PACR 在任何连接互联网的计算机上的 Web 浏览器中运行,并以引人入胜的类似游戏的格式实施。 参与者选择当天安排的一项认知练习,并进行十五分钟的练习。 练习本身包含核心科学刺激和任务,内置于类似游戏的体验中。 参与者在 15 分钟的课程中进行数十到数百次试验,每次试验都提供听觉和视觉反馈和奖励,以表明试验是否正确执行。 每次试验后,都会更新下一次试验的难度,以确保在一个会话中,参与者能够正确完成约 85% 的试验。 因此,培训是针对个人量身定制的,以最大限度地提高其有效性。 包括游戏指标(点数、级别)和锻炼指标(使用情况、进度)的摘要屏幕会在每个会话结束时显示给参与者。 调度机制确保患者按照定义的顺序进行练习,通常在 8 周的体验过程中从更简单的(早期感觉处理)练习转移到更复杂的(多模式、认知控制)练习。 在任何时间点,参与者只能访问这些练习的一个子集(通常是六个),其中每天执行四个。
每个练习都有特定的完成标准,在满足这些标准后,该练习将从活动集中删除,并添加下一个练习。 这种机制确保了参与者持续的新鲜感和参与度,并确保参与者在程序使用期间顺利完成整套练习。
将免费提供 PACR 计划,并将以法语和英语开发量身定制的认知培训计划,专门针对受 HIV 影响最大的领域和机制。 这将包括选择最合适的培训模块(计划 12 个),以及优化基于 Web 的演示和反馈以确保该目标群体的可接受性。
治疗目标是在 30 分钟的疗程中使用分配的程序,每周 5 次,随机化后持续 8 周;计划使用将是在家(或社区互联网资源)或诊所会议的任意组合。
结果将是 B-CAM 上的响应者状态(定义为 >0.5 logits 的改善)。 假设结果是从二项式分布中得出的,在没有干预的情况下,预期反应概率为 10% (n=3),如果 7或多人响应。 在两组中观察到的反应将提供更准确的估计,以计划将这项工作扩大到全面试验。 探索性分析将仅评估那些完成至少 60% 培训课程的人的反应,认识到这里的力量会降低,但信息仍然很重要。
来自两组的参与者也将与所有有资格随机化到整个平台干预的人进行比较。 广义估计方程 (GEE) 将作为次要的、更通用的方法在这里应用,允许对其他时间点进行建模,并考虑其他结果。 这适用于二元(响应者状态)或连续(认知测试得分)结果。 该分析使用回归模型,但控制了一段时间内结果的聚类。 对于二元结果,组(直接或对照)的影响表示为比值比;对于连续结果,该参数是相当于调整后的配对 t 检验的效果大小。 交互项测试效果是否因组而异(即假设的直接训练组中效果更大)。
将使用额外的分析来解释 B-CAM 评分的变化是干预预期变化的函数。 由于干预队列很小,我们将使用一致性参数而不是回归模型来量化干预运作的假设机制的变化与结果(认知能力)的变化(在个体层面)一致的程度).
