PRIMÆRE MÅL:

I. For å bestemme 3-års hendelsesfri overlevelse, definert som andelen av pasienter uten invasiv eller metastatisk tilbakefall etter definitiv gemcitabinhydroklorid (gemcitabin) og cisplatin (standard eller dosetett) kjemoterapi hos de pasientene som har transurethral reseksjon av før-behandling. blæretumor (TURBT)-svulster har skadelige DDR-genendringer og som oppnår < cT1-respons på kjemoterapi.

SEKUNDÆRE MÅL:

I. For å bestemme den kliniske responsraten (< cT1) for pasienter som har skadelige DDR-genendringer etter dosetett gemcitabin og cisplatin.

II. For å bestemme blære-intakt og total overlevelse for DDR-endrede pasienter med < cT1.

OVERSIKT:

Deltakerne får gemcitabinhydroklorid intravenøst ​​(IV) over 30 minutter på dag 1, cisplatin IV på dag 1 og 2, og pegfilgrastim subkutant (SC) på dag 3. Behandlingen gjentas hver 14. dag i opptil 6 kurer i fravær av sykdomsprogresjon eller uakseptert toksisitet. Deltakerne blir deretter tildelt 1 av 2 armer.

ARM I: Deltakere med DDR-genforandring og sykdomsstadium < cT1 gjennomgår blæresparing.

Inklusjonskriterier:

  • Trinn 1 Pasientregistrering Kvalifikasjonskriterier
  • Histologisk bekreftet muskelinvasivt urotelialt karsinom i blæren. Urothelialt karsinom som invaderer prostatastroma uten histologisk muskelinvasjon er tillatt, forutsatt at omfanget av sykdommen bekreftes via bildediagnostikk og/eller undersøkelse under anestesi (EUA). Den diagnostiske TURBT-prøven må være innhentet innen 60 dager før registrering
  • 20 ufargede objektglass (10 mikron tykkelse) av formalinfiksert parafininnstøpt (FFPE) pre-treatment diagnostic transurethral resection (TUR) prøve tilgjengelig (for sekvensering), med 2 (5 mikron) objektglass ved starten og slutten av de 20 objektglassene , for totalt 22 ufargede lysbilder. En FFPE-blokk er også akseptabel
  • Klinisk stadium T2-T4aN0/xM0 sykdom
  • Medisinsk passende kandidat for radikal cystektomi vurdert av kirurg
  • Ingen samtidig multifokalt karsinom in situ; ett enkelt fokus er tillatt
  • En enkelt muskelinvasiv blæretumor som måler ≤5 cm i størrelse som definert av kirurgene ved cystoskopisk evaluering. Når det er dokumentert, vil patologisk størrelse ved cystoskopi og TURBT ha forrang over radiografiske målinger av tumorstørrelse.
  • Ingen kliniske eller radiografiske bevis for lokalt avansert eller metastatisk sykdom
  • Ingen tidligere anti-PD-1- eller anti-PD-L1-behandlinger, eller systemisk kjemoterapi i løpet av de siste 5 årene (tidligere intravesikal induksjonsimmunterapi for ikke-muskelinvasiv sykdom er tillatt, definert som BCG x6-doser og vedlikeholdsterapi); BCG-refraktær sykdom, definert som sykdomsresidiv innen 3 måneder etter BCG-behandling, er ikke tillatt.   • Ingen tidligere strålebehandling til blæren eller prostata
  • Ingen større operasjon eller strålebehandling =< 4 uker med registrering (TURBT er tillatt).

  • Ingen tidligere malignitet bortsett fra: tilstrekkelig behandlet basal- eller plateepitelhudkreft, in situ livmorhalskreft, adekvat behandlet stadium I eller II kreft som pasienten for øyeblikket er i fullstendig remisjon fra, eller annen kreft som pasienten har vært sykdomsfri fra fem år. Pasienter med lokalisert prostatakreft som følges av et aktivt overvåkingsprogram er også kvalifisert
