라이드(10미크론 두께) 20개(시퀀싱용), 20개 슬라이드의 시작과 끝에 2개(5미크론) 슬라이드 포함 , 총 22개의 염색되지 않은 슬라이드. FFPE 블록도 허용됩니다.
  • 임상 단계 T2-T4aN0/xM0 질병
  • 외과 의사가 평가한 근치 방광 절제술에 의학적으로 적합한 후보
  • 수반되는 다발성 상피내 암종 없음; 단일 초점이 허용됩니다
  • 방광경 평가에서 외과의가 정의한 크기가 ≤5 cm인 단일 근육 침윤성 방광 종양. 문서화되면 방광경 검사 및 TURBT의 병리학적 크기가 종양 크기의 방사선학적 측정보다 우선합니다.
  • 국소 진행성 또는 전이성 질환에 대한 임상적 또는 방사선학적 증거 없음
  • 지난 5년 이내에 이전 항-PD-1 또는 항 PD-L1 요법 또는 전신 화학요법이 없음(비근육 침습성 질환에 대한 사전 방광내 유도 면역요법은 허용되며, BCG x6 용량 및 유지 요법으로 정의됨); BCG 치료 후 3개월 이내에 질병 재발로 정의되는 BCG 불응성 질환은 허용되지 않습니다. 방광 내 화학 요법이 허용됩니다.
  • 방광이나 전립선에 대한 사전 방사선 치료 없음
  • 대수술이나 방사선 요법 없음 = 등록 후 4주 미만(TURBT 허용).
  • 임신하지 않고 수유하지 않습니다. 이 연구에는 유전독성, 돌연변이 유발 및 최기형성 효과가 있는 것으로 알려진 제제가 포함됩니다. 가임 여성의 경우에만 등록 전 14일 미만에 실시한 음성 임신 테스트가 필요합니다.
  • 동부 협력 종양학 그룹(ECOG) 수행 상태 0-1
  • 절대호중구수(ANC) >= 1,000/mm^3
