연구의 1차 및 2차 목적:

주요 목표:

I. 절제 가능한 비소세포폐암 환자에 대한 수술 전후 및 보조 면역요법 기반 치료 간의 3년 실제 무사건 생존율(rwEFS)과 전체 생존율(OS)을 비교합니다(이중 평가변수).

2차 목표:

I. 두 팔의 수술적 절제율을 비교한다. II. 두 팔의 완전 절제율(R0)을 비교합니다. III. 양군 간에 영구적인 치료 중단, 입원 또는 사망을 초래하는 이상사례(AE)의 비율을 요약하고 비교합니다.

IV. 수술 전후 부문(2 부문)에 무작위 배정된 환자에서 국소적으로 정의된 병리학적 완전 반응(pCR)과 rwEFS 간의 연관성을 평가합니다.

실험적: 1군(수술, 보조요법)

수술: 환자는 등록 후 28일 이내에 수술을 받습니다.

보조 요법: 환자는 최대 4주기 동안 백금 기반 이중 화학 요법(시스플라틴, 카보플라틴, 페메트렉시드, 젬시타빈, 도세탁셀 및/또는 비노렐빈)을 받고 최대 1주기 동안 면역 체크포인트 억제제 요법(니볼루맙, 펨브롤리주맙 및/또는 아테졸리주맙)을 받습니다. 현재 승인된 지침에 따라 질병 진행이나 허용할 수 없는 독성이 없으면 1년입니다.

환자는 또한 연구 기간 내내 CT를 받고 선별검사 시 MRI 및/또는 PET/CT를 받을 수도 있습니다.

신보조요법: 등록 후 28일 이내에 환자는 백금 기반 이중 화학요법(시스플라틴, 카보플라틴, 페메트렉시드, 젬시타빈, 도세탁셀 및/또는 비노렐빈)과 면역 체크포인트 억제제(니볼루맙, 펨브롤리주맙 및/또는 또는 atezolizumab) 현재 승인된 지침에 따라.

수술: 환자는 수술을 받습니다.

보조 요법: 환자는 현재 승인된 지침에 따라 질병 진행이 없거나 허용할 수 없는 독성이 없는 경우 최대 1년 동안 면역 관문 억제제 요법(니볼루맙, 펨브롤리주맙 및/또는 아테졸리주맙)을 받습니다.

환자는 또한 연구 기간 내내 CT를 받고 선별검사 시 MRI 및/또는 PET/CT를 받을 수도 있습니다.
