Celem tego badania jest poznanie umiejętności dialektycznej terapii behawioralnej

Szkolenie dla młodzieży (DBT-A-ST) zajmuje się zaburzeniami emocjonalnymi u nastolatków. Główne pytania, na które ma odpowiedzieć, to:

Czy DBT-A-ST zapewnia szybszą poprawę (tj. bardziej strome nachylenie) w porównaniu z ASG, w zakresie pierwotnych i wtórnych pomiarów wyników podczas leczenia i 3-miesięcznej obserwacji?

Czy DBT-A-ST zapewnia większą poprawę w porównaniu z ASG, jeśli chodzi o pierwotne i wtórne pomiary wyników?

Uczestnicy będą:

Losowo przydzielany do (a) dostosowanej kulturowo DBT-A-ST lub (b) grupy wsparcia opartej na działaniach (ASG).

Otrzymano pięć ocen przed rozpoczęciem badania (T1), po 4, 8 sesjach (T2, T3), po interwencji (T4) i po 3 miesiącach obserwacji (T5).

Przegląd badań

Szczegółowy opis

Na oddziale psychiatrii dzieci i młodzieży w ośrodku medycznym zostanie przeprowadzone randomizowane, kontrolowane badanie kliniczne. Uczestnicy w wieku 12-17 lat z obecnymi zaburzeniami depresyjnymi lub lękowymi zdiagnozowanymi za pomocą K-SADS-E zostaną zrekrutowani i losowo przydzieleni w stosunku 1:1 do jednego z dwóch ramion badania: a) grupa DBT-A-ST dostosowana kulturowo, b) Grupa wsparcia oparta na działaniach. Obie interwencje mają charakter grupowy i obejmują 15 cotygodniowych sesji po 120 minut/każda sesja. Na podstawie obliczeń mocy uwzględniona zostanie docelowa wielkość próby wynosząca 160 młodych ludzi. Oceny zostaną przeprowadzone przed rozpoczęciem badania, podczas 4. i 8. sesji terapeutycznej, na końcu oraz podczas 3-miesięcznej obserwacji kontrolnej.

Podstawowymi wynikami są nasilenie depresji i lęku oceniane przez niewidomych oceniających. Drugorzędne wyniki obejmują ogólną psychopatologię, liczbę zaburzeń DSM-5, jakość życia i pomiary zgłaszane przez samych pacjentów. Zbadane zostaną również mechanizmy leczenia i procesy wdrażania.

  • łączny wynik PHQ-9 ≥ 10 lub GAD-7 ≥ 10,
  • aktualne rozpoznanie zaburzeń depresyjnych lub lękowych na podstawie wywiadu K-SADS-E, w tym depresji (F32-33); uporczywe zaburzenie nastroju (F34), zaburzenie lękowe w postaci fobii (F40), inne zaburzenie lękowe (F41) i zaburzenia adaptacyjne (F43.2).

Do badania włączane są także osoby ze współistniejącymi diagnozami kilku zaburzeń emocjonalnych. Choroby współistniejące nie będą stratyfikowane, ponieważ znacznie zwiększy to złożoność projektu (tj. niektórzy pacjenci mogą mieć wiele chorób współistniejących).

  • ADHD bez podwyższonych obszarów w obszarach lęku, depresji lub problemów ze stresem w CBCL,
  • niedawna (w ciągu ostatnich 4 tygodni) zmiana leków psychiatrycznych lub niechęć do utrzymywania stałej dawki leku podczas udziału w badaniu,
  • przeszedłeś w przeszłości pięć lub więcej sesji terapii poznawczo-behawioralnej lub nie chcesz zaprzestać psychoterapii równoległej,
  • każdy stan psychiczny lub fizyczny wymagający hospitalizacji,
  • ciężki stan chorobowy.
